Pyelonephritis

Pyelonephritis is a bacterial infection of the kidney and renal pelvis that can impair urinary tract function and cause serious systemic illness if untreated. In most cases, bacteria ascend from the bladder through the ureters, triggering inflammation in renal tissue; common features include fever, flank pain, dysuria, nausea, and urinary frequency. Clinical evaluation combines medical history, physical examination, urinalysis, urine culture, and, when indicated, imaging to identify complications such as obstruction or abscess. Prompt antimicrobial treatment and management of underlying conditions help prevent renal damage, bloodstream infection, and recurrent disease, making pyelonephritis an important condition in clinical medicine.

2025

Pyelonephritis is a bacterial infection that primarily affects the renal parenchyma and collecting system, including the renal pelvis, tubules, and interstitial tissue of one or both kidneys. It can be classified as either acute—a sudden, severe infection—or chronic, which refers to long-term or recurrent kidney infections. The primary cause of acute pyelonephritis (APN) is bacterial infection, with Escherichia coli accounting for approximately 70-80% of cases. Other bacteria, such as Proteus,...

Introduction:For diagnosing acute pyelonephritis, a comprehensive patient history is collected to identify symptoms such as dysuria, frequent or urgent urination, flank pain, or costovertebral angle (CVA) tenderness that may suggest a kidney infection.Physical ExaminationDuring the physical examination, CVA tenderness is assessed. This involves gentle percussion over the costovertebral angle, where tenderness often indicates a kidney infection.Diagnostic TestsUrinalysis: Used to identify white...

This manuscript describes a rat surgical model of pyelonephritis using direct intra-renal infection by Escherichia coli into the renal pelvis. The experimental procedure can be utilized to study the pathogenesis of pyelonephritis as well as the associated inflammation and fibrosis.

Source: Gupta, K., et al. Intrarenal Injection of Escherichia coli in a Rat Model of Pyelonephritis. J. Vis. Exp. (2017).This video demonstrates the intrarenal injection of E. coli into the renal pelvis of an anesthetized rat to induce a localized kidney infection, modeling acute pyelonephritis. It highlights key surgical steps and aseptic techniques necessary for establishing a reproducible in vivo infection model for downstream analysis.
