A large number of measures may be employed in clinical practice and for epidemiologic studies to quantify and risk stratify diabetic patients with neuropathy. However, not all measures are suitable for assessing the benefits of therapeutic intervention. Therefore, for the purpose of this review we focus on measures that may be employed to define the efficacy of interventions in clinical trials of human diabetic neuropathy. Two major types of end points are used: 1) those that assess symptoms for defining efficacy in painful diabetic neuropathy, and 2) those that assess neurologic deficits that assess the effects of treatments that may prevent further degeneration or promote repair.

Allerdings eignen sich nicht alle Ma&szlig;nahmen f&uuml;r die Beurteilung der Vorteile der therapeutische Intervention. Pertanto, ai fini di questa recensione ci concentriamo sulle misure che possono essere impiegati per definire l&#39;efficacia degli interventi in studi clinici di neuropatia diabetica umana. Surrogate markers of diabetic neuropathy are being actively sought to facilitate the diagnosis, measure the progression, and assess the benefits of therapeutic intervention in patients with diabetic neuropathy. We have quantified small nerve fiber pathological changes using the technique of intraepidermal nerve fiber (IENF) assessment and the novel in vivo technique of corneal confocal microscopy (CCM). Fifty-four diabetic patients stratified for neuropathy, using neurological evaluation, neurophysiology, and quantitative sensory testing, and 15 control subjects were studied. They underwent a punch skin biopsy to quantify IENFs and CCM to quantify corneal nerve fibers. IENF density (IENFD), branch density, and branch length showed a progressive reduction with increasing severity of neuropathy, which was significant in patients with mild, moderate, and severe neuropathy. CCM also showed a progressive reduction in corneal nerve fiber density (CNFD) and branch density, but the latter was significantly reduced even in diabetic patients without neuropathy. Both IENFD and CNFD correlated significantly with cold detection and heat as pain thresholds. Intraepidermal and corneal nerve fiber lengths were reduced in patients with painful compared with painless diabetic neuropathy. Both IENF and CCM assessment accurately quantify small nerve fiber damage in diabetic patients. However, CCM quantifies small fiber damage rapidly and noninvasively and detects earlier stages of nerve damage compared with IENF pathology. This may make it an ideal technique to accurately diagnose and assess progression of human diabetic neuropathy.

Hemos cuantificado peque&#xF1;os cambios patol&#xF3;gicos de las fibras nerviosas, mediante la t&#xE9;cnica de la fibra del nervio intraepid&#xE9;rmica (IENF) la evaluaci&#xF3;n y la novela en la t&#xE9;cnica in vivo de la microscop&#xED;a confocal de la c&#xF3;rnea (CCM). Cincuenta y cuatro pacientes diab&#xE9;ticos estratificados para la neuropat&#xED;a, mediante una evaluaci&#xF3;n neurol&#xF3;gica, la neurofisiolog&#xED;a, y las pruebas sensoriales cuantitativas, y 15 sujetos control fueron estudiados. Se sometieron a un procedimiento de biopsia cut&#xE1;nea para cuantificar IENFs y MCP para cuantificar las fibras nerviosas corneales. Densidad de IENF (IENFD), densidad de ramas, y longitud de las ramas mostraron una reducci&#xF3;n progresiva al aumentar la gravedad de la neuropat&#xED;a, que fue significativa en pacientes con neuropat&#xED;a leve, moderada y severa. CCM tambi&#xE9;n mostr&#xF3; una reducci&#xF3;n progresiva de la densidad de fibras nerviosas de la c&#xF3;rnea (CNFD) y la densidad de la rama, pero este &#xFA;ltimo se redujo significativamente, incluso en pacientes diab&#xE9;ticos sin neuropat&#xED;a. Tanto IENFD y CNFD correlacion&#xF3; significativamente con la detecci&#xF3;n de fr&#xED;o y calor, como el umbral del dolor. Longitudes de los nervios corneales y intraepid&#xE9;rmicas de fibra se reduce en pacientes con dolor en comparaci&#xF3;n con la neuropat&#xED;a diab&#xE9;tica dolorosa. Tanto IENF y la evaluaci&#xF3;n de CCM cuantificar con precisi&#xF3;n los da&#xF1;os peque&#xF1;a de fibras nerviosas en pacientes diab&#xE9;ticos. Sin embargo, CCM cuantifica da&#xF1;os en la fibra peque&#xF1;a r&#xE1;pida y no invasiva y detectar las primeras etapas de da&#xF1;o a los nervios en comparaci&#xF3;n con IENF patolog&#xED;a. The commonest cause of peripheral neuropathy is diabetes and pain occurs in approximately 30% of diabetic patients with neuropathy. Corneal confocal microscopy is a novel clinical technique for the study of corneal cellular structure. It provides images which are comparable to in-vitro histochemical techniques delineating corneal epithelium, Bowman's layer, stroma, Descemet's membrane and the corneal endothelium. Because, corneal confocal microscopy is a non invasive technique for in vivo imaging of the living cornea it has huge clinical potential to investigate numerous corneal diseases. Thus far it has been used in the detection and management of pathologic and infectious conditions, corneal dystrophies and ecstasies, monitoring contact lens induced corneal changes and for pre and post surgical evaluation (PRK, LASIK and LASEK, flap evaluations and Radial Keratotomy), and penetrating keratoplasty. Most recently it has been used as a surrogate for peripheral nerve damage in a variety of peripheral neuropathies and may have potential in acting as a surrogate marker for endothelial abnormalities.

