Nontypeable Haemophilus influenzae (NTHi) is a mucosal pathogen that is a major cause of respiratory infection, including sinusitis, otitis media and bronchitis. This bacterium has evolved a number of mechanisms to facilitate its survival in the human host. Recently it has been recognized that it is capable of intracellular survival in monocytes/macrophages and epithelial cells. Previous work by the authors has demonstrated that the protective response to NTHi is Th1 predominant. This information led to the hypothesis that the intracellular survival of NTHi in human monocytes may be reduced by two key effector mechanisms of Th1-mediated immunity: interferon gamma and ligation of CD40. This study assessed the effect of interferon gamma and ligation of CD40 on the intracellular survival of NTHi in human monocytes. Responses were studied in monocytes from subjects with bronchiectasis and persistent airway infection with NTHi and compared with control subjects. The results demonstrated that different isolates of NTHi were able to survive inside monocytes. Killing of one strain of NTHi could be enhanced by the addition of interferon gamma and CD40 ligation in both control and bronchiectasis subjects. Other strains were more resistant.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) is characterized by pulmonary inflammation that persists after the cessation of smoking. T cells have a major role in driving inflammation in patients with COPD and are activated by specific antigens to produce mediators, such as cytokines. The antigens that activate lung T cells have not been clearly defined. Nontypeable Haemophilus influenzae (NTHi) is the dominant bacterium isolated from the lungs of patients with COPD. Objective: We sought to measure the response of lung tissue T cells to stimulation with NTHi. METHODS: We obtained lung tissue from 69 subjects having lobectomies for lung cancer. Of the group, 39 subjects had COPD, and 30 without COPD were classified as control subjects. The lung tissue was dispersed into single-cell suspensions and stimulated with live NTHi. Cells were labeledÂ with antibodies for 5 important inflammatory mediators in patients with COPD and analyzed by using flow cytometry. RESULTS: NTHi produced strong activation of both T(H) cells and cytotoxic T cells in the COPD cohort. The COPD cohort had significantly higher levels of cells producing TNF-Î±, IL-13, and IL-17 in both T-cell subsets. When control subjects were divided into those with and without a significant smoking history and compared with patients with COPD, there was a progressive increase in the numbers of T cells producing cytokines from nonsmoking control subjects to smoking control subjects to patients with COPD. CONCLUSION: NTHi activates lung T cells in patients with COPD. This proinflammatory profibrotic response might be a key cause of inflammation in patients with COPD and has implications for treatment.

Lung T-cell responses to nontypeable Haemophilus influenzae in patients with chronic obstructive pulmonary disease.

Rappel des faits : La maladie pulmonaire obstructive chronique (MPOC) est caract&eacute;ris&eacute;e par une inflammation pulmonaire qui persiste apr&egrave;s la cessation du tabagisme. Les lymphocytes T jouent un r&ocirc;le important dans la conduite de l&#39;inflammation chez les patients atteints de MPOC et sont activ&eacute;s par des antig&egrave;nes sp&eacute;cifiques pour produire des m&eacute;diateurs, tels que les cytokines. Les antig&egrave;nes qui activent les cellules pulmonaires T n&#39;ont pas &eacute;t&eacute; clairement d&eacute;finis. Nontypeable Haemophilus influenzae (polysaccharides) est la bact&eacute;rie dominante isol&eacute;e dans les poumons des personnes atteintes de MPOC. Objectif : Nous avons cherch&eacute; &agrave; mesurer la r&eacute;ponse des cellules de T de tissu pulmonaire &agrave; une stimulation avec des polysaccharides. SFONDO: Malattia polmonare ostruttiva cronica (BPCO) &egrave; caratterizzata da infiammazione polmonare che persiste dopo la cessazione del fumo. Le cellule T hanno un ruolo fondamentale nel guidare l&#39;infiammazione nei pazienti con BPCO e vengono attivate da antigeni specifici per la produzione di mediatori, come citochine. CONCLUSIONE: NTHi attiva le cellule T del polmone in pazienti con BPCO. FONDO: La enfermedad pulmonar obstructiva cr&oacute;nica (EPOC) se caracteriza por la inflamaci&oacute;n pulmonar que persiste tras el cese del h&aacute;bito de fumar. Las c&eacute;lulas T tienen un papel importante en la conducci&oacute;n de la inflamaci&oacute;n en pacientes con EPOC y se activan por ant&iacute;genos espec&iacute;ficos para producir mediadores, como las citocinas. Los ant&iacute;genos que activan las c&eacute;lulas del pulm&oacute;n T no han sido claramente definidos. Nontypeable Haemophilus influenzae (NTHi) es la bacteria dominante aislada de los pulmones de pacientes con EPOC. Objetivo: Intentamos medir la respuesta de las c&eacute;lulas de T del tejido pulmonar a la estimulaci&oacute;n con NTHi. M&Eacute;TODOS: Se obtuvieron el tejido pulmonar de 69 temas teniendo lobectomies por c&aacute;ncer de pulm&oacute;n. Del grupo, 39 sujetos ten&iacute;an EPOC, y 30 sin EPOC fueron clasificados como sujetos de control. El tejido pulmonar fue dispersado en suspensiones unicelular y estimulado con NTHi vivo. Las c&eacute;lulas fueron etiquetadas con anticuerpos para 5 importantes mediadores inflamatorios en pacientes con EPOC y analizadas por mediante citometr&iacute;a de flujo. Resultados: NTHi produjo fuerte activaci&oacute;n de c&eacute;lulas t y las c&eacute;lulas T citot&oacute;xicas en la cohorte de EPOC. La cohorte de EPOC ten&iacute;a niveles significativamente m&aacute;s altos de c&eacute;lulas productoras de TNF-&alpha; Il-&alpha;-13 y IL-17 en dos subconjuntos de c&eacute;lulas T. Cuando sujetos de control se divide en aquellos con y sin antecedentes de tabaquismo importante y en comparaci&oacute;n con los pacientes con EPOC, hubo un aumento progresivo del n&uacute;mero de c&eacute;lulas T, produciendo citocinas de control se somete a temas de control de fumar a los pacientes con EPOC. CONCLUSI&Oacute;N: NTHi activa las c&eacute;lulas T del pulm&oacute;n en pacientes con EPOC. Esta respuesta de citosinas proinflamatoria pudo ser una causa clave de la inflamaci&oacute;n en pacientes con EPOC y tiene implicaciones para el tratamiento.

