The naive phenotype of cord blood (CB) T cells may reduce graft-versus-host disease after umbilical cord blood transplantation, but this naivety and their low absolute numbers also delays immune reconstitution, producing higher infection-related mortality that is predominantly related to CMV, adenovirus (Adv), and EBV. Adoptive immunotherapy with peripheral blood-derived virus-specific cytotoxic T lymphocytes (CTLs) can effectively prevent viral disease after conventional stem cell transplantation, and we now describe the generation of single cultures of CTLs from CB that are specific for multiple viruses. Using EBV-infected B cells transduced with a clinical-grade Ad5f35CMVpp65 adenoviral vector as sources of EBV, Adv, and CMV antigens, we expanded virus-specific T cells even from CB T cells with a naive phenotype. After expansion, each CTL culture contained both CD8(+) and CD4(+) T-cell subsets, predominantly of effector memory phenotype. Each CTL culture also had HLA-restricted virus-specific cytotoxic effector function against EBV, CMV, and Adv targets. The CB CTLs recognized multiple viral epitopes, including CD4-restricted Adv-hexon epitopes and immunosubdominant CD4- and CD8-restricted CMVpp65 epitopes. Notwithstanding their naive phenotype, it is therefore possible to generate trivirus-specific CTLs in a single culture of CB, which may be of value to prevent or treat viral disease in CB transplant recipients. Viral infection or reactivation remains a major cause of morbidity and mortality after allogeneic stem cell transplantation. We now show that infusions of single cytotoxic T lymphocyte (CTL) lines (5 x 10(6)-1.35 x 10(8) cells/m(2)) with specificity for 2 commonly detected viruses, Epstein-Barr virus (EBV) and adenovirus, can be safely administered to pediatric transplantation recipients receiving partially human leukocyte antigen-matched and haploidentical stem cell grafts (n = 13), without inducing graft-versus-host disease. The EBV-specific component of the CTLs expanded in vivo and persisted for more than 12 weeks, but the adenovirus-specific component only expanded in vivo in the presence of concomitant adenoviral infection. Nevertheless, adenovirus-specific T cells could be detected for at least 8 weeks in peripheral blood, even in CTL recipients without viral infection, provided the adenovirus-specific component of their circulating lymphocytes was first expanded by exposure to adenoviral antigens ex vivo. After infusion, none of these 13 high-risk recipients developed EBV-associated lymphoproliferative disease, while 2 of the subjects had resolution of their adenoviral disease. Hence, bispecific CTLs containing both EBV- and adenovirus-specific T cells can safely reconstitute an antigen responsive "memory" population of CTLs after human leukocyte antigen-mismatched stem cell transplantation and may provide antiviral activity. Viral infections and leukemic relapse account for the majority of treatment failures in patients with B-cell acute lymphoblastic leukemia (B-ALL) receiving allogeneic hematopoietic stem cell (HSC) or cord blood (CB) transplants. Adoptive transfer of virus-specific cytotoxic T lymphocytes (CTLs) provides protection against common viruses causing serious infections after HSC transplantation without concomitant graft-versus-host disease. We have now generated CTL lines from peripheral blood (PB) or CB units that recognize multiple common viruses and provide antileukemic activity by transgenic expression of a chimeric antigen receptor (CAR) targeting CD19 expressed on B-ALL. PB-derived CAR(+) CTLs produced interferon-gamma (IFNgamma) in response to cytomegalovirus-pp65, adenovirus-hexon, and Epstein-Barr virus pepmixes (from 205 +/- 104 to 1034 +/- 304 spot-forming cells [SFCs]/10(5) T cells) and lysed primary B-ALL blasts in (51)Cr-release assays (mean, 66% +/- 5% specific lysis; effector-target [E/T] ratio, 40:1) and the CD19(+) Raji cell line (mean, 78% +/- 17%) in contrast to nontransduced controls (8% +/- 8% and 3% +/- 2%). CB-derived CAR(+) CTLs showed similar antiviral and antitumor function and both PB and CB CAR(+) CTLs completely eliminated B-ALL blasts over 5 days of coculture. This approach may prove beneficial for patients with high-risk B-ALL who have recently received an HSC or CB transplant and are at risk of infection and relapse.

