Urinary Concentrating Capacity

Urinary concentrating capacity is the kidney’s ability to conserve water by producing urine with a higher solute concentration than blood plasma, an essential function for maintaining fluid and electrolyte balance. It depends on countercurrent multiplication in the loop of Henle, which establishes a hyperosmotic medullary gradient, while antidiuretic hormone (ADH) increases aquaporin-2 water channels in collecting-duct cells so water leaves the tubular fluid and returns to the circulation. In medicine, measuring urine osmolality or specific gravity helps evaluate hydration, renal concentrating defects, and disorders involving ADH or kidney tubules. These assessments support the diagnosis and monitoring of renal and systemic disease.