Porque, la microscop&#xED;a confocal corneal es una t&#xE9;cnica no invasiva para la visualizaci&#xF3;n in vivo de la c&#xF3;rnea vida que tiene un enorme potencial cl&#xED;nico para investigar numerosas enfermedades de la c&#xF3;rnea. Patients with idiopathic small fibre neuropathy (ISFN) have been shown to have significant intraepidermal nerve fibre loss and an increased prevalence of impaired glucose tolerance (IGT). It has been suggested that the dysglycemia of IGT and additional metabolic risk factors may contribute to small nerve fibre damage in these patients. Twenty-five patients with ISFN and 12 aged-matched control subjects underwent a detailed evaluation of neuropathic symptoms, neurological deficits (Neuropathy deficit score (NDS); Nerve Conduction Studies (NCS); Quantitative Sensory Testing (QST) and Corneal Confocal Microscopy (CCM)) to quantify small nerve fibre pathology. Eight (32%) patients had IGT. Whilst all patients with ISFN had significant neuropathic symptoms, NDS, NCS and QST except for warm thresholds were normal. Corneal sensitivity was reduced and CCM demonstrated a significant reduction in corneal nerve fibre density (NFD) (P<0.0001), nerve branch density (NBD) (P<0.0001), nerve fibre length (NFL) (P<0.0001) and an increase in nerve fibre tortuosity (NFT) (P<0.0001). However these parameters did not differ between ISFN patients with and without IGT, nor did they correlate with BMI, lipids and blood pressure. Corneal confocal microscopy provides a sensitive non-invasive means to detect small nerve fibre damage in patients with ISFN and metabolic abnormalities do not relate to nerve damage.

Patienten mit idiopathischer kleine Faser Neuropathie (ISFN) nachweislich erhebliche Intraepidermales Nerv Faser Verlust und eine erh&ouml;hte Pr&auml;valenz von gest&ouml;rte Glukosetoleranz (IGT) haben. Neuropathy is a cause of significant disability in patients with Fabry disease, yet its diagnosis is difficult. In this study we compared the novel noninvasive techniques of corneal confocal microscopy (CCM) to quantify small-fiber pathology, and non-contact corneal aesthesiometry (NCCA) to quantify loss of corneal sensation, with established tests of neuropathy in patients with Fabry disease. Ten heterozygous females with Fabry disease not on enzyme replacement therapy (ERT), 6 heterozygous females, 6 hemizygous males on ERT, and 14 age-matched, healthy volunteers underwent detailed quantification of neuropathic symptoms, neurological deficits, neurophysiology, quantitative sensory testing (QST), NCCA, and CCM. All patients with Fabry disease had significant neuropathic symptoms and an elevated Mainz score. Peroneal nerve amplitude was reduced in all patients and vibration perception threshold was elevated in both male and female patients on ERT. Cold sensation (CS) threshold was significantly reduced in both male and female patients on ERT (P < 0.02), but warm sensation (WS) and heat-induced pain (HIP) were only significantly increased in males on ERT (P < 0.01). However, corneal sensation assessed with NCCA was significantly reduced in female (P < 0.02) and male (P < 0.04) patients on ERT compared with control subjects. According to CCM, corneal nerve fiber and branch density was significantly reduced in female (P < 0.03) and male (P < 0.02) patients on ERT compared with control subjects. Furthermore, the severity of neuropathic symptoms and the neurological component of the Mainz Severity Score Index correlated significantly with QST and CCM. This study shows that CCM and NCCA provide a novel means to detect early nerve fiber damage and dysfunction, respectively, in patients with Fabry disease.

