Discharge Pathways

Discharge pathways are structured clinical processes that guide a patient’s transition from hospital care to home, rehabilitation, or another healthcare setting. They combine assessment of readiness, medication reconciliation, discharge instructions, communication with patients and caregivers, and coordination with community or follow-up services. In medicine, effective pathways match ongoing care needs with appropriate resources while clarifying warning signs, treatment responsibilities, and follow-up appointments. By supporting continuity across care settings, they help reduce preventable errors, improve patient understanding, and promote safer recovery after hospitalization. Pathway design can also reveal gaps in care coordination and inform quality-improvement efforts.
