Abdominal Exam II: Percussion

1. Ogólna afekt brzucha

Abdominal percussion is a physical exam technique that uses tapping to find differences in sound. The pitch changes because the body wall vibrates differently depending on the organs underneath, the strength of the tap, and the condition of the body wall. Doctors use this method to help judge organ size, find masses, and look for fluid or areas of tissue consolidation.

Three main percussion sounds are heard in medical exam. Resonance is heard over the lungs. Tympany is heard over air-filled bowel loops, and dullness is heard over fluid or solid organs. The contrast between dullness and the more hollow sounds helps the examiner define organ margins and detect abnormal areas.

During an abdominal exam, percussion comes after visual inspection and auscultation, which means listening with a stethoscope. The examiner first percusses the nine abdominal regions: epigastric, right hypochondriac, left hypochondriac, umbilical, right lumbar, left lumbar, hypogastric, right inguinal, and left inguinal. Tenderness with percussion is not normal and can suggest peritoneal inflammation.

Most of the abdomen normally gives a tympanic sound because air-filled bowel loops lie close to the abdominal wall. Large areas of dullness should raise concern for organ enlargement, intra-abdominal masses, or fluid. The exam should also estimate liver and spleen size by percussion, including over the lower anterior chest. Normally, dullness is heard over the right anterior chest where the liver lies, while the left anterior chest is usually tympanic over the gastric air bubble and the splenic flexure of the colon.
