We onderzoeken hoe diepe hersenstimulatie van de globus pallidus de netwerkactiviteit van het cerebellum verandert en op deze manier de neuronale functie bij dystonie herstelt. De high-density microelectrode arrays maken gelijktijdige registratie van motorische luscircuits mogelijk om de netwerkbrede effecten van diepe hersenstimulatie bij bewegingsstoornissen zoals dystonie te onderzoeken. Begin hiermee door de elektronica van het STELLA Stimulation System te onderbrengen in een op maat gemaakte, 3D-geprinte capsule van polyethyleentereftalaatglycol of PETG.Gebruik silicone om het gebied tussen het deksel en de capsule en de behuizing bij de kabeluitgang af te dichten. Controleer na het drogen de silicone op zichtbare openingen en herhaal de afdichting als er openingen aanwezig zijn. Na volledige uitharding van de silicone wordt de epoxyhars voorbereid met een mengverhouding van 100:35 op basis van gewicht.Gebruik de Vevor RS-1.5 om luchtbellen uit de hars te verwijderen onder vacuüm. Breng vóór montage een dunne laag Vaseline aan op het contactoppervlak van de op maat gemaakte siliconenvorm. Plaats de stimulatorcapsule in de vorm.Vul het langzaam met epoxyhars totdat de stimulator en kabelverbindingen volledig zijn bedekt, waarbij luchtbellen tijdens het vullen worden vermeden. Verwijder de capsule na uitharding van de hars uit de mal. Gebruik een fijne vijl of polijstinstrument om het epoxy-oppervlak glad te maken en scherpe randen te verwijderen.Dompel de capsule onder in vloeibare siliconen om een laatste dunne siliconenlaag aan te brengen. Gebruik een plastic pipet om extra siliconen aan te brengen rond de kabel nabij het verbindingsgebied. Gebruik een stokje om overtollige siliconen te verwijderen vóór het uitharden om een te grote dikte van de coating te voorkomen.Om een functionaliteitstest na de inkapseling uit te voeren vóór chirurgische implantatie of experimenteel gebruik, plaatst u het STELLA-stimulatiesysteem in Ringer-oplossing en activeert u de ingebouwde magnetische zelftestfunctie. Bevestig het juiste knipperpatroon van 2 bij 2 van de lichtemitterende diode of LED, wat duidt op een correcte stimulatie-output en het uitsluiten van kortsluitingen, open circuits of een lage batterijstatus. Schakel het apparaat na het testen uit om batterij te besparen.Bereid de werkplek voor met dissectie-instrumenten, sucrose-oplossing, Petrischalen en de opstelling van de vibratoom. Voeg ijskoude, slush-achtige sucrose-oplossing toe aan een Petrischaal met filterpapier op de bodem en handhaaf continue carbogenatie voor de extractie en het snijden van de hersenen. Verplaats de hersenen, die binnen 15 minuten na de 11-daagse periode van diepe hersenstimulatie (deep brain stimulation of DBS) zijn uitgenomen, naar de voorbereide Petrischaal met ijskoude, slush-achtige sucrose.Gebruik een scalpel om het cerebellum te verwijderen door een corneaal snijvlak door de pons te maken. Verwijder vervolgens de cerebellaire hemisfeer via een parasagittale snede. Bereid de vibratoomplaat voor met een druppel secondelijm.Verwijder het cerebellum voorzichtig uit de Petrischaal. Dep het parasagittale sectievlak op filterpapier om het te drogen voordat deze zijde wordt vastgelijmd. Plaats de vibratoomplaat in het bakje.Vul de tray met voldoende half-gesuspendeerde sucrose-oplossing om het cerebellum volledig onder te dompelen. Doorgas de tray met carbogeengas. Snijd parasagittale cerebellaire coupes met een dikte van 200 micrometers bij een constante snelheid van 0,08 millimeter per seconde.Bewaar de coupes in kunstmatig cerebrospinaal vocht of ACSF in een bekerglas dat constant wordt doorstroomd met carbogen bij kamertemperatuur. Laat de coupes ongeveer 40 minuten herstellen in gekarbonateerde ACSF bij kamertemperatuur voordat ze worden overgebracht naar de high-density microelectrode array of HD-MEA voor opwarming en stabilisatie voorafgaand aan de registratie. Reinig de vergulde contactpunten met 100% ethanol en de elektrode-array met 70% ethanol en een zachte borstel.Spoel vervolgens de elektrode-array af met Milli-Q water. Plaats de CorePlate single well in de opname-opstelling voordat de weefselcoupes worden aangebracht. Sluit de HD-MEA met CorePlate TM single well aan op het opnamesysteem.Pipette ongeveer 2 milliliters ACSF zonder luchtbellen in het well. Start de live acquisitie in de BrainWave 5 of BW5 software en stel de parameters in, zoals de Cockpit Mode en het Tissue Model. Verifieer dat de achtergrondruis binnen het optimale bereik van 9-20 microvolts valt.Neem gedurende ten minste 30 seconden op. Open na het stoppen van de opname het opgeslagen bestand voor analyse. Om de ruisinterface te analyseren, klikt u op Results en selecteert u Playback.Klik op Media Export en selecteer in het vervolgkeuzemenu Raw Traces de optie Activity Map om de piek-tot-piekamplitude en standaarddeviatie te analyseren ter bepaling van de basislijnruis. Gebruik vervolgens een Pasteurpipet om de weefselsnijde onder een microscoop in het midden van de enkele put van de CorePlate te plaatsen en verwijder de ACSF. Plaats het anker bovenop de snijde om deze te fixeren.Gebruik een Pasteurpipet van 3 milliliter om een enkele druppel ACSF aan te brengen en te verwijderen om het sneetje te wassen, en herhaal dit 5-10 keer. Gebruik een gedraaid papieren lontje om de resterende ACSF te verwijderen. Breng druppels ACSF aan in het midden van de well om het contact tussen het sneetje en de elektroden te verbeteren voor een optimale registratie.Positioneer de inlooplijn boven de slice en de uitlooplijn ervan af. Verwijder alle luchtbellen van de randelektrode voordat u overgaat tot gegevensacquisitie en analyse. De analyse van het totaal aantal spikes in de moleculaire Purkinjecellaire en granulaire cellagen van de cerebellaire cortex toonde aan dat dystone weefselslices significant minder spikes genereerden, terwijl pallidale DBS het aantal spikes verhoogde tot niveaus die vergelijkbaar waren met die van gezonde controles.De Kruskal-Wallis H-toets bevestigde verder een statistisch significante toename van het aantal spikes in de moleculaire laag van de cerebellaire cortex voor de dystone DBS-groep vergeleken met zowel de onbehandelde dystone groep als de dystone sham-groep. De dystone DBS-groep vertoonde daarnaast een toename van spikes in de Purkinje-cellaag en de korrelcellaag, waarbij waarden werden bereikt die vergelijkbaar waren met die van de controlegroep. Het handhaven van een stabiele HD-MEA-registratie tijdens alle metingen, met een juiste aarding, goed behouden elektrodecontact en experimentele omstandigheden met weinig ruis, is van cruciaal belang.Dit protocol maakt hoogresolutiemetingen mogelijk van de activiteit in het cerebellaire netwerk en de effecten van de modulatie daarvan door diepe hersenstimulatie.