Respuestas de c&eacute;lulas T de pulm&oacute;n a no tipificable Haemophilus influenzae en pacientes con enfermedad pulmonar obstructiva cr&oacute;nica.

Nontypeable Haemophilus influenzae (NTHi) is a prevalent bacterium found in a variety of chronic respiratory diseases. The role of this bacterium in the pathogenesis of lung inflammation is not well defined. In this study we examined the effect of NTHi on two important lung inflammatory processes 1), oxidative stress and 2), protease expression. Bronchoalveolar macrophages were obtained from 121 human subjects, blood neutrophils from 15 subjects, and human-lung fibroblast and epithelial cell lines from 16 subjects. Cells were stimulated with NTHi to measure the effect on reactive oxygen species (ROS) production and extracellular trap formation. We also measured the production of the oxidant, 3-nitrotyrosine (3-NT) in the lungs of mice infected with this bacterium. NTHi induced widespread production of 3-NT in mouse lungs. This bacterium induced significantly increased ROS production in human fibroblasts, epithelial cells, macrophages and neutrophils; with the highest levels in the phagocytic cells. In human macrophages NTHi caused a sustained, extracellular production of ROS that increased over time. The production of ROS was associated with the formation of macrophage extracellular trap-like structures which co-expressed the protease metalloproteinase-12. The formation of the macrophage extracellular trap-like structures was markedly inhibited by the addition of DNase. In this study we have demonstrated that NTHi induces lung oxidative stress with macrophage extracellular trap formation and associated protease expression. DNase inhibited the formation of extracellular traps.

Nontypeable Haemophilus influenzae induces sustained lung oxidative stress and protease expression.

The aim of this study was to determine if natural killer cell number (CD3 /CD16 /CD56 ) and cytotoxic killing function predicts severity and frequency of infection in kidney transplant recipients. A cohort of 168 kidney transplant recipients with stable graft function underwent assessment of natural killer cell number and functional killing capacity immediately prior to entry into this prospective study. Participants were followed for 2 years for development of severe infection, defined as hospitalization for infection. Area under receiver operating characteristic (AUROC) curves were used to evaluate the accuracy of natural killer cell number and function for predicting severe infection. Adjusted odds ratios were determined by logistic regression. Fifty-nine kidney transplant recipients (35%) developed severe infection and 7 (4%) died. Natural killer cell function was a better predictor of severe infection than natural killer cell number: AUROC 0.84 and 0.75, respectively (P = .018). Logistic regression demonstrated that after adjustment for age, transplant function, transplant duration, mycophenolate use, and increasing natural killer function (odds ratio [OR] 0.82, 95% confidence interval [CI] 0.74-0.90; P < .0001) but not natural killer number (OR 0.96, 95% CI 0.93-1.00; P = .051) remained significantly associated with a reduced likelihood of severe infection. Natural killer cell function predicts severe infection in kidney transplant recipients.
