Changes in the Appendicular Skeleton with Age

The appendicular skeleton develops as limb buds during embryogenesis and consists of hyaline cartilage and fibrous membranes.
As the embryo grows, most of the appendicular skeleton develops by endochondral ossification, in which bones form by replacing cartilage.
Only the clavicles develop by intramembranous ossification, in which the bone forms directly from the embryonic connective tissue or mesenchyme.
After birth, the lower limbs grow rapidly, becoming proportionate to the trunk&#39;s size by age ten, and continue to grow to reach the adult length.
Other parts, such as the female pelvis, broaden during puberty in preparation for childbearing and continue to enlarge until age twenty-five.
After twenty-five, equal rates of bone resorption and deposition result in very few changes in a healthy skeleton until middle age.
With advancing age, the bone resorption rate exceeds the bone deposition rate. Thus, bones become more porous, which increases the risk of fracture, especially in the neck of the femur.

The upper and lower limb initially develops as a small bulge called a limb bud, which appears on the lateral side of the early embryo. The upper limb bud appears near the end of the fourth week of development, with the lower limb bud appearing shortly after.
Initially, the limb buds consist of a core of mesenchyme covered by a layer of ectoderm. The ectoderm at the end of the limb bud thickens to form a narrow crest called the apical ectodermal ridge. This ridge stimulates the underlying mesenchyme to proliferate rapidly, producing the outgrowth of the developing limb. As the limb bud elongates, cells located farther from the apical ectodermal ridge slow their rates of cell division and begin to differentiate. In this way, the limb develops along a proximal-to-distal axis.
During the sixth week of development, the distal ends of the upper and lower limb buds expand and flatten into a paddle shape. This region will become the hand or foot. The wrist or ankle areas then appear as a constriction that develops at the base of the paddle. Shortly after this, a second constriction on the limb bud appears at the future site of the elbow or knee. Within the paddle, areas of tissue undergo cell death, producing separations between the growing fingers and toes. Also, during the sixth week of development, mesenchyme within the limb buds begins to differentiate into hyaline cartilage, which will form models of the future limb bones.
The early outgrowth of the upper and lower limb buds initially has the limbs positioned so that the regions that will become the palm of the hand or the bottom of the foot are facing medially toward the body, with the future thumb or big toe both oriented toward the head. During the seventh week of development, the upper limb rotates laterally by 90 degrees so that the palm of the hand faces anteriorly and the thumb points laterally. In contrast, the lower limb undergoes a 90-degree medial rotation, bringing the big toe to the medial side of the foot.
All of the girdle and limb bones, except for the clavicle, develop by the process of endochondral ossification. By the twelfth week, a primary ossification center appears in the diaphysis (shaft) region of the long bones, initiating the process that converts the cartilage model into bone. A secondary ossification center will appear in each epiphysis (expanded end) of these bones at a later time, usually after birth. The primary and secondary ossification centers are separated by the epiphyseal plate, a layer of growing hyaline cartilage. This plate is located between the metaphysis and each epiphysis. It continues to grow and is responsible for the lengthening of the bone. The epiphyseal plate is retained for many years until the bone reaches its final adult size, at which time the epiphyseal plate disappears, and the epiphysis fuses to the metaphysis.
The clavicle is the first bone to begin ossification, with ossification centers appearing during the fifth week of development. Initially, two ossification centers form from intramembranous ossification and fuse to form the middle of the clavicle. Then endochondral ossification occurs at both the medial and lateral ends after birth, with the medial epiphysis being the last bone site to complete ossification in the body.
During this process, mesenchymal cells differentiate directly into bone-producing cells, which produce the clavicle directly without first making a cartilage model. Because of this early production of bone, the clavicle is the first bone of the body to begin ossification, with ossification centers appearing during the fifth week of development. However, the ossification of the clavicle is not complete until age 25.

The appendicular skeleton begins as limb buds on the side of the early embryo. The upper limb bud appears near the end of the fourth week of development, and the lower limb bud follows soon after. Each limb bud has a core of mesenchyme, which is a loose embryonic tissue, covered by ectoderm, the outer cell layer.

