Subjective Database

A subjective database is a structured collection of health information reported by a patient or caregiver, including symptoms, perceptions, concerns, and relevant personal history. In clinical assessment, practitioners gather these data through interviews, active listening, and open-ended questions, then document the patient’s descriptions and evaluate their completeness and reliability alongside objective findings. Subjective databases help establish a patient-centered baseline, guide focused physical examinations, support clinical reasoning, and identify changes that may require intervention. When updated over time, they also improve communication among healthcare professionals and provide context for interpreting measurements, test results, treatment responses, and patient priorities.
