Respiratory syncytial virus disease is a common respiratory illness in children. It is typically caused by the respiratory syncytial virus, or RSV.The virus spreads between humans primarily through respiratory droplets, entering the host via the nasal mucosa.After entry, RSV targets the apical surface of ciliated epithelial cells in the respiratory tract.The virus envelope contains the G protein, which binds to cellular receptors, and the F protein, which initiates fusion of viral and host membranes.After fusion, the viral nucleocapsid enters the host cell cytoplasm.RSV first transcribes its RNA into mRNA, which directs viral protein production for capsid assembly and genome replication. The virus then uses the host&#x2019;s cellular machinery to complete replication inside the cell.Next, the new virions bud off to infect neighboring cells.The host immune system responds by releasing inflammatory cytokines, which contribute to damage of the respiratory lining.This causes cellular debris and mucus accumulation in the respiratory tract, which obstructs the airways and impairs normal breathing.

Human respiratory syncytial virus (RSV) is a widespread pathogen that primarily targets infants and young children but also poses a serious health risk to elderly and immunocompromised individuals. Belonging to the Pneumoviridae family, RSV is a negative-sense, single-stranded RNA virus within the Pneumovirus genus. Its global health burden is significant, with millions of cases annually resulting in hospitalizations and mortality, particularly in resource-limited settings. Although most infections are limited to mild upper respiratory symptoms, severe lower respiratory tract involvement&#x2014;particularly bronchiolitis&#x2014;is common in young children and can progress to pneumonia or respiratory failure.Structural Features and Host Cell Entry:RSV is an enveloped virus comprising a lipid bilayer that encases a ribonucleoprotein core. The viral envelope displays several key glycoproteins, most notably the attachment glycoprotein (RSV-G) and the fusion glycoprotein (RSV-F). These surface proteins are critical for viral entry. RSV-G facilitates initial adherence to host epithelial cells, particularly targeting apical ciliated cells in the respiratory tract. Subsequent fusion of the viral envelope with the host membrane is mediated by RSV-F, allowing insertion of the nucleocapsid into the host cytoplasm.Viral Life Cycle and Host Immune Response:After entry, RSV begins its replication cycle in the cytoplasm. The negative-sense RNA genome is transcribed by viral RNA-dependent RNA polymerase to produce mRNA and a positive-sense antigenome for replication. This generates mRNA transcripts for viral protein synthesis and positive-sense genomic RNA templates for replication. Viral assembly occurs in the cytoplasm, and new virions bud from the host cell membrane, acquiring their lipid envelope in the process.Host immune responses contribute significantly to pathogenesis. Viral replication and immune-mediated cytotoxicity lead to epithelial necrosis, mucus hypersecretion, and airway obstruction. The inflammation includes early polymorphonuclear infiltration followed by a lymphomononuclear response, which impairs mucociliary clearance and reduces lung compliance.The structure-function relationship of RSV&#x2019;s membrane proteins underlies both its infectivity and its recognition as a target for therapeutic and prophylactic interventions, including monoclonal antibodies such as palivizumab and nirsevimab, which target the fusion protein to prevent cellular entry. Understanding RSV&#x2019;s pathogenesis is vital for developing more effective antiviral therapies and vaccines.

Respiratory syncytial virus (RSV) is a common respiratory virus that can infect the lungs and airways. It mainly causes illness in infants and young children, but it can also seriously affect older adults and people with weak immune systems. RSV belongs to the Pneumoviridae family and is a negative-sense, single-stranded RNA virus in the Pneumovirus genus.

RSV has a major global health impact. It causes millions of infections each year and leads to hospitalizations and deaths, especially in places with fewer medical resources. Many cases cause only mild upper respiratory symptoms. But in young children, RSV can move into the lower respiratory tract and cause bronchiolitis. In severe cases, it can also lead to pneumonia or respiratory failure.

The virus is enveloped, which means it has a lipid bilayer around a ribonucleoprotein core. Its surface carries important glycoproteins, including the attachment glycoprotein RSV-G and the fusion glycoprotein RSV-F. RSV-G helps the virus stick to host epithelial cells, especially apical ciliated cells in the respiratory tract. RSV-F then drives fusion of the viral envelope with the host membrane, which lets the nucleocapsid enter the host cytoplasm.

After entry, RSV replicates in the cytoplasm. Its negative-sense RNA genome is copied by a viral RNA-dependent RNA polymerase to make mRNA and a positive-sense antigenome. The mRNA supports viral protein production, while the antigenome serves as a template for making new genomic RNA. New virus particles assemble in the cytoplasm and bud from the host cell membrane, picking up a lipid envelope as they leave.

Host immune responses also shape RSV disease. Viral replication and immune-mediated cell damage can cause epithelial necrosis, mucus hypersecretion, and airway obstruction. The inflammation begins with early polymorphonuclear infiltration and is followed by a lymphomononuclear response. These changes reduce mucociliary clearance and lower lung compliance, which makes breathing harder.

RSV membrane proteins are also important targets for prevention and treatment. Their structure and function help explain why the fusion protein is such a useful target for therapy. Monoclonal antibodies such as palivizumab and nirsevimab bind to the fusion protein and help prevent viral entry into cells. Understanding RSV pathogenesis is important for developing better antiviral therapies and vaccines.
