Pneumoperitoneum

Pneumoperitoneum is the presence of gas within the peritoneal cavity, a finding that may be intentionally created during minimally invasive surgery or indicate a potentially life-threatening abdominal perforation. In laparoscopy, clinicians insufflate carbon dioxide through a trocar to expand the abdomen, improving visualization and instrument movement; pathologic pneumoperitoneum commonly results when air escapes from a perforated gastrointestinal organ. Detection with upright radiography, computed tomography, or ultrasound helps identify free intraperitoneal gas and guide urgent evaluation. In clinical practice, interpretation alongside symptoms, examination findings, and recent procedures supports decisions ranging from observation to emergency surgery and treatment of the underlying cause.

Single Port Donor Nephrectomy

Single port laparoscopic surgery is changing the standard of care in surgical care like nothing since the laparoscopic technique was introduced 20 years ago. We present out technique of single port donor nephrectomy using the Gelpoint device. We have successfully performed this surgery in 100 patients.

Here, we describe a minimally invasive laparoscopic technique for serial sampling of liver and mesenteric lymph nodes (MLN) in macaques that allows for increased sampling frequency, and reduces the potential for surgical complications when compared to performing a laparotomy.

A video demonstrates the technique in a patient with main duct-IPMN, highlighting its feasibility as a minimally invasive, organ-preserving alternative to pancreaticoduodenectomy.

In early gastric cancer, the aim of surgery is to precisely remove the distal stomach including the primary tumor. To do this, accurate localization of the tumor is crucial, especially in totally laparoscopic surgery. This protocol describes a procedure for intraoperative gastroscopy in totally laparoscopic subtotal gastrectomy.
