Alpha Glucosidase Inhibitors

Alpha glucosidase inhibitors are oral antidiabetic drugs that reduce the rate at which dietary carbohydrates enter the bloodstream, making them important tools for controlling postprandial blood glucose in type 2 diabetes. They act in the small intestine by reversibly inhibiting brush-border alpha glucosidase enzymes, which normally break oligosaccharides and disaccharides into absorbable monosaccharides; this delays glucose absorption rather than increasing insulin secretion. Drugs such as acarbose, miglitol, and voglibose are often taken with meals and can improve glycemic control, particularly after carbohydrate-rich foods. Because undigested carbohydrates reach the colon, gastrointestinal effects such as flatulence and diarrhea are common.
