Resident muscularis macrophages initiate an inflammatory cascade during ischemia/reperfusion that is associated with dysmotility and the activation of immunologic processes. We hypothesized that these muscularis macrophages may also play a potential immunologic role for acute allograft rejection in intestinal transplantation. Orthotopic SBTx (BN-Lew) was performed without immunosuppression. Animals were sacrificed 7 days after SBTx. The role of resident macrophages was evaluated by transplantation of macrophage-depleted and gadolinium chloride-treated gut. Leukocyte infiltration was investigated in muscularis whole mounts by immunohistochemistry. Mediator mRNA expression was determined by Real-Time-RT-PCR. Apoptosis was evaluated by TUNEL. Smooth muscle contractility was assessed in a standard organ bath. In comparison to vehicle-treated grafts, macrophage-depleted grafts exhibited significantly lower mediator mRNA peak expression (IL-6, IL-2, IL-10, MCP-1, iNOS, TNFalpha, IFNgamma, FasL), leukocyte infiltrates (ED1- and ED2 positive monocytes and macrophages, neutrophils, CD4(+) and CD8(+) lymphocytes), apoptosis rates and an improved histologic rejection grading. Vehicle-treated grafts showed a 77% decrease in smooth muscle contractility compared to naÃ¯ve controls, while macrophage-depleted gut exhibited only a 51% decrease in contractile activity. Transplantation of macrophage-depleted gut attenuates the functionally relevant molecular and cellular immunologic response within the graft muscularis in acute allograft rejection. Resident macrophages participate in initiating these processes.

Role of resident macrophages in the immunologic response and smooth muscle dysfunction during acute allograft rejection after intestinal transplantation.

Fahrzeug-behandelten Transplantate zeigten eine 77 % Abnahme der Kontraktilit&auml;t der glatten Muskulatur gegen&uuml;ber naiv-Steuerelemente, w&auml;hrend Makrophage-abgereichertem Darm nur eine 51 % Abnahme der kontraktilen Aktivit&auml;t ausgestellt. Transplantation von Makrophagen-abgereichertem Gut d&auml;mpft die funktionell relevante Molekulare und zellul&auml;re immunologische Reaktion innerhalb der Transplantat-Muscularis in akuten Allograft Ablehnung. Abbiamo ipotizzato che questi macrofagi muscolare possono anche giocare un ruolo potenziale immunologico rigetto acuto di allotrapianto nel trapianto intestinale. Innesti veicolo-trattati hanno mostrato un calo del 77% nel muscolo liscio contrattilit&agrave; rispetto ai controlli na&iuml;ve, mentre macrofago-impoverito intestino esposto solo il 51% di diminuzione nell&#39;attivit&agrave; contrattile. Trapianto di intestino macrofago-esaurito attenua la risposta funzionalmente rilevante molecolare e cellulare immunologica all&#39;interno dell&#39;innesto muscolare nel rigetto acuto di allotrapianto. Los macr&oacute;fagos residentes muscularis inician una cascada inflamatoria durante la isquemia/reperfusi&oacute;n asociado dismotilidad y la activaci&oacute;n de procesos inmunol&oacute;gicos. Presumimos que estos macr&oacute;fagos muscularis tambi&eacute;n pueden desempe&ntilde;ar un papel inmunol&oacute;gico potencial rechazo agudo del aloinjerto en trasplante intestinal. Orthotopic SBTx (BN-Lew) fue realizada sin inmunosupresi&oacute;n. Los animales fueron sacrificados 7 d&iacute;as despu&eacute;s de SBTx. Se evalu&oacute; el papel de los macr&oacute;fagos residentes por trasplante de macr&oacute;fago-agotado y los intestinos tratados con cloruro de gadolinio. Infiltraci&oacute;n de leucocitos fue investigada en montajes todo muscular por inmunohistoqu&iacute;mica. Expresi&oacute;n del mRNA de mediador se determin&oacute; por Real-Time-RT-PCR. Apoptosis fue evaluada por el TUNEL. Se evalu&oacute; la contractilidad del m&uacute;sculo liso en un ba&ntilde;o de &oacute;rgano est&aacute;ndar. En comparaci&oacute;n con los tratados con veh&iacute;culo injertos, injertos de macr&oacute;fago-agotado exhiben menor expresi&oacute;n del pico de mRNA de mediador (IL-6, IL-2, IL-10, MCP-1, iNOS, TNFalpha, IFNgamma, FasL), infiltrados del leucocito (ED1 - y ED2 positivos los monocitos y macr&oacute;fagos, neutr&oacute;filos, CD4 y CD8 linfocitos), &iacute;ndices de apoptosis y un rechazo histologic mejor clasificaci&oacute;n. Injertos de veh&iacute;culo tratados mostraron una disminuci&oacute;n del 77% en la contractilidad del m&uacute;sculo liso comparado con controles no tratados con anterioridad, mientras que el macr&oacute;fago-agotado el intestino hab&iacute;a expuesto s&oacute;lo una disminuci&oacute;n del 51% en la actividad contr&aacute;ctil. Trasplante de intestino macr&oacute;fago-agotado aten&uacute;a la respuesta inmunol&oacute;gica celular y molecular funcionalmente relevante dentro de los muscularis del injerto en el rechazo agudo del aloinjerto. Macr&oacute;fagos residentes participan en la iniciaci&oacute;n de estos procesos.

