The Michaelis-Menten equation parameters–Michaelis constant, KM, and the maximum process rate, Vmax – are estimated post-drug administration.
For instance, phenytoin, exhibiting capacity-limited kinetics, requires two different dose regimens until achieving steady-state concentration or Css.
KM and Vmax can be determined by a direct linear plot. This involves plotting two Css against corresponding dosing rates, then plotting the x- and y-intercepts at the point of intersection to obtain KM and Vmax, respectively.
Alternatively, the Lineweaver Burk plot gives KM/Vmax as the slope and 1/Vmax as the y-intercept.
Another method plots the dosing rate, DR, against the DR to Css ratio, yielding −KM as slope and Vmax as y-intercept.
Clinically, KM values vary among patients.
Patients with lower KM values may experience larger shifts in plasma concentrations during dosage adjustments and a more significant change in the rate of drug elimination compared to those with higher KM values.