Statin treatment has been shown to reduce graft-versus-host disease while preserving graft-versus-tumor effect in allogeneic stem cell transplantation. Herein, we investigated whether lovastatin treatment affects the function of human cytolytic T lymphocytes (CTLs). Upon T-cell receptor stimulation, lovastatin significantly inhibited the proliferation of both CD4+ and CD8+ T cells from healthy donors whereas their intracellular cytokine production including interferon-Î³ and tumor necrosis factor-Î± remained the same with a slight decrease of interleukin-2. Moreover, the specific lysis of target cells by CTL lines derived from patients and normal donors specific for Epstein-Barr virus-encoded antigen latent membrane protein-2 or cytomegalovirus-encoded antigen pp65 was uncompromised in the presence of lovastatin. In addition, we evaluated the effect of lovastatin on the proliferation and effector function of the CD8+ tumor-infiltrating lymphocytes (TILs) derived from melanoma patients specific for MART-1 antigen. Lovastatin significantly reduced the expansion of antigen-specific TILs upon MART-1 stimulation. However, the effector function of TILs, including the specific lysis of target cells and secretion of cytokine interferon-Î³, remained intact with lovastatin treatment. Taken together, these data demonstrated that lovastatin inhibits the proliferation of Epstein-Barr virus, cytomegalovirus, and MART-1-specific CTLs without affecting cytolytic capacity. The differential effect of lovastatin on the proliferation versus cytotoxicity of CTLs might shed some light on elucidating the possible mechanisms of graft-versus-host disease and graft-versus-tumor effect elicited by alloimmune responses.

Glioblastoma (GBM) is the most common primary brain cancer in adults and is virtually incurable. Recent studies have shown that cytomegalovirus (CMV) is present in majority of GBMs. To evaluate whether the CMV antigens pp65 and IE1, which are expressed in GBMs, could be targeted by CMV-specific T cells, we measured the frequency of T cells targeting pp65 and IE1 in the peripheral blood of a cohort of 11 sequentially diagnosed CMV-seropositive GBM patients, and evaluated whether it was feasible to expand autologous CMV-specific T cells for future clinical studies. All 11 CMV-seropositive GBM patients had T cells specific for pp65 and IE1 in their peripheral blood assessed by IFNÎ³ enzyme-linked immunospot assay. However, the precursor frequency of pp65-specific T cells was decreased in comparison with healthy donors (P=0.001). We successfully reactivated and expanded CMV-specific T cells from 6 out of 6 GBM patients using antigen-presenting cells transduced with an adenoviral vector encoding pp65 and IE1. CMV-specific T-cell lines contained CD4 as well as CD8 T cells, recognized pp65 and IE1 targets and killed CMV-infected autologous GBM cells. Infusion of such CMV-specific T-cell lines may extend the benefits of T-cell therapy to patients with CMV GBMs.

Generation of Polyclonal CMV-specific T Cells for the Adoptive Immunotherapy of Glioblastoma.

Le glioblastome (GBM) est le cancer du cerveau primaire la plus fr&#xE9;quente chez les adultes et il est pratiquement incurable. Des &#xE9;tudes r&#xE9;centes ont montr&#xE9; que le cytom&#xE9;galovirus (CMV) est pr&#xE9;sent dans la majorit&#xE9; des GBM. Pour &#xE9;valuer si les antig&#xE8;nes de CMV pp65 et IE1, qui sont exprim&#xE9;es dans les glioblastomes multiformes, pourraient &#xEA;tre cibl&#xE9;s par le CMV des cellules T sp&#xE9;cifiques, nous avons mesur&#xE9; la fr&#xE9;quence des cellules T ciblant pp65 et IE1 dans le sang p&#xE9;riph&#xE9;rique d&#x26;#39;une cohorte de 11 s&#xE9;quentiellement diagnostiqu&#xE9; CMV- patients atteints de GBM s&#xE9;ropositifs, et a &#xE9;valu&#xE9; si il &#xE9;tait possible d&#x26;#39;&#xE9;largir autologue CMV des cellules T sp&#xE9;cifiques pour de futures &#xE9;tudes cliniques. Toutefois, la fr&#xE9;quence des pr&#xE9;curseurs des lymphocytes T sp&#xE9;cifiques pp65-a diminu&#xE9; en comparaison avec les donneurs sains (P = 0,001). Nous avons r&#xE9;ussi &#xE0; r&#xE9;activer et &#xE9;largir le CMV des cellules T sp&#xE9;cifiques de 6 sur 6 patients atteints de GBM en utilisant des cellules pr&#xE9;sentatrices d&#x26;#39;antig&#xE8;ne transduites avec un vecteur ad&#xE9;noviral codant pp65 et IE1. CMV-sp&#xE9;cifiques des cellules T CD4 lignes contenues ainsi que les cellules CD8 T, reconnue pp65 et IE1 cibles et tu&#xE9; infect&#xE9;es par le CMV cellules autologues GBM. G&#xE9;n&#xE9;ration d&#x26;#39;polyclonaux CMV des cellules T sp&#xE9;cifiques de l&#x26;#39;immunoth&#xE9;rapie adoptive de glioblastome

