We report the outcomes of 24 patients with high-risk hematologic malignancies or bone marrow failure (BMF) who received haploidentical bone marrow transplantation (BMT) after ex vivo induction of alloantigen-specific anergy in donor T cells by allostimulation in the presence of costimulatory blockade. Ninety-five percent of evaluable patients engrafted and achieved full donor chimerism. Despite receiving a median T-cell dose of 29 x10(6)/kg, only 5 of 21 evaluable patients developed grade C (n = 4) or D (n = 1) acute graft-versus-host disease (GVHD), with only one attributable death. Twelve patients died from treatment-related mortality (TRM). Patients reconstituted T-cell subsets and immunoglobulin levels rapidly with evidence of in vivo expansion of pathogen-specific T cells in the early posttransplantation period. Five patients reactivated cytomegalovirus (CMV), only one of whom required extended antiviral treatment. No deaths were attributable to CMV or other viral infections. Only 1 of 12 evaluable patients developed chronic GVHD. Eight patients survive disease-free with normal performance scores (median follow-up, 7 years). Thus, despite significant early TRM, ex vivo alloanergization can support administration of large numbers of haploidentical donor T cells, resulting in rapid immune reconstitution with very few viral infections. Surviving patients have excellent performance status and a low rate of chronic GVHD.

Riportiamo i risultati di 24 pazienti con neoplasie ematologiche ad alto rischio o insufficienza del midollo osseo (BMF) che hanno ricevuto praticare il trapianto di midollo osseo (BMT) dopo ex vivo induzione di anergia alloantigen specifici nelle cellule del donatore T da allostimulation in presenza di blocco di costimolazione. Novantacinque per cento dei pazienti evaluable trapiantati e raggiunto il chimerismo donatore completo. Nonostante la ricezione di una dose di T-cellula mediana di 29 x 6 / kg, solo 5 dei 21 pazienti evaluable sviluppato grado C (n = 4) o D (n = 1) acuta graft - versus - host disease (GVHD), con la sola morte attribuibili. Dodici pazienti sono deceduti da mortalit&agrave; correlati al trattamento (TRM). Se presentan los resultados de 24 pacientes con alto riesgo de neoplasias hematol&#xF3;gicas o insuficiencia de la m&#xE9;dula &#xF3;sea (BMF), que recibi&#xF3; haploid&#xE9;ntico trasplante de m&#xE9;dula &#xF3;sea (TMO), despu&#xE9;s de la inducci&#xF3;n ex vivo de aloant&#xED;genos espec&#xED;ficos de anergia en las c&#xE9;lulas T del donante por allostimulation en presencia de bloqueo co-estimuladoras . Allogeneic hematopoietic stem-cell transplantation can cure some patients with high-risk B-cell malignancies, but disease relapse following transplantation remains a significant problem. One approach that could be used to augment the donor T-cell-mediated antitumor effect is the infusion of allogeneic donor-derived T cells expressing a chimeric antibody receptor (CAR) specific to the B-cell antigen CD19. However, the use of such cells might result in toxicity in the form of graft-versus-host disease mediated by CD19-specific (CD19-CAR) T cells possessing alloreactive endogenous T-cell receptors. We therefore investigated whether nonalloreactive tumor-specific human T cells could be generated from peripheral blood mononuclear cells of healthy donors by the combination of CD19 redirection via CAR expression and subsequent alloanergization by allostimulation and concomitant blockade of CD28-mediated costimulation. Alloanergization of CD19-CAR T cells resulted in efficient and selective reduction of alloresponses in both CD4(+) and CD8(+) T cells, including allospecific proliferation and cytokine secretion. Importantly, T-cell effector functions including CAR-dependent proliferation and specific target cytolysis and cytokine production were retained after alloanergization. Our data support the application of CD19 redirection and subsequent alloanergization to generate allogeneic donor T cells for clinical use possessing increased antitumor activity but limited capacity to mediate graft-versus-host disease. Immunotherapy with such cells could potentially reduce disease relapse after allogeneic transplantation without increasing toxicity, thereby improving the outcome of patients undergoing allogeneic transplantation for high-risk B-cell malignancies.

