Posterior Rectus Sheath

The posterior rectus sheath is a layered aponeurotic structure behind the rectus abdominis muscle that helps form the anterior abdominal wall and is important in surgical anatomy. Above the arcuate line, it is formed mainly by the posterior lamina of the internal oblique aponeurosis and the transversus abdominis aponeurosis; below this landmark, these aponeuroses pass anteriorly, leaving the rectus against the transversalis fascia. This arrangement encloses and stabilizes the rectus muscle, distributes tension across the abdominal wall, and defines tissue planes used in abdominal surgery, hernia assessment, imaging interpretation, and regional anesthetic approaches.
