Growth factor gene transfer-induced therapeutic angiogenesis has become a novel approach for the treatment of myocardial ischemia. In order to provide a basis for the clinical application of an adenovirus with hepatocyte growth factor gene (Ad-HGF) in the treatment of myocardial ischemia, we established a minipig model of chronically ischemic myocardium in which an Ameroid constrictor was placed around the left circumflex branch of the coronary artery (LCX). A total of 18 minipigs were randomly divided into 3 groups: a surgery control group, a model group and an Ad-HGF treatment group implanted with Ameroid constrictor. Ad-HGF or the control agent was injected directly into the ischemic myocardium, and an improvement in heart function and blood supply were evaluated. The results showed that myocardial perfusion remarkably improved in the Ad-HGF group compared with that in both the control and model groups. Four weeks after the treatment, the density of newly formed blood vessels was higher and the number of collateral blood vessels was greater in the Ad-HGF group than in the model group. The area of myocardial ischemia reduced evidently and the left ventricular ejection fraction improved significantly in the Ad-HGF group. These results suggest that HGF gene therapy may become a novel approach in the treatment of chronically ischemic myocardium.

Treatment of chronical myocardial ischemia by adenovirus-mediated hepatocyte growth factor gene transfer in minipigs.

G&egrave;ne facteur de croissance induite par le transfert d&#39;angiogen&egrave;se th&eacute;rapeutique est devenue une nouvelle approche pour le traitement de l&#39;isch&eacute;mie myocardique. Afin de fournir une base pour l&#39;application clinique d&#39;un ad&eacute;novirus avec g&egrave;ne de facteur de croissance des h&eacute;patocytes (Ad-HGF) dans le traitement de l&#39;isch&eacute;mie myocardique, nous avons &eacute;tabli un mod&egrave;le miniature du myocarde isch&eacute;mique chronique dans lequel un constrictor Am&eacute;ro&iuml;de a &eacute;t&eacute; plac&eacute; autour de la branche circonflexe gauche de l&#39;art&egrave;re coronaire (LCX). Un total de 18 porcs miniatures ont &eacute;t&eacute; r&eacute;partis au hasard en 3 groupes : un groupe de contr&ocirc;le de la chirurgie, un groupe de mod&egrave;les et un groupe de traitement Ad-HGF implant&eacute; avec Am&eacute;ro&iuml;de constrictor. Ad-HGF ou l&#39;agent de contr&ocirc;le a &eacute;t&eacute; inject&eacute; directement dans le myocarde isch&eacute;mique, et une am&eacute;lioration de l&#39;approvisionnement en sang et la fonction coeur ont &eacute;t&eacute; &eacute;valu&eacute;s. Wachstumsfaktor gen induziert durch &Uuml;bertragung therapeutische Angiogenese ist ein neuartiger Ansatz zur Behandlung von myokardialer Ischemia geworden. Um eine Grundlage f&uuml;r die klinische Anwendung der ein Adenovirus mit Hepatozyten-Wachstumsfaktor-gen (Ad-HGF) in der Behandlung von myokardialer Ischemia zu bieten, haben wir ein Minipig-Modell der chronisch isch&auml;mischen Myokard, in dem ein Ameroid Constrictor um den linken circumflex Zweig der Koronararterien (LCX) gebracht wurde. Insgesamt 18 Minipigs wurden nach dem Zufallsprinzip in drei Gruppen aufgeteilt: eine Kontrollgruppe Chirurgie, eine Modellgruppe und ein Ad-HGF-Behandlungsgruppe mit Ameroid Constrictor implantiert. Anzeige-HGF oder der Control Agent direkt in isch&auml;mischen Myokard injiziert wurde und eine Verbesserung der Herz-Funktion und Blut-Versorgung wurden ausgewertet. Die Ergebnisse zeigten, dass myokardiale Perfusion bemerkenswert verbessert in der Ad-HGF-Gruppe gegen&uuml;ber, die in die Kontrolle und die Modell-Gruppen. Vier Wochen nach der Behandlung die Dichte des neu gebildeten Blutgef&auml;&szlig;e h&ouml;her war und die Zahl der Kollateralsch&auml;den Blutgef&auml;&szlig;e in der Ad-HGF-Gruppe als in der Modellgruppe gr&ouml;&szlig;er war. Bereich myokardialer Ischemia offenbar reduziert und die linke ventrikul&auml;re Ejektionsfraktion verbesserte sich deutlich in der Ad-HGF-Gruppe. Diese Ergebnisse legen nahe, dass sich HGF-Gentherapie entwickelt einen neuartigen Ansatz in der Behandlung von chronisch isch&auml;mischen Myokard.

Fattore di crescita gene transfer-indotta angiogenesi terapeutica &egrave; diventato un nuovo approccio per il trattamento dell&#39;ischemia del miocardio. Un totale di 18 minipigs casualmente sono stati suddivisi in 3 gruppi: un gruppo di controllo di chirurgia, un modello e un gruppo Ad-HGF trattamento impiantato con Ameroid constrictor. Annuncio-HGF o l&#39;agente di controllo &egrave; stato iniettato direttamente nel miocardio ischemico, e un miglioramento nella fornitura di funzione e sangue di cuore sono stati valutati. I risultati hanno mostrato che perfusione miocardica notevolmente migliorata nel gruppo di annunci-HGF rispetto a quella in gruppi sia il controllo e il modello. La disminuci&#xF3;n anormal o la falta de suministro de ox&#xED;geno a las c&#xE9;lulas y los tejidos se denomina hipoxia. Esta condici&#xF3;n se ve com&#xFA;nmente en diversas enfermedades como la artritis reumatoide y la aterosclerosis, tambi&#xE9;n en c&#xE1;nceres s&#xF3;lidos. Los estudios precl&#xED;nicos y cl&#xED;nicos han demostrado que los c&#xE1;nceres de hipoxia son extremadamente agresivos, resistente a las terapias est&#xE1;ndar (quimioterapia y radioterapia), y por lo tanto muy dif&#xED;cil de erradicar. La hipoxia afecta tanto al tumor y las c&#xE9;lulas del sistema inmune a trav&#xE9;s de diversas v&#xED;as.
