Retroperitoneal Laparoscopy

Retroperitoneal laparoscopy is a minimally invasive surgical technique that accesses organs and structures behind the peritoneal lining through small abdominal or flank incisions, avoiding entry into the main peritoneal cavity. Surgeons create a working space in the retroperitoneum, typically insufflate carbon dioxide to separate tissues, and use a laparoscope and specialized instruments to visualize and dissect the operative area. The approach is used in urologic and other procedures involving the kidneys, adrenal glands, ureters, and nearby tissues. By limiting manipulation of the abdominal contents, it can support targeted access, smaller incisions, and postoperative recovery while requiring advanced anatomical and technical expertise.

This article presents a method for measuring the working abdominal volume during laproscopic myomectomy using the surgical grasper, and applies this technique to obtain pilot data on whether deep neuromuscular blockade (NMB) can enable the use of lower insufflation pressure. Reduced insufflation pressure during laproscopic surgery has been shown to reduce post-operative pain, and thus the use of NMB during surgery may enable improved patient outcomes.

Ovine Lumbar Intervertebral Disc Degeneration Model Utilizing a Lateral Retroperitoneal Drill Bit Injury

Intervertebral disc degeneration is a significant contributor to back pain and a leading cause of disability worldwide. Numerous animal models of intervertebral disc degeneration exist. We demonstrate an ovine model of intervertebral disc degeneration, utilizing a drill bit, which achieves a consistent disc injury and reproducible level of disc degeneration.

To improve the drainage of patients with necrotizing pancreatitis complicated with peripancreatic abscess and reduce the mortality of patients with necrotizing pancreatitis, we adopted a retroperitoneal approach for peripancreatic abscess debridement and drainage by laparoscopy.

Neuronavigation and Laparoscopy Guided Ventriculoperitoneal Shunt Insertion for the Treatment of Hydrocephalus

Patient outcomes of ventriculoperitoneal (VP) shunt surgery, the mainstay treatment for hydrocephalus in adults, are poor due to high shunt failure rates. We present intraoperative footage of VP shunt insertion using neuronavigation and laparoscopy guidance, with the goal to reduce the risks of proximal and distal shunt catheter failures, respectively.