Significant improvements of graft and patient survival have been achieved over the past 20 years in the field of intestinal transplantation. Tacrolimus monotherapy with corticosteroids, or in combination with sirolimus is the most commonly used immunosuppressive regimen. Early (24h) after experimental allogenic small bowel transplantation in rats, sirolimus reduces the cellular and molecular inflammatory response with subsequent graft dysmotility more efficiently than tacrolimus, with contrary effects at 7 days after transplantation. This study evaluates three immunosuppressive strategies in the post-acute phase after intestinal transplantation - tacrolimus or sirolimus monotherapy and the combination therapy of tacrolimus with infliximab. After orthotopic intestinal transplantation between Brown Norway and Lewis rats, animals received 14 days of immunosuppressive treatment. Histology, infiltration of neutrophils, monocytes and macrophages, cytokine and mediator mRNA expression (real time RT-PCR) and smooth muscle function in a standard organ bath were assessed at 14 days after transplantation in all treatment groups and isogenic controls. Allogenic transplanted rats without immunosuppressive therapy and non-transplanted animals served as further control. Tacrolimus prevented acute rejection and graft dysmotility more effectively (p â‰¥ 0.05) than sirolimus. Reduced mRNA expression levels of CD4, IFN-Î³, IL-6, IL-10, iNOS, NFÎºB, TNF-Î± and MCP-1 (p â‰¤ 0.05) were observed in tacrolimus treated animals compared to sirolimus. Additional infliximab application did not influence the cellular and molecular inflammatory response in the post-acute phase after transplantation. In conclusion, the severe cellular and molecular inflammatory response in allogenic transplanted grafts without immunosuppressive therapy is ameliorated by all three immunosuppressive regimens, but tacrolimus was found to be more efficient than sirolimus at 14 days after transplantation. Our findings do not rule out the usage of sirolimus as single immunosuppressive therapy, but indicate and confirm the potency and effectivity of tacrolimus as basis immunosuppressant in the field of intestinal transplantation.

Fr&uuml;he (24 Stunden) nach der experimentellen allogene D&uuml;nndarm-Transplantation bei Ratten, reduziert Sirolimus die zellul&auml;re und Molekulare entz&uuml;ndliche Reaktion mit nachfolgenden Transplantat Dysmotility effizienter als Tacrolimus, mit gegenteilige Wirkungen bei 7 Tage nach der Transplantation. Diese Studie wertet drei immunsuppressive Strategien in der post-acute Phase nach der Darm-Transplantation - Tacrolimus oder Sirolimus Monotherapie und Kombinationstherapie von Tacrolimus mit Infliximab. Nach Orthotopic intestinale Transplantation zwischen Brown Norwegen und Lewis-Ratten erhielt Tiere 14 Tagen immunsuppressive Behandlung. Histologie, Infiltration von Neutrophilen, Monozyten und Makrophagen, Cytokine und Mediator mRNA Ausdruck (Real-Time RT-PCR) und glatten Muskel-Funktion in einem standard-Orgel-Bad wurden 14 Tage nach der Transplantation in allen Behandlungsgruppen und isogene Kontrollen beurteilt. Ridotti livelli di espressione di mRNA di CD4, IFN-&gamma; IFN-&gamma;-6, IL-10, iNOS, NF&kappa;B, TNF-&alpha; e MCP-1 (p &le; 0,05) sono stati osservati in tacrolimus animali trattati rispetto al sirolimus. Applicazione aggiuntiva infliximab non ha influenzato la risposta infiammatoria cellulare e molecolare in fase acuta dopo trapianto. In conclusione, la severa risposta infiammatoria cellulare e molecolare nel trapianto allogenico innesti trapiantati senza terapia immunosoppressiva &egrave; migliorata da tutti i tre regimi immunosoppressivi, ma tacrolimus &egrave; stato trovato per essere pi&ugrave; efficiente di sirolimus a 14 giorni dopo il trapianto. Se han logrado mejoras significativas de injerto y supervivencia de los pacientes en los &uacute;ltimos 20 a&ntilde;os en el campo del trasplante intestinal. Monoterapia con tacrolimus con corticosteroides, o en combinaci&oacute;n con sirolimus es el r&eacute;gimen inmunosupresor m&aacute;s utilizado. Temprana (24 horas) despu&eacute;s del trasplante de intestino al&oacute;geno experimental en ratas, sirolimus reduce la respuesta inflamatoria celular y molecular con dismotilidad posterior injerto m&aacute;s eficientemente que el tacrolimus, con efectos contrarios a los 7 d&iacute;as despu&eacute;s del trasplante. Este estudio eval&uacute;a tres estrategias inmunosupresoras en la fase post aguda despu&eacute;s del trasplante intestinal - monoterapia con tacrolimus o sirolimus y la terapia de combinaci&oacute;n de tacrolimus con infliximab. Despu&eacute;s del trasplante intestinal de orthotopic entre marr&oacute;n Noruega y las ratas de Lewis, los animales recibieron 14 d&iacute;as del tratamiento inmunosupresor. Histolog&iacute;a, infiltraci&oacute;n de neutr&oacute;filos, monocitos y macr&oacute;fagos, citocinas y expresi&oacute;n del mRNA de mediador (RT-PCR en tiempo real) y funci&oacute;n del m&uacute;sculo liso en un ba&ntilde;o de &oacute;rgano est&aacute;ndar se evaluaron a los 14 d&iacute;as despu&eacute;s del trasplante en todos los grupos de tratamiento y controles isog&eacute;nicas. Ratas trasplantadas al&oacute;genos sin terapia inmunosupresiva y animales trasplantados no sirven como control. Tacrolimus prevenido dismotilidad injerto y rechazo agudo m&aacute;s con eficacia (p &ge; 0,05) que sirolimus. Reduce los niveles de expresi&oacute;n de mRNA de CD4, IFN-&gamma; IFN-&gamma;-6, IL-10, iNOS, NF&kappa;B, TNF-&alpha; y MCP-1 (p &le; 0.05) fueron observados en tacrolimus animales tratados en comparaci&oacute;n con sirolimus. Aplicaci&oacute;n de infliximab adicional no influy&oacute; en la respuesta inflamatoria celular y molecular en la fase post aguda despu&eacute;s del trasplante. En conclusi&oacute;n, la respuesta inflamatoria celular y molecular severa en injertos al&oacute;genos trasplantados sin terapia inmunosupresiva es mejorada por todos tres reg&iacute;menes inmunosupresores, pero tacrolimus fue encontrado para ser m&aacute;s eficiente que sirolimus en 14 d&iacute;as despu&eacute;s del trasplante. Nuestros resultados no descartan el uso de sirolimus como &uacute;nico tratamiento inmunosupresor, pero indican y confirmar la potencia y efectividad de tacrolimus como inmunosupresor de base en el campo del trasplante intestinal.

