Transoral Laser Surgery

Transoral laser surgery is a minimally invasive procedure that uses laser energy delivered through the mouth to remove or treat abnormal tissue in the upper aerodigestive tract, often without an external incision. It works by focusing a controlled beam to vaporize, cut, or coagulate tissue while the surgeon views the operative field through a microscope or endoscope. In medicine, clinicians use this approach for selected lesions of the larynx, pharynx, and oral cavity, including some early cancers and benign growths. By limiting disruption to surrounding structures, it can support organ preservation and functional recovery, although patient selection depends on lesion location and extent.

Here, we present a protocol to resect benign infratemporal fossa tumors via an endoscopic-assisted transoral lateral molar approach with low-temperature plasma ablation and intraoperative mandibular nerve monitoring. This minimally invasive technique prioritizes anatomical preservation, functional recovery, and avoidance of external scarring, demonstrating feasibility for well-demarcated lesions ≤4 cm in diameter.

An “All-laser” Endothelial Transplant

An all laser procedure is proposed in the endothelial transplant. The surgical technique is based on the use of a femtosecond laser to prepare the donor tissue. Laser welding technique is then used to secure the donor endothelium in the correct position.

A protocol to detect and automate mode locking in a pre-adjusted nonlinear polarization rotation fiber laser is presented. The detection of a sudden change in the output polarization state when mode locking occurs is used to command the alignment of an intra-cavity polarization controller in order to find mode-locking conditions.
