Renal Orthotopic Transplantation

Renal orthotopic transplantation is a surgical procedure that replaces a diseased kidney by placing a donor kidney in the recipient’s native renal fossa, restoring renal function in selected cases of kidney failure. The graft is connected to the recipient’s renal blood vessels and ureter, allowing blood filtration, urine production, and drainage through anatomically appropriate pathways; immunosuppressive therapy helps limit immune-mediated rejection. Although most kidney transplants use a heterotopic position in the pelvis, orthotopic transplantation can be valuable when pelvic vessels or urinary anatomy are unsuitable. It provides a model for studying organ compatibility, transplantation biology, graft survival, and long-term management of end-stage renal disease.

The rat orthotopic renal transplantation model contributes to investigating the mechanism of renal allograft rejection. The current model increases the recipients' survival without interference with blood supply and venous reflux of the lower body using an end-to-end anastomosis of kidney implantation and an end-to-side "tunnel" method of ureter-bladder anastomosis.
