Macrofagen in systemische lupus erythematosus: een bibliometrische analyse van onderzoekstrends en opkomende grenzen

Deze studie schetst systematisch het onderzoekslandschap en de zich ontwikkelende grenzen van het vakgebied systemische lupus erythematodes en macrofagen van 2016 tot 2025 met behulp van bibliometrische analyse.

Samenvatting

Deze studie analyseerde systematisch 1.197 artikelen over de rol van macrofagen in systemische lupus erythematosus (SLE), gepubliceerd tussen 2016 en 2025, met behulp van bibliometrische methoden gebaseerd op de Web of Science Core Collection. De analyse onderzocht publicatietrends, samenwerkingsnetwerken, onderzoeksthema's en de kennisbasis om het huidige onderzoekslandschap, kernonderzoekskrachten en grensontwikkeling in dit vakgebied te verduidelijken. Het aantal publicaties piekte op 154 in 2022, terwijl de jaarlijkse citaties in totaal stegen van 184 in 2016 tot 9.030 in 2025, wat wijst op actief en impactvol onderzoek. China stond op de eerste plaats in publicatievolume (379 artikelen), terwijl de Verenigde Staten vooraanvoerden in totaal aantal citaties (18.838) en internationale samenwerkingscentraliteit. Shanghai Jiao Tong Universiteit en Karolinska Instituut waren de meest productieve instellingen, elk met 23 artikelen. Frontiers in Immunology (impactfactor = 5,9; Journal Citation Reports kwartiel 1) was het meest productieve tijdschrift. Keyword timeline clustering identificeerde 13 belangrijke onderzoeksthema's, waaronder belangrijke componenten van de aangeboren immuniteit zoals neutrofiele extracellulaire vallen, dendritische cellen en Toll-achtige receptoren, evenals adaptieve immuniteit met T-cellen, B-cellen en plasmacytoïde dendritische cellen. Deze thema's omvatten ook kernmechanismen zoals macrofagenpolarisatie, de NOD-achtige receptorfamilie pyrine-bevattende 3-inflammasoom (NLRP3) en het verwijderen van apoptotische cellen, samen met klinische hotspots zoals lupusnefritis, macrofagenactivatiesyndroom en reumatoïde artritis. De evolutie van onderzoekshotspots weerspiegelt een ontwikkeling van vroege basismechanismen van apoptotische cellen en immuuncomplexen tot verfijnde regulatie van cytokines, aangeboren immuniteit en adaptieve immuniteit, en uiteindelijk naar studies naar therapieën gericht op signaalroutes, klinisch beheer en multicentervalidatie, waarbij "pathway" en "multicenter diagnose" opvallen als huidige onderzoeksfronten.

Inleiding

Systemische lupus erythematosus (SLE) is een chronische auto-immuunziekte die wordt gekenmerkt door de productie van autoantistoffen, afzetting van immuuncomplexen en multi-orgaanontsteking 1,2. Ondanks aanzienlijke vooruitgang in diagnostische benaderingen en therapeutische strategieën in de afgelopen decennia, blijft SLE ernstige schade veroorzaken aan de nieren, het hematologische systeem en het centrale zenuwstelsel 3,4,5. Bestaande behandelingen, waaronder glucocorticoïden, immunosuppressiva en biologische middelen, hebben over het algemeen beperkte therapeutische effectiviteit en worden geassocieerd met bijwerking in verschillende gradaties 6,7,8. Celengineeringstrategieën, zoals chimerische antigeenreceptor T (CAR-T) celtherapie, hebben aangetoond dat ze het potentieel hebben om duurzame geneesmiddelvrije remissie in SLE te bereiken door diepe uitputting van autoreactieve B-cellen 9,10,11. Als sleutelleden van het aangeboren immuunsysteem vervullen macrofagen cruciale functies bij het opruimen van immuuncomplexen, efferocytose van apoptotische cellen en cytokinesecretie, waardoor de immuunhomeostasebehouden blijft. Steeds meer bewijs wijst erop dat meerdere vormen van macrofagendisfunctie betrokken zijn bij SLE-pathogenese 13,14,15. Ontregelde macrofagenpolarisatie in pro-inflammatoire M1- versus anti-inflammatoire M2-fenotypes is consequent waargenomen bij lupusnefritis, waarbij M1-macrofagen weefselschade bevorderen terwijl M2-macrofagen bijdragen aan weefselherstel en fibrotische remodellering16,17. Overmatige vorming van neutrofiele extracellulaire vallen (NET's) en abnormale activatie van interferonsignaalroutes versterken verder macrofaag-gemedieerde ontstekingsreacties 18,19,20. Daardoor is macrofagendisfunctie niet alleen een centrale knoop in de SLE-pathogenese geworden, maar ook een veelbelovend therapeutisch doelwit21.

