Focal Coronary Stenosis

Focal coronary stenosis is a localized narrowing within a coronary artery that can restrict blood flow to the heart muscle and contribute to ischemic heart disease. It most often results from atherosclerotic plaque buildup, which reduces the arterial lumen and may limit blood delivery, particularly when the heart requires more oxygen during exertion. Clinicians assess its severity using symptoms, electrocardiography, coronary imaging, and measurements of blood-flow significance. Characterizing focal stenosis helps guide treatment decisions, including medical therapy, catheter-based angioplasty and stenting, or surgical revascularization, with the goal of relieving ischemia and reducing cardiovascular risk.

Coronary flow reserve (CFR) is useful for assessment of myocardial oxygen demand and evaluation of cardiovascular risk. This study establishes a step-by-step transthoracic Doppler echocardiographic (TTDE) method for longitudinal monitoring of the changes in CFR, as measured from coronary artery in mice, under the experimental pressure overload of aortic banding.

Intracoronary acetylcholine testing has been established for the assessment of epicardial coronary spasm more than 30 years ago. Recently, the focus has shifted towards the microcirculation and it has been shown that microvascular spasm can be detected using ACH-testing. This article describes the ACH-test and its implementation in daily routine.

Catheter-based Endovascular Angioplasty for Fibrosing Mediastinitis-associated Pulmonary Vein Stenosis

2025

Pulmonary vein stenosis (PVS) is a life-threatening complication of fibrosing mediastinitis (FM), with a poor prognosis if not properly treated. Currently, catheter-based endovascular angioplasty is a promising therapeutic strategy for FM-associated PVS. This protocol primarily outlines a stepwise approach to the transcatheter procedure for FM-associated PVS.
