Question 1

What are the main components of the external ear?

Accepted Answer

The external ear consists of the cartilaginous auricle, which includes the helix, antihelix, lobule, and tragus. The auricle collects sound waves and channels them through the slightly curving auditory canal to the tympanic membrane. The mastoid process is positioned just behind the earlobe. These structures form the outer ear's framework for sound collection.

Question 2

How does sound travel through the ear to the brain?

Accepted Answer

Sound waves collected by the external ear travel through the auditory canal to the tympanic membrane, which vibrates and transmits sound to three connected ossicles in the middle ear: the malleus, incus, and stapes. These vibrations are transformed into electrical signals by the cochlea in the inner ear and carried to the brain by the cochlear nerve, comprising both conductive and sensorineural phases.

Question 3

What is the proper technique for inspecting and palpating the external ear?

Accepted Answer

Begin by inspecting the auricles and surrounding tissue for skin changes, nodules, and deformities. Grasp the helix superiorly between thumb and forefinger, gently pulling up and backward to check for discomfort. Then palpate the tragus and mastoid process for tenderness. This systematic approach identifies external ear abnormalities and patient sensitivity.

Question 4

How is the whispered voice test performed to assess hearing?

Accepted Answer

Ensure the room is quiet and stand about two feet behind the patient. Gently press and rub a finger against the tragus of the non-test ear to block other sounds. Whisper a combination of three numbers and letters like "1A 2B 3C" and ask the patient to repeat them. Correctly reporting all three or at least three out of six per side constitutes a normal result.

Question 5

What do the Weber and Rinne tuning fork tests reveal about hearing loss?

Accepted Answer

The Weber test uses a 256 or 512 Hz tuning fork placed on the patient's head midline to detect lateralization, indicating unilateral hearing loss. The Rinne test compares bone conduction to air conduction by placing the vibrating fork on the mastoid process, then near the external canal. In conductive loss, bone conduction equals or exceeds air conduction; in neurosensory loss, air conduction remains superior.

Question 6

What landmarks should you identify when examining the tympanic membrane?

Accepted Answer

The malleus and incus are normally visible on the tympanic membrane, with the malleus near center and incus just posterior. The umbo marks the point of contact between the membrane and malleus tip, with a cone of light emanating downward and anteriorly from it. The short process of the malleus demarcates the pars flaccida superiorly and the larger pars tensa inferiorly. The membrane normally appears pink-gray and reflects light readily.

Question 7

What is the correct procedure for using an otoscope during ear examination?

Accepted Answer

Turn on the otoscope and select the brightest setting, then attach the largest speculum that comfortably fits the auditory canal. Hold the otoscope in your dominant hand like a pencil, stabilizing it by resting your fourth and fifth fingers on the patient's cheek. Pull the auricle slightly up and back to straighten the canal, gently insert the speculum slightly forward and down, and examine the canal before the tympanic membrane for redness, discharge, swelling, or masses.
