Cirrhosis begins with repeated liver injury, which triggers three major processes: inflammation, immune system activation, and cell death.
Together, these processes drive progressive liver damage.
Inflammation attracts immune cells to the liver, prompting the release of inflammatory cytokines, such as tumor necrosis factor-alpha and interleukin-1.
These cytokines stimulate the release of transforming growth factor-beta, the main driver of fibrosis.
Immune system activation triggers hepatic stellate cells to become activated and produce excess collagen and extracellular matrix proteins.
Continuous activation leads to the accumulation of dense fibrous bands within liver tissue, distorting its normal architecture and impeding blood flow.
Hepatocyte apoptosis and necrosis occur alongside these changes, triggering disorganized regeneration as fibrosis progresses.
This disordered regeneration results in nodules of liver cells surrounded by dense fibrotic tissue.
The resulting distortion increases resistance to portal blood flow, causing portal hypertension.