Het afgelopen decennium heeft een snelle uitbreiding gekend van publicaties die zich richten op de rol van macrofagen in SLE, met uiteenlopende onderzoeksrichtingen, waaronder efferocytose, NETosis, macrofagenpolarisatie, metabole herprogrammering en gerichte modulatie van signaalroutes 22,23,24,25,26 . De groeiende literatuur brengt echter ook uitdagingen met zich mee, waaronder gefragmenteerde onderzoeksthema's, onduidelijke identificatie van kernonderzoekskrachten en een dubbelzinnige mapping van grensevolutie, wat de dringende noodzaak van een systematisch overzicht van het huidige onderzoekslandschap onderstreept. Bibliometrie maakt gebruik van wiskundige en statistische methoden om de distributiekenmerken en evolutionaire patronen van kennisdragers kwantitatief te analyseren, en levert objectieve gegevens over publicatietrends, samenwerkingsnetwerken, onderzoekshotspots en grensdynamiek 27,28,29,30,31 . Hoewel bibliometrische benaderingen met succes zijn toegepast op kennismapping in SLE-immunotherapie, biomarkerontdekking en aanverwante vakgebieden, blijft bibliometrische mapping die specifiek gericht is op macrofagen in SLE beperkt 32,33,34. Deze studie haalt relevante literatuur op uit de Web of Science Core Collection-database van 2016 tot 2025 en maakt gebruik van CiteSpace, VOSviewer en Scimago Graphica voor multidimensionale visuele analyses. Door publicatie-outputs, land/regio- en institutionele samenwerkingen, auteurs- en tijdschriftnetwerken, keyword co-occurrence en tijdlijnclustering, evenals citation burst-detectie, wil deze studie systematisch de huidige onderzoeksstatus, kernonderzoekskrachten, thematische evolutie en opkomende grenzen op het gebied van macrofagen in SLE verduidelijken, waardoor een objectieve kenniskaart en richtingsreferentie voor toekomstige onderzoeken worden geboden.

Protocol

Voor deze studie was geen ethische goedkeuring of geïnformeerde toestemming vereist, aangezien alle gegevens afkomstig waren van openbaar beschikbare bibliografische records in de Web of Science Core Collection-database. Er waren geen menselijke deelnemers, dieren of klinische monsters betrokken bij dit onderzoek.

Gegevensverzameling en zoekstrategie

Gegevens werden op één datum (27 april 2026) opgehaald uit de Web of Science Core Collection (WOSCC) database om dataconsistentie en reproduceerbaarheid te waarborgen. De zoekopdracht gebruikte Booleaanse operatoren met de volgende structuur: TS=("Lupus Erythematosus, Systemisch" OF "Systemisch Lupus Erythematosus" OF "Lupus Erythematosus Disseminatus") EN TS=("macrofaag*"). Documenttypes waren beperkt tot artikelen en overzichtsartikelen. De publicatieperiode liep van 1 januari 2016 tot 31 december 2025. In totaal werden 1.197 geldige artikelen behouden voor latere bibliometrische analyse.

Analytische hulpmiddelen

Er werd een analytische pijplijn gebruikt die meerdere softwarepakketten omvatte. CiteSpace (Research Resource Identifier [RRID]: SCR_020932) werd gebruikt voor clustering van zoekwoordtijdlijnen en citatieburstanalyse35. VOSviewer (RRID: SCR_016598) werd gebruikt om coauteurschapsnetwerken te construeren en te visualiseren voor landen/regio's, instellingen en auteurs, en om trefwoordco-occurrencekaartente genereren 36. Scimago Graphica werd gebruikt om landen/regio samenwerkingsnetwerkvisualisaties te genereren uit netwerkbestanden die werden geëxporteerd vanuit VOSviewer37. Basisstatistische berekeningen en het maken van figuren werden uitgevoerd met WPS Excel en GraphPad Prism (RRID: SCR_002798). De algehele bibliometrische workflow wordt geïllustreerd in Figuur 1.

Parameterconfiguratie in CiteSpace en VOSviewer

In CiteSpace werd de tijdsperiode vastgesteld van 2016 tot 2025 met intervallen van één jaar. De g-index (k = 10) werd gekozen als de drempel voor knoopkeuze. Het Pathfinder snoeialgoritme werd twee keer toegepast: eerst om elk individueel tijdslice-netwerk te vereenvoudigen, en opnieuw na het samenvoegen van alle slices om de visuele complexiteit te verminderen. In VOSviewer werd het LinLog/modulariteitsalgoritme gekozen voor netwerkindelingsoptimalisatie. Knooppuntgroottes werden gewogen op basis van het aantal documenten of citatiefrequenties, afhankelijk van de specifieke analyse. Voor land/regio samenwerkingsmapping met Scimago Graphica weerspiegelden knoopdiameters publicatievolumes en vertegenwoordigden linkdikte samenwerkingsintensiteiten tussen landen.

Resultaten

Jaarlijkse publicatie

Tussen 2016 en 2025 werden in totaal 1.197 artikelen over macrofagen in SLE gepubliceerd. Het jaarlijkse publicatievolume fluctueerde, beginnend bij 112 in 2016, piekte op 154 in 2022, en daalde vervolgens licht tot 133 in 2025. In dezelfde periode steeg het jaarlijkse citatiecijfer in totaal van 184 naar 9.030, met kleine schommelingen, wat wijst op een geleidelijke toename van de academische impact in dit vakgebied (

Discussie

Deze bibliometrische studie schetst systematisch het onderzoekslandschap van macrofagen in SLE over het afgelopen decennium (2016–2025). Door 1.197 publicaties uit de Web of Science Core Collection te analyseren, identificeerden we wereldwijde publicatietrends, samenwerkingsnetwerken, kernonderzoekskrachten, thematische evolutie en kennisbasisdynamiek, waarmee we een objectieve routekaart boden voor toekomstig onderzoek in dit snel evoluerende vakgebied.
