Laparoscopic Pancreatectomy

Laparoscopic pancreatectomy is a minimally invasive surgical technique for removing part or, in selected cases, all of the pancreas through small abdominal incisions, offering an alternative to open surgery. Surgeons create a pneumoperitoneum, insert a camera and specialized instruments, and carefully dissect pancreatic tissue and nearby structures before dividing the gland and extracting the specimen. The approach is used for selected pancreatic tumors, cystic lesions, and other localized disease, with procedures such as distal pancreatectomy sometimes preserving the spleen. Appropriate patient selection and advanced surgical expertise are essential because pancreatic leaks, bleeding, and impaired endocrine or digestive function can occur.

Oncologically safe left pancreatectomy requires radical resection (R0), Gerota’s (perirenal) fascia resection, and adequate lymph node dissection. This study describes the technical details of laparoscopic radical left pancreatectomy (LRLP), used in the first international multicenter randomized trial comparing minimally invasive with open left pancreatectomy for pancreatic cancer, the DIPLOMA trial.

In early gastric cancer, the aim of surgery is to precisely remove the distal stomach including the primary tumor. To do this, accurate localization of the tumor is crucial, especially in totally laparoscopic surgery. This protocol describes a procedure for intraoperative gastroscopy in totally laparoscopic subtotal gastrectomy.
