Question 1

What are the main components of a basic ophthalmoscope?

Accepted Answer

A basic ophthalmoscope consists of a light source with a dimmer for adjusting brightness, an aperture to look through, a disc for selecting lenses of different diopters, and a diopter indicator displaying the lens power. These components work together to allow physicians to focus light and examine the fundus, or inner wall of the posterior eye, systematically.

Question 2

How do diopter settings help examine patients with different vision types?

Accepted Answer

A diopter of zero means the ophthalmoscope lens neither converges nor diverges light. Turning the dial counterclockwise toward negative (red) diopter settings helps examine myopic or nearsighted patients whose retina lies closer than normal. Turning clockwise toward positive (green) settings helps examine hyperopic or farsighted patients whose retina lies farther away.

Question 3

What key anatomical landmarks should you identify during a fundoscopic exam?

Accepted Answer

The primary landmarks are the optic disc, where the optic nerve and retinal vessels enter the eye; the fovea, a darkened circular area marking central vision; and the macula, an oval-shaped pigmented area surrounding the fovea. The optic disc contains a central whitish physiologic cup that normally occupies less than half the disc's diameter.

Question 4

What is the red reflex and why is it important during ophthalmoscopic examination?

Accepted Answer

The red reflex is an orange-red glow visible when the ophthalmoscope's light is directed at the patient's pupil. It indicates light reflection from the retina and helps confirm proper positioning. You should note any opacities that interfere with the red reflex, as these may indicate lens problems or other abnormalities.

Question 5

How should you position yourself and the ophthalmoscope when examining a patient's right eye?

Accepted Answer

Hold the ophthalmoscope in your right hand and look through the aperture with your right eye, keeping both eyes open. Position yourself about a foot away with your eye and the patient's eye at the same level. Place the ophthalmoscope about 15 degrees laterally to the patient's line of vision, then move it inward until almost on top of the eye while maintaining focus.

Question 6

What should you examine when evaluating the optic disc and retinal vessels?

Accepted Answer

Carefully examine the disc for color, outline clarity, relative size of the central cup, and symmetry with the other eye. Follow retinal vessels in all four directions, noting that veins appear redder and wider than arteries. Look for spontaneous venous pulsations, arteriovenous crossings, and any retinal lesions, noting their size, shape, and location.

Question 7

How do you examine the fovea and macula, and what appearance should you expect?

Accepted Answer

Ask the patient to look directly into the ophthalmoscope's light to examine the fovea and surrounding macula. The macula often appears to shimmer. To check for lens opacities, adjust the diopters to between 10 positive and 12 positive settings. This systematic approach ensures complete evaluation of the posterior eye structures.
