Anatomical Hepatectomy

Anatomical hepatectomy is a liver resection that removes a tumor-bearing segment, section, or lobe according to the liver’s internal portal venous anatomy, while preserving uninvolved parenchyma. Surgeons identify and control the corresponding Glissonian pedicle, which contains the portal vein, hepatic artery, and bile duct branches, then divide the liver along the associated portal territory. This approach is used primarily to treat selected primary or metastatic liver tumors and can help achieve complete tumor removal while limiting unnecessary tissue loss. Understanding Couinaud segmental anatomy, vascular inflow, biliary drainage, and remaining liver function is essential for safe operative planning and postoperative recovery.

We have described a protocol for performing partial hepatectomy (PHx) and cell transplantation via spleen in NOD.SCID (NOD.CB17-Prkdcscid/J) mice. In this protocol, an incision is made to expose and resect the left lobe of the liver followed by another incision for the intrasplenic transplantation of cells.

Numerous studies have demonstrated the advantages of anatomic resection. Nonetheless, whether anatomic resection can increase R0 resection rates remains controversial. Consequently, the present study describes an innovative procedure involving the en bloc concept combined with anatomic resection in laparoscopic hepatectomy, which can reduce postoperative recurrence and metastasis.
