Medical management of peptic ulcer disease, or PUD, involves a comprehensive approach.
Medication is the first-line treatment for 10 to 14 days to eradicate H.pylori using a combination of antibiotics, proton pump inhibitors or PPI, and occasionally bismuth salts.
The regimen may entail triple therapy with two antibiotics, such as metronidazole and clarithromycin, plus a PPI.
Alternatively, quadruple therapy includes two antibiotics, metronidazole, and tetracycline, alongside PPI and bismuth salts.
For Zollinger-Ellison syndrome, where gastrin hypersecretion stimulates hydrochloric acid release, treatment with PPIs or Octreotide is indicated.
Additionally, cytoprotective drugs such as sucralfate and adjunct drugs, including H2 receptor blockers, antacids, and a prostaglandin analog, may be used to treat PUD.
Next, dietary modification is crucial to avoid foods that stimulate acid secretion, such as spicy foods, carbonated drinks, and caffeine.
Patients should be educated to rest, avoid alcohol and smoking, and ensure regular follow-up care.