A thickened part of the ectoderm forms the apical ectodermal ridge. This ridge signals the mesenchyme below it to divide quickly and drive limb outgrowth. As the limb bud grows longer, cells farther from the ridge slow their division and start to specialize. This creates development along a proximal-to-distal axis, from the body outward.

By the sixth week, the ends of the upper and lower limb buds widen and flatten into a paddle shape. This paddle becomes the hand or foot. A constriction then appears at the base of the paddle to mark the future wrist or ankle. A second constriction forms later at the future elbow or knee. At the same time, tissue in the paddle breaks down in places, which separates the growing fingers and toes.

Also during the sixth week, mesenchyme in the limb buds begins to change into hyaline cartilage. This cartilage forms the early models of the future limb bones. The upper and lower limb buds also start out with an unusual orientation. The future palm or sole faces toward the middle of the body, and the future thumb or big toe points toward the head.

During the seventh week, the upper limb rotates laterally by 90 degrees. This turns the palm to the front and moves the thumb to the lateral side. The lower limb rotates medially by 90 degrees. This places the big toe on the medial side of the foot.

Most girdle and limb bones, except the clavicle, form by endochondral ossification. In this process, bone replaces a cartilage model. By the twelfth week, a primary ossification center appears in the diaphysis, or shaft, of the long bones. A secondary ossification center forms later in each epiphysis, or expanded end, usually after birth. The growing cartilage between these centers is the epiphyseal plate. It sits between the metaphysis and each epiphysis and allows the bone to lengthen.

The epiphyseal plate stays in place for many years until the bone reaches adult size. Then it disappears, and the epiphysis fuses with the metaphysis. The clavicle follows a different pattern. It is the first bone to begin ossification, with centers appearing during the fifth week of development. Two centers form by intramembranous ossification and fuse to make the middle of the clavicle. After birth, endochondral ossification occurs at the medial and lateral ends, and the medial epiphysis is the last bone site in the body to finish ossifying. The clavicle forms directly from mesenchymal cells without first making a cartilage model, and its ossification is not complete until about age 25.

There are four main cell types that compose bone tissue, each with a specific function: osteogenic cells, osteocytes, osteoblasts and osteoclasts. The microscopic anatomy of compact bone includes osteons. Bones are classified by their shape. The structure of a typical long bone can be explored. Specific bones of the skeleton can be studied by their subdivisions: the axial skeleton and the appendicular skeleton. Cartilage is found in areas of the nose, ears, ribs and joints. Joints can be classified by structure or by function. The general structure of synovial joints may be explored. Ligaments connect bone to bone, stabilizing joints. The skeletal system provides support for the human body, protects soft organs, allows for movement due to attachment of muscles, stores minerals and fat and forms blood cells. Processes of the skeletal system include hematopoiesis, ossification and bone growth and remodeling. A comparison of male to female, juvenile to adult or human to other vertebrate skeletons may be explored. Homeostatic imbalances are explored. These include, but are not limited to, osteoporosis, malnutrition, fractures, anterior cruciate ligament (ACL) injuries and arthritis. Investigations are used to understand and explain the skeletal system in a variety of inquiry and design scenarios that can incorporate evolutionary concepts, scientific reasoning, comparative analysis, communication skills and real-world applications.

Smooth muscle cells, found in the hollow organs and blood vessels, are mononucleated, spindle-shaped and nonstriated. Skeletal muscle cells, found attached to bones and skin, are multinucleated, cylindrical and striated. The muscles of the body can be studied by group, which include the muscles of the head, face and neck, the trunk and the upper and lower limbs. Processes of the muscular system include gross body movements produced by skeletal muscles as they interact with the skeletal system, and muscle contraction. The connection between the nervous system and the skeletal system should be explored through the study of action potentials and the resulting contraction of sarcomeres, as described by the sliding filament theory. Energy processing and muscle responses to stimuli can be studied along with building muscle tissue through exercise. The effects of steroids can also be investigated. Homeostatic imbalances are explored. These include, but are not limited to, muscular dystrophy and atrophy. Investigations are used to understand and explain the muscular system in a variety of inquiry and design scenarios that can incorporate evolutionary concepts, scientific reasoning, comparative analysis, communication skills and real-world applications.