Corneal confocal microscopy: a novel noninvasive means to diagnose neuropathy in patients with Fabry disease.

La neuropathie est une cause d&#x26;#39;invalidit&#xE9; significative chez les patients atteints maladie de Fabry, mais son diagnostic est difficile. Dans cette &#xE9;tude nous avons compar&#xE9; les techniques non invasives de nouveaux microscopie confocale corn&#xE9;enne (CCM) afin de quantifier les petites fibres de la pathologie et non-contact aesthesiometry la corn&#xE9;e (NCCA) pour quantifier la perte de la sensibilit&#xE9; corn&#xE9;enne, avec des tests &#xE9;tablis de la neuropathie chez les patients atteints de la maladie de Fabry. Dix femelles h&#xE9;t&#xE9;rozygotes atteintes de la maladie de Fabry ne figurant pas sur la th&#xE9;rapie de remplacement enzymatique (TRE), 6 femelles h&#xE9;t&#xE9;rozygotes, 6 hommes h&#xE9;mizygotes sur ERT, et 14 appari&#xE9;s selon l&#x26;#39;&#xE2;ge, des volontaires sains a subi une quantification d&#xE9;taill&#xE9;e des sympt&#xF4;mes de neuropathie, les d&#xE9;ficits neurologiques, de la neurophysiologie, des tests quantitatifs sensorielle (TVQ ), NCCA, et CCM. Tous les patients atteints de la maladie de Fabry a eu d&#x26;#39;importants sympt&#xF4;mes neuropathiques et un score &#xE9;lev&#xE9; Mayence. Amplitude nerf sciatique poplit&#xE9; externe a &#xE9;t&#xE9; r&#xE9;duite chez tous les patients et seuil de perception des vibrations a &#xE9;t&#xE9; &#xE9;lev&#xE9; dans les deux patients de sexe masculin et f&#xE9;minin sur l&#x26;#39;ERT. Froide sensation (CS) de seuil a &#xE9;t&#xE9; significativement r&#xE9;duite chez les patients &#xE0; la fois masculins et f&#xE9;minins sur ERT (P &#x26;lt;0,02), mais une sensation de chaleur (WS) et de la chaleur induite par la douleur (hanche) ont &#xE9;t&#xE9; seulement de mani&#xE8;re significative chez les m&#xE2;les sur l&#x26;#39;ERT (P &#x26;lt;0,01) . Cependant, la sensibilit&#xE9; corn&#xE9;enne &#xE9;valu&#xE9;e par NCCA a &#xE9;t&#xE9; significativement r&#xE9;duit chez les femelles (P &#x26;lt;0,02) et masculin (p &#x26;lt;0,04) chez les patients ERT par rapport aux sujets t&#xE9;moins. Selon CCM, des fibres nerveuses de la corn&#xE9;e et la densit&#xE9; de branche a &#xE9;t&#xE9; significativement r&#xE9;duit chez les femelles (P &#x26;lt;0,03) et masculin (p &#x26;lt;0,02) chez les patients ERT par rapport aux sujets t&#xE9;moins. En outre, la s&#xE9;v&#xE9;rit&#xE9; des sympt&#xF4;mes neuropathiques et la composante neurologique de l&#x26;#39;indice de Mayence Severity Score corr&#xE9;lation significative avec la TVQ et la CCM. Corn&#xE9;e microscopie confocale: un nouveau moyen non invasives pour diagnostiquer la neuropathie chez les patients atteints de la maladie de Fabry