Abbiamo quindi valutato se le cellule T nonalloreactive tumore specifico umane potrebbero essere generate da cellule mononucleate di sangue periferico di donatori sani dalla combinazione di CD19 reindirizzamento tramite auto espressione e successive alloanergization da allostimulation e concomitante blocco di CD28-mediata costimulation. Cellule Alloanergization di CD19-auto T ha provocato riduzione efficiente e selettiva di alloresponses nelle cellule sia CD4(+) e cellule T, compresi allospecific secrezione di proliferazione e citochine. Soprattutto, sono state mantenute funzioni effettrici delle cellule T compreso auto-dipendente proliferazione e destinazione specifica citolisi e cytokine produzione dopo alloanergization. I nostri dati supportano l&#39;applicazione di redirezione CD19 e successive alloanergization per generare le cellule del donatore allogenico T per uso clinico che possiede maggiore attivit&agrave; antitumorale, ma limitata capacit&agrave; di mediare graft - versus - host disease. Immunoterapia con tali cellule potrebbe potenzialmente ridurre recidiva di malattia dopo trapianto allogenico senza aumentare la tossicit&agrave;, migliorando cos&igrave; il risultato di pazienti sottoposti a trapianto allogenico per tumori maligni delle cellule B ad alto rischio.

Combinando CD19 redirezione e alloanergization per generare cellule T umane di tumore-specifici per la terapia dei tumori maligni delle cellule B cellule allogeniche.

Alog&#xE9;nico de c&#xE9;lulas madre hematopoy&#xE9;ticas de trasplante puede curar a algunos pacientes con alto riesgo de c&#xE9;lulas B malignas, pero recidiva de la enfermedad despu&#xE9;s de un trasplante sigue siendo un problema significativo. Un enfoque que podr&#xED;a ser utilizado para aumentar el donante de c&#xE9;lulas T mediada por efecto antitumoral es la infusi&#xF3;n de alog&#xE9;nicas derivadas del donante de c&#xE9;lulas T que expresan un receptor de anticuerpo quim&#xE9;rico (CAR) espec&#xED;fico para el ant&#xED;geno de c&#xE9;lulas B CD19. Sin embargo, el uso de tales c&#xE9;lulas podr&#xED;a dar lugar a toxicidad en la forma de la enfermedad de injerto contra hu&#xE9;sped mediada por CD19-espec&#xED;fica (CD19-CAR) las c&#xE9;lulas T que poseen alorreactivas end&#xF3;genos receptores de c&#xE9;lulas T. Por lo tanto, se investig&#xF3; si nonalloreactive espec&#xED;ficos del tumor las c&#xE9;lulas T humanas podr&#xED;a ser generado a partir de c&#xE9;lulas mononucleares de sangre perif&#xE9;rica de donantes sanos por la combinaci&#xF3;n de CD19 a trav&#xE9;s de la redirecci&#xF3;n de expresi&#xF3;n CAR y alloanergization posterior por allostimulation y bloqueo concomitante de la coestimulaci&#xF3;n CD28 mediada. Alloanergization de las c&#xE9;lulas CD19-CAR T result&#xF3; en una reducci&#xF3;n eficaz y selectiva de alloresponses en ambos CD4 (+) y CD8 (+) las c&#xE9;lulas T, incluyendo la proliferaci&#xF3;n aloespec&#xED;ficas y secreci&#xF3;n de citoquinas. Es importante destacar que las c&#xE9;lulas T efectoras funciones incluyendo CAR-dependiente de la proliferaci&#xF3;n y la cit&#xF3;lisis objetivo espec&#xED;fico y la producci&#xF3;n de citoquinas fueron retenidos despu&#xE9;s de alloanergization. Nuestros datos apoyan la aplicaci&#xF3;n de la redirecci&#xF3;n y CD19 alloanergization posterior para generar linfocitos T alog&#xE9;nicos de donantes para el uso cl&#xED;nico que poseen actividad antitumoral mayor, pero la limitada capacidad de mediar del injerto contra el hu&#xE9;sped.
