Donor Lung Assessment

Donor lung assessment is the systematic evaluation of lungs from a potential organ donor to determine their suitability for transplantation and reduce risks to the recipient. Clinicians integrate the donor’s medical history with oxygenation measurements, chest imaging, bronchoscopy, airway findings, and microbiological testing to identify adequate function, reversible abnormalities, infection, or structural damage. This assessment supports decisions about accepting, declining, or selectively using lungs, including organs from extended-criteria donors, and helps guide perioperative planning. Accurate evaluation can expand the transplant pool while promoting recipient safety and improving the likelihood of effective graft function after transplantation.

Here, we describe the necessary steps for establishing a rat EVLP model and show the inflammatory profile associated with the perfused lungs. The aim is to propagate knowledge and experiences about the rat EVLP model, enabling the integral understanding of the biological responses associated with that revolutionary technique.
