Treatment of recurrent or in-transit unresectable melanoma continues to be a major therapeutic challenge. Electrochemotherapy (ECT) is a therapeutic option for those patients whose lesions are not suitable for surgical resection and who have exhausted all other treatment modalities. ECT combines electroporation of tumor cells with the administration either of intravenous or intratumoral antineoplastic drugs, such as bleomycin or cisplatin. The cell membranes are thus rendered permeant to these impermeant hydrophilic drugs with a several hundred-fold increase in intracellular delivery and cytotoxicity. ECT is an effective treatment in the palliative management of unresectable recurrent disease with overall objective response rates of approximately 80-90%. ECT technology continues to evolve allowing application to deeper lesions. By combining ECT with tumor-specific immunostimulating approaches, such as perilesional IL-2, CpG oligonucleotides or prior immunogene therapy, there is a promise of both local and systemic control of this disease.

Traitement du m&eacute;lanome non r&eacute;s&eacute;cable r&eacute;current ou en transit continue d&#39;&ecirc;tre un enjeu th&eacute;rapeutique majeur. Electrochemotherapy (ECT) est une option th&eacute;rapeutique pour les patients dont les l&eacute;sions ne sont pas adapt&eacute;es pour la r&eacute;section chirurgicale et qui a &eacute;puis&eacute; toutes les autres modalit&eacute;s de traitement. ECT combine &eacute;lectroporation des cellules tumorales avec l&#39;administration ou l&#39;autre des m&eacute;dicaments anticanc&eacute;reux en intraveineuse ou intratumorale, comme la bl&eacute;omycine ou cisplatine. Les membranes cellulaires sont ainsi rendues perm&eacute;able &agrave; ces m&eacute;dicaments hydrophiles imperm&eacute;ants avec une augmentation de 100 fois plusieurs dans la d&eacute;livrance intracellulaire et la cytotoxicit&eacute;. Tratamiento del melanoma no resecable recurrente o en tr&aacute;nsito contin&uacute;a siendo un reto terap&eacute;utico. Electrochemotherapy (ECT) es una opci&oacute;n terap&eacute;utica para los pacientes cuyas lesiones no son adecuados para la resecci&oacute;n quir&uacute;rgica y que han agotado otras modalidades de tratamiento. ECT combina electroporaci&oacute;n de c&eacute;lulas del tumor con la administraci&oacute;n de f&aacute;rmacos antineopl&aacute;sicos intravenoso o intratumoral, como bleomicina o cisplatino. Las membranas celulares se representan as&iacute; Florescente a estas drogas hidrof&iacute;licas impermeant con el transporte intracelular y la citotoxicidad aumento varias centenas. La terapia electroconvulsiva es un tratamiento eficaz en el tratamiento paliativo de resecables recurrente con tasas de respuesta objetiva general de aproximadamente 80-90%. Tecnolog&iacute;a ECT sigue evolucionando la aplicaci&oacute;n que permite a las lesiones m&aacute;s profundas. Combinando ECT con inmunoestimulante tumor-espec&iacute;ficos enfoques, como perilesional IL-2, oligonucle&oacute;tidos CpG o terapia previa immunogene, hay una promesa de control local y sist&eacute;mica de la enfermedad.