Neuropathie ist eine Ursache f&uuml;r erhebliche Behinderung bei Patienten mit Fabry-Krankheit, doch seine Diagnose ist schwierig. In dieser Studie verglichen wir die neuartigen nicht-invasive Techniken der Hornhaut confocal Mikroskopie (CCM) und klein-Faser Pathologie quantifizieren und ber&uuml;hrungslose Hornhaut Aesthesiometry (NCCA), Verlust der Hornhaut Sensation, mit etablierten Tests der Neuropathie bei Patienten mit Fabry-Krankheit zu quantifizieren. Zehn heterozygote Frauen mit Fabry-Krankheit nicht auf die Enzymersatztherapie (ERT), 6 heterozygote Frauen und 6 Hemizygous M&auml;nner auf ERT und 14 altersbezogenen, gesunde Freiwillige unterzog sich genaue Quantifizierung der neuropathische Symptome, neurologische Defizite, Neurophysiologie, quantitative sensorische Pr&uuml;fung (QST), NCCA und CCM. Alle Patienten mit Fabry-Krankheit hatte erhebliche neuropathische Symptome und eine erh&ouml;hte Mainz-Ergebnis. Peroneal Nerv Amplitude wurde bei allen Patienten reduziert und Vibration Wahrnehmung Schwellenwert wurde in m&auml;nnliche und weibliche Patienten auf ERT erhoben. Kalte Empfindung (CS) Schwellenwert wurde deutlich reduziert, sowohl m&auml;nnliche als auch weibliche Patienten auf ERT (P &lt; 0,02), aber warme Empfindung (WS) und Hitze-induzierte Schmerzen (HIP) wurden nur deutlich erh&ouml;ht, bei M&auml;nnern auf ERT (P &lt; 0,01). Nach CCM, Hornhaut Nerve Fiber und Branch Dichte wurde deutlich reduziert, bei weiblichen (P &lt; 0,03) und m&auml;nnlich (P &lt; 0,02) Patienten auf ERT im Vergleich zu Kontrollpersonen. Au&szlig;erdem korreliert der Schweregrad der neuropathische Symptome und die neurologische Komponente des Mainzer Severity Score Index deutlich mit QST und CCM. Diese Studie zeigt, dass CCM und NCCA bedeutet ein Roman bieten um fr&uuml;hen Nerve-Fiber-Sch&auml;den und St&ouml;rungen, bzw. bei Patienten mit Fabry-Krankheit zu erkennen.

La neuropatia &egrave; una causa di invalidit&agrave; significativa nei pazienti con malattia di Fabry, ma la sua diagnosi &egrave; difficile. Ampiezza del nervo peroneo &egrave; stato ridotto in tutti i pazienti e la soglia di percezione di vibrazioni &egrave; stata elevata nei pazienti di sesso maschili e femminili su ERT. Soglia di sensazione di freddo (CS) &egrave; stato significativamente ridotto nei pazienti sia maschi che femminili su ERT (P &lt; 0.02), ma calda sensazione (WS) e dolore indotta da calore (anca) sono stati solo significativamente aumentati nei maschi su ERT (P &lt; 0,01). Tuttavia, sensazione corneale valutata con NCCA era significativamente ridotto nella femmina (P &lt; 0,02) e maschile (P &lt; 0,04) pazienti su ERT rispetto ai soggetti di controllo. La neuropat&#xED;a es una causa importante de discapacidad en pacientes con enfermedad de Fabry, sin embargo, su diagn&#xF3;stico es dif&#xED;cil. En este estudio se compararon las nuevas t&#xE9;cnicas no invasivas de la microscop&#xED;a confocal de la c&#xF3;rnea (CCM) para cuantificar la peque&#xF1;a de fibra de patolog&#xED;a, y de no contacto corneal aesthesiometry (NCCA) para cuantificar la p&#xE9;rdida de la sensibilidad de la c&#xF3;rnea, con las pruebas establecidas de la neuropat&#xED;a en los pacientes con enfermedad de Fabry. Diez mujeres heterocigotas con enfermedad de Fabry no est&#xE1;n en terapia de reemplazo enzim&#xE1;tico (TRE), 6 mujeres, 6 varones heterocigotos hemicigotos en TRE, y 14 de la misma edad, los voluntarios sanos se sometieron cuantificaci&#xF3;n detallada de los s&#xED;ntomas neurop&#xE1;ticos, d&#xE9;ficits neurol&#xF3;gicos, la neurofisiolog&#xED;a, la prueba sensorial cuantitativa (QST ), NCCA, y CCM. Todos los pacientes con enfermedad de Fabry ten&#xED;a una participaci&#xF3;n importante los s&#xED;ntomas neurop&#xE1;ticos y una elevada puntuaci&#xF3;n de Maguncia. La amplitud del nervio peroneo se redujo en todos los pacientes y el umbral de percepci&#xF3;n de las vibraciones se elevan en los pacientes masculinos y femeninos en la ERT. Sensaci&#xF3;n de fr&#xED;o (CS) de umbral se redujo significativamente en los pacientes de ambos sexos en la ERT (P &#x26;lt;0,02), pero sensaci&#xF3;n de calor (WS) y el dolor inducido por el calor (HIP) s&#xF3;lo fueron significativamente mayor en los hombres en TRE (P &#x26;lt;0,01) . Sin embargo, la sensibilidad de la c&#xF3;rnea evaluada con NCCA se redujo significativamente en las mujeres (P &#x26;lt;0,02) y varones (p &#x26;lt;0,04) en pacientes con ERT en comparaci&#xF3;n con los sujetos control.
