Thymic Graft Transplantation

Thymic graft transplantation is a cellular therapy that implants functional thymic tissue into patients whose thymus cannot support normal T-cell development, helping restore essential adaptive immunity. After implantation, donor thymic epithelial cells provide the specialized microenvironment where recipient hematopoietic precursors mature, undergo positive and negative selection, and develop a diverse, self-tolerant T-cell repertoire. The procedure is primarily used for congenital athymia, including severe forms associated with DiGeorge syndrome, in which absent or severely impaired thymic function causes profound susceptibility to infection. By reconstituting cellular immunity, thymic grafts provide an important approach in immunology and infection research and may inform future therapies for immune-development disorders.

This video demonstrates a protocol for generating functional antigen-specific T cells from iPSC-derived immature T cells using a three-dimensional thymic culture system. Co-culturing the iPSC-derived immature T cells with endogenous lymphocyte-depleted mice fetal thymic lobes resulted in successful maturation of the T cells to form CD8αβ+ MHC class I+ iPSC-derived thymic emigrants (iTEs).

The severe organ shortage has resulted in increased use of marginal kidney grafts for transplantation. This has triggered interest in alternative storage methods, since marginal grafts especially tolerate cold storage poorly. The technique of normothermic ex vivo kidney perfusion (NEVKP) represents a novel preservation method for kidney grafts prior to transplantation.
