In de huidige studie hebben we een stapsgewijze aanpak voor de WJ-celafgifteprocedure gedemonstreerd en gepresenteerd en een vervolg van kwantitatief onderzoek gebruikt om het effect van WJ-afgifte op cellulaire kenmerken te beoordelen: cellulaire levensvatbaarheid en biomechanische kenmerken (d.w.z. EM). Na WJ-injectie was 85,9% van de geoogste cellen levensvatbaar. In termen van WN-injectie behield 97,2% van de cellen hun levensvatbaarheid na injectie. Urine-incontinentie (UI) is een wijdverspreide aandoening met een prevalentie van 1,8 - 30,5% in Europese populaties1 en wordt voornamelijk gekenmerkt door slecht functioneren van de urethrale sluitspier. Vanuit een klinisch perspectief wordt chirurgische behandeling vaak aangeboden aan patiënten wanneer conservatieve therapieën of fysiotherapie er niet in slagen om de opkomende symptomen aan te pakken en te verlichten.
Celtherapie voor het potentiële regeneratieve herstel van de sfinctercomplexstoring is in opkomst als een avant-gardistische benadering voor de behandeling van UI-pathologie 2,3. De belangrijkste doelen zijn het vervangen, repareren en herstellen van de biologische functionaliteit van het beschadigde weefsel. In diermodellen voor UI heeft stamceltransplantatie veelbelovende resultaten laten zien in urodynamische uitkomsten 2,4,5. Stamcellen ontstaan als optimale cellulaire kandidaten omdat ze het vermogen hebben om zelfvernieuwing en multipotente differentiatie te ondergaan, waardoor de aangetaste weefselregeneratiewordt bevorderd 6. Ondanks het aanstaande regeneratieve potentieel, blijft het praktische gebruik van celtherapie belemmerd, aangezien minimaal invasieve toediening van cellen nog steeds voor verschillende uitdagingen staat met betrekking tot de injectieprecisie en dekking van het doelwit. Hoewel de huidige aanpak die wordt gebruikt voor celafgifte injectie via een naald-spuitsysteem7 is, resulteert dit meestal in een algemeen tekort aan levensvatbare cellen, met gerapporteerde levensvatbaarheden zo laag als 1% - 31% na transplantatie8. Bovendien is aangetoond dat celafgifte via naaldinjectie ook van invloed is op de plaatsing, de retentiesnelheid en de distributie van getransplanteerde cellen in het beoogde weefsel 9,10,11. Een haalbare, nieuwe aanpak die de bovengenoemde beperking overwint, is de naaldvrije celafgifte via waterstraaltechnologie.
Waterjet (WJ) technologie is in opkomst als een nieuwe aanpak die een hoge doorvoer levering van cellen door cystoscoop onder visuele controle in de urethrale sluitspier 12,13 mogelijk maakt. De WJ maakt celafgifte mogelijk bij verschillende drukken (E = effecten in bar) variërend van E5 tot E8013. In de eerste fase (weefselpenetratiefase) wordt isotone oplossing toegepast met hoge druk (d.w.z. E60 of E80) om de extracellulaire matrix rond het beoogde weefsel los te maken en kleine onderling verbonden microlacunes te openen. In de tweede fase (de injectiefase) wordt de druk binnen milliseconden verlaagd (d.w.z. tot E10) om de cellen voorzichtig in het beoogde weefsel af te leveren. Na deze toepassing in twee stappen worden de cellen niet blootgesteld aan extra druk tegen het weefsel wanneer ze worden uitgeworpen, maar zweven ze in een lagedrukstroom in een met vloeistof gevuld caverneus gebied13. In een ex vivo modelsetting waarbij stamcellen via WJ in cadaveric urethraweefsel werden geïnjecteerd, konden levensvatbare cellen daarna worden geaspireerd en uit het weefsel worden gehaald en in vitro verder worden uitgebreid 13. Hoewel een studie uit 2020 door Weber et al. de haalbaarheid en toepasbaarheid van WJ aantoonde om voetafdrukvrije cardiomyocyten in hetmyocardium 14 te leveren, moet er rekening mee worden gehouden dat de WJ-technologie zich nog in een prototypefase bevindt.
Het volgende protocol beschrijft hoe varkens vetweefsel-afgeleide stromale cellen (pADSC) kunnen worden voorbereid en gelabeld en hoe ze kunnen worden afgeleverd in vanvangvloeistof en kadaverweefsel via WJ-technologie en Williams cystoscopienaalden (WN). Na cellulaire injectie wordt de cellulaire vitaliteit en elasticiteit via atomaire krachtmicroscopie (AFM) beoordeeld. Via stapsgewijze instructies geeft het protocol een duidelijke en beknopte aanpak om betrouwbare gegevens te verkrijgen. Het discussiegedeelte presenteert en beschrijft de belangrijkste voordelen, beperkingen en toekomstperspectieven van de techniek. De WJ-levering van cellen en de sequela-posttransformatieanalyses die hier worden gerapporteerd, vervangen de standaard naaldinjectie en bieden een solide celafgiftekader voor regeneratieve genezing van het doelweefsel. In onze recente studies leverden we bewijs dat WJ cellen nauwkeuriger en ten minste met een vergelijkbare levensvatbaarheid afleverde in vergelijking met naaldinjecties15,16.

Injection of Porcine Adipose Tissue-Derived Stroma Cells via Waterjet Technology

Urine-incontinentie (UI) is een veel voorkomende aandoening die wordt gekenmerkt door het tekort aan de urethrale sluitspier. Regeneratieve geneeskunde takken, met name celtherapie, zijn nieuwe benaderingen om de urethrale sluitspierfunctie te verbeteren en te herstellen. Hoewel injectie van actieve functionele cellen routinematig wordt uitgevoerd in klinische omgevingen met naald en spuit, hebben deze benaderingen aanzienlijke nadelen en beperkingen. In deze context is naaldvrije waterstraaltechnologie (WJ) een haalbare en innovatieve methode die levensvatbare cellen kan injecteren door visueel geleide cystoscopie in de urethrale sluitspier. In de huidige studie gebruikten we WJ om van varkensweefsel afgeleide stromale cellen (pADSCs) af te leveren in cadaveric urethraal weefsel en onderzochten vervolgens het effect van WJ-afgifte op celopbrengst en levensvatbaarheid. We beoordeelden ook de biomechanische kenmerken (d.w.z. elasticiteit) door atomaire krachtmicroscopie (AFM) metingen. We toonden aan dat door WJ geleverde pADSC's significant verminderd waren in hun cellulaire elasticiteit. De levensvatbaarheid was aanzienlijk lager in vergelijking met controles, maar ligt nog steeds boven de 80%.

Na celafgifte via de twee benaderingen was de levensvatbaarheid van cellen die via het WN werden toegediend (97,2 ± 2%, n = 10, p<0,002) hoger in vergelijking met injecties door WJ met behulp van de E60-10-instellingen (85,9 ± 0,16%, n = 12) (figuur 2). Waterstraalinjecties van actieve componenten, zoals vloeistoffen, nanodeeltjes, microdeeltjes, ingekapselde geneesmiddelen of zelfs levende cellen kunnen veel nauwkeuriger worden toegepast in vergelijking met naaldinjecties in verschillende beoogde weefsels. Minimaal invasieve benaderingen vereisen nauwkeurige lokalisatie van de medicatie in vooraf bepaalde lagen van het beoogde weefsel door de injectiedrukken vooraf te selecteren.
