Using Diffusion Tensor Imaging in Traumatic Brain Injury

diffusion mri reveals white matter injury

Diffusion MRI can show white matter injury in the brain, especially when standard MRI cannot. Standard brain scans are very good at showing the large structures of the brain and the border between gray and white matter. They can show the size and shape of brain regions, but they do not show the internal structure or integrity of white matter networks. These networks are made of axon bundles that connect nearby and distant parts of the brain.

This method works by tracking how water molecules move. In diffusion MRI, the pulse sequences are sensitive to water diffusion, or the spread of water molecules. Water inside an axon does not move freely in every direction. It tends to move more along the length of the axon, which is called anisotropic movement. When movement is equal in all directions, it is isotropic. These diffusion patterns can suggest features of white matter such as fiber density, axon thickness, and myelination. One common measure is fractional anisotropy, or FA, which ranges from 0 for completely isotropic movement to 1 for the most anisotropic movement.

The study uses diffusion tensor imaging, or DTI, to measure white matter integrity in traumatic brain injury, or TBI. TBI happens when an external force injures the brain, such as a blow to the head or a sudden movement in a car accident. Mechanical injury like this can cause diffuse axonal injury, which damages white matter throughout the brain. Because the injury affects white matter integrity, standard neuroimaging may not show the damage well. Diffusion measures are more sensitive to these changes. The study compares healthy controls with people who have TBI and also tests the link between white matter integrity and attention.

The analysis focuses on three white matter tracts using a region of interest, or ROI, approach. The tracts are the splenium of the corpus callosum, the anterior corona radiata, and the superior longitudinal fasciculus. The splenium is the most posterior part of the corpus callosum. These regions are defined from the ICBM DTI-81 atlas and are shown in horizontal brain slices.
