De Novo Hepatitis B

De novo hepatitis B is a newly acquired hepatitis B virus (HBV) infection in a person without previously recognized infection, particularly a liver transplant recipient, and it matters because immunosuppression can accelerate viral replication and liver injury. Infection may arise from an HBV-infected donor graft or another exposure, including an occult donor infection that is not detected by routine surface-antigen testing; immunosuppressive therapy then weakens immune control of the virus. In medicine, recognizing de novo hepatitis B supports donor screening, recipient monitoring, antiviral prophylaxis with nucleos(t)ide analogues, and, when indicated, hepatitis B immunoglobulin. These strategies reduce transmission, chronic infection, graft dysfunction, and related complications.
