Phenylbutazone Warfarin

The phenylbutazone-warfarin interaction is a clinically important drug-drug interaction in which an anti-inflammatory medicine can intensify the anticoagulant effects of warfarin and increase bleeding risk. Phenylbutazone may displace warfarin from plasma proteins, inhibit its hepatic metabolism, and add gastrointestinal and platelet-related effects that promote hemorrhage. In clinical practice, this interaction highlights the importance of reviewing medication combinations, monitoring prothrombin time and international normalized ratio, and assessing patients for bruising or gastrointestinal bleeding. Understanding its mechanisms supports safer analgesic selection and more effective management of patients receiving anticoagulant therapy.

The accompanying video describes a procedure for percutaneous placement of the WATCHMAN Left Atrial Appendage (LAA) Device. The WATCHMAN is a nitinol device designed to be permanently implanted at, or slightly distal to, the opening of the left atrial appendage (LAA) to trap blood clots before they exit the LAA, preventing thromboembolic stroke.

Source: Vy M. Dong and Faben Cruz, Department of Chemistry, University of California, Irvine, CA This experiment will demonstrate the concept of organocatalysis by illustrating the proper setup of a reaction that utilizes enamine catalysis. Organocatalysis is a form of catalysis that uses substoichiometric amounts of small organic molecules to accelerate reactions. This type of catalysis is complementary to other forms of catalysis such as transition metal or biocatalysis. Transition metal...