Glioblastom (GBM) ist der h&auml;ufigste prim&auml;re Gehirntumoren bei Erwachsenen und praktisch unheilbar. J&uuml;ngste Studien haben gezeigt, dass das Zytomegalievirus (CMV) in der Mehrheit der GBMs vorhanden ist. Um zu bewerten, ob die CMV Antigene pp65 und IE1, die in GBMs ausgedr&uuml;ckt werden, durch CMV-spezifische T-Zellen ausgerichtet sein k&ouml;nnte, haben wir die H&auml;ufigkeit der T-Zellen gezielt pp65 gemessen und IE1 im peripheren Blut eine Kohorte von 11 sequenziell CMV-seropositiven GBM-Patienten diagnostiziert und bewertet, sei es m&ouml;glich, autologe CMV-spezifische T-Zellen f&uuml;r zuk&uuml;nftige klinische Studien zu erweitern. Alle 11 CMV-seropositiven GBM-Patienten hatte T-Zellen spezifisch f&uuml;r pp65 und IE1 im peripheren Blut von IFN&gamma; Enzym-Immunospot Assay bewertet. Jedoch wurde die Vorl&auml;ufer-Frequenz von pp65-spezifische T-Zellen im Vergleich zu gesunden Spender gesenkt (P = 0,001). Wir erfolgreich reaktiviert und erweitert CMV-spezifische T-Zellen aus 6 von 6-GBM-Patienten, die Antigen-pr&auml;sentierenden Zellen transduzierten mit einer adenoviral Vektor pp65 und IE1-Codierung verwenden. CMV-spezifische T-Zell-Linien enthalten CD4, CD8 T-Zellen, anerkannte pp65 und IE1-Zielen sowie und CMV-infizierten autologe GBM-Zellen get&ouml;tet. Infusion von solchen CMV-spezifische T-Zell-Linien kann die Vorteile der T-Zell-Therapie f&uuml;r Patienten mit CMV GBMs verl&auml;ngern.

Recenti studi hanno dimostrato che citomegalovirus (CMV) &egrave; presente nella maggioranza dei GBMs. Per valutare se il CMV pp65 antigeni e IE1, che sono espressi in GBMs, potrebbe essere mirati da parte delle cellule T specifiche per CMV, abbiamo misurato la frequenza di cellule T targeting pp65 e IE1 nel sangue periferico di una coorte di 11 sequenzialmente diagnosticato i pazienti sieropositivi CMV GBM e valutato se era fattibile per espandere autologhe cellule T CMV-specifico per futuri studi clinici. Tutti i pazienti sieropositivi CMV GBM 11 avevano T cellule specifiche per pp65 e IE1 nel loro sangue periferico valutata mediante saggio enzyme-linked immunospot IFN&gamma;. Tuttavia, la frequenza di precursori di cellule T specifiche per pp65 &egrave; stata diminuita rispetto ai donatori sani (P = 0,001). Abbiamo correttamente riattivato ed espanso cellule T specifiche per CMV da 6 di 6 pazienti con GBM utilizzando cellule presentanti l&#39;antigene transduced con un vettore adenovirale codifica pp65 e IE1. CMV-specifiche della T-cellula conteneva CD4 cos&igrave; come le cellule T CD8, riconosciuto pp65 e obiettivi IE1 e ucciso le cellule infettate da CMV autologhe GBM. Infusione di tali linee di cellule T specifiche per CMV possa estendere i benefici della terapia della T-cellula a pazienti con CMV GBMs.

El glioblastoma (GBM) es el c&#xE1;ncer cerebral primario m&#xE1;s com&#xFA;n en adultos y es pr&#xE1;cticamente incurable. Estudios recientes han demostrado que el citomegalovirus (CMV) est&#xE1; presente en la mayor&#xED;a de GBMs. Para evaluar si los ant&#xED;genos de CMV pp65 y IE1, que se expresan en GBMs, podr&#xED;an ser el blanco de CMV espec&#xED;ficos de las c&#xE9;lulas T, que mide la frecuencia de c&#xE9;lulas T dirigidas pp65 y IE1 en la sangre perif&#xE9;rica de un grupo de 11 diagnosticados de forma secuencial por CMV pacientes seropositivos GBM, y evaluaron si era factible ampliar el CMV aut&#xF3;logo de c&#xE9;lulas T espec&#xED;ficas para futuros estudios cl&#xED;nicos. Todos los 11 pacientes seropositivos para CMV GBM ten&#xED;an c&#xE9;lulas T espec&#xED;ficas para pp65 y IE1 en la sangre perif&#xE9;rica evaluada por el ensayo de IFN inmunoensayo enzim&#xE1;tico. Sin embargo, la frecuencia de precursores de la pp65 c&#xE9;lulas T espec&#xED;ficas se redujo en comparaci&#xF3;n con los donantes sanos (p = 0,001).
