The management of injury to the ulnar and radial collateral ligaments at the metacarpophalangeal joint of the thumb is complex. Treatment is dependent upon a number of factors with a wide variety of options for each ligament. Inadequate treatment has the potential to lead to a poor functional outcome. We present the relevant clinical anatomy, mechanism of injury, methods of treatment available and suggest a single treatment algorithm for use in the management of these injuries.

Collateral ligament injuries of the metacarpophalangeal joint of the thumb: a treatment algorithm.

Diathermy has revolutionised modern surgery and is an important tool for efficient and safe surgical practice. It has evolved to become the modern day scalpel, being used for cutting and coagulating tissues. This article addresses the functioning and safe use of diathermy in the perioperative setting. The various precautionary checks before surgery, which are prerequisite for safe usage, are highlighted along with the necessary safety drills during and after operation. A specific note is included about the use of electrical equipment in specialised surgeries like cardiac and laparoscopic surgery. The issues regarding the maintenance and repair of equipment are acknowledged and finally, the anticipation for a future with precise and safer cutting and coagulation devices is addressed.

Diathermy in perioperative practice.

Human parathyroid hormone (PTH) is known to be efficacious in the treatment of osteoporosis and reduction of risk of vertebral and nonvertebral fractures; however, its role in the enhancement of human fracture healing is controversial. Our objective is to conduct a systematic literature review on the use of recombinant PTH in human fracture healing to (1) evaluate the evidence for recombinant PTH in human fracture healing and (2) assess whether there are notable differences between previous case reports and prospective trials.

The role of recombinant PTH in human fracture healing: a systematic review.

The major factors that determine a favorable long-term clinical and functional outcome after conventional total knee arthroplasty (TKA) include correct implant positioning and restoration of the mechanical axis with soft tissue balancing to reduce aseptic failure; hence, the need for further developmental strategies that improve the accuracy and reproducibility of the surgical technique remains paramount for contemporary navigation research. Not all navigation systems are the same. The literature published thus far on mid-term results of navigated TKA relies on software that has no step-by-step soft tissue balancing with the tibia-first technique. The results are equivalent to those of conventional TKA.Therefore, the current authors conducted a minimum 5-year follow-up of a soft tissue-based navigated TKA system with the goal of soft tissue balancing. They analyzed intraoperative alignment and range of motion measurements, functional outcomes, radiographic assessment, and survival rates of high-flexion, high-conformity unresurfaced patella TKAs. The results at 5 years revealed a component revision rate of 0% compared with other nonnavigated TKAs (2.8% revision rate). The authors achieved a well-balanced TKA with a 0°±2° mechanical axis and an improved range of motion from 95° preoperatively to 110° postoperatively.

Atypical femoral fractures have attracted a great deal of attention and controversy in the osteoporosis community because of their association with prolonged bisphosphonate therapy. The purpose of this article is to review the epidemiologic evidence linking bisphosphonates to atypical fractures and to highlight the potential pathologic mechanisms involved in such fractures. A management plan is provided based on the available evidence.

Bisphosphonate therapy and atypical fractures.

Long-term bisphosphonate use has often been associated with atypical femoral fractures. These fractures evolve from incomplete femoral fractures. A previous study demonstrated that the presence of a radiolucent line in an incomplete fracture can indicate a high risk of progression to complete fracture.

Management strategy for symptomatic bisphosphonate-associated incomplete atypical femoral fractures.

Management of the degenerate hip in patients with neuromuscular conditions should be aimed at improving quality of life and ease of nursing care. Arthroplasty poses a significant challenge with predisposition to dislocation and loosening due to anatomical abnormalities, soft tissue contractures and impaired muscle tone.

Resurfacing hip arthroplasty in neuromuscular hip disorders - A retrospective case series.

Although periprosthetic fractures of the acetabulum are relatively uncommon after total hip arthroplasty, a variety of patient-, surgeon-, and implant-related risk factors can contribute to the occurrence of this serious complication. These risk factors, combined with the increased use of cementless acetabular cups, will likely result in an increased prevalence of these fractures in the future. By better understanding the risk factors, classification schemes, and treatment options for periprosthetic fractures of the acetabulum, orthopaedic surgeons can achieve better outcomes for their patients.