OBJECTIVE: To assess trainee satisfaction in their surgery residency with a validated instrument and identify the contributing factors. BACKGROUND: Currently, surgery is deemed unattractive by medical students and ignored by many candidates planning to enter an academic career. New insights on the rational for such lack of interest are needed. Job satisfaction is a central concept in organizational and behavioral research that is well understood by large companies such as Google, IBM, and Toyota. Similar assessment can likewise be used to improve trainee satisfaction in surgery residency. METHODS: A survey among 2039 surgery residents was conducted in three European countries analyzing satisfaction at work using the Global Job Satisfaction Instrument (validated in Emergency Room physicians). Crucial factors covering different aspects of surgery residency where identified using the GJS instrument combined with multiple logistic regression analysis. RESULTS: With an overall response rate of 23%, we identified trainee dissatisfaction in one third of residents. Factors affecting satisfaction related almost exclusively to training issues, such as assignment of surgery procedures according to skills (OR 4.2), training courses (OR 2.7), availability of a structured training curriculum (OR 2.4), bedside teaching, and availability of morbidity-mortality conferences (OR 2.3). A good working climate among residents (OR 3.7) and the option for part time work (OR 2.1) were also significant factors for trainee satisfaction. Increased working hours had a modest (OR 0.98)-though cumulative- negative effect. The sex of the trainee was not related to trainee satisfaction. CONCLUSION: Validated measurement of job satisfaction as used in the industry appears to be an efficient tool to assess trainee satisfaction in surgery residency and thereby identify the key contributing factors. Improvement of conceptual training structures and working conditions might facilitate recruitment, decrease drop-out, and attract motivated candidates with possibly better quality of care.

SFONDO: Attualmente, chirurgia &egrave; ritenuto poco attraente per gli studenti di medicina e ignorato da molti candidati progettando di entrare in una carriera accademica. Sono necessarie nuove intuizioni sul razionale per tale mancanza di interesse. Soddisfazione sul lavoro &egrave; un concetto centrale nella ricerca organizzativa e comportamentale che &egrave; capito bene da grandi aziende come Google, IBM e Toyota. Valutazione simile allo stesso modo pu&ograve; essere utilizzato per migliorare la soddisfazione del tirocinante nella residenza di chirurgia. Metodi: Un sondaggio tra i residenti di chirurgia 2039 &egrave; stato condotto in tre paesi europei analizzando la soddisfazione sul lavoro utilizzando lo strumento di soddisfazione del lavoro globale (convalidato in pronto soccorso i medici). OBJETIVO: Evaluar la satisfacci&oacute;n del participante en su residencia en cirug&iacute;a con un instrumento validado e identificar los factores que contribuyen. FONDO: Actualmente, la cirug&iacute;a es considerada poco atractiva por los estudiantes de medicina e ignorada por muchos candidatos planea entrar en una carrera acad&eacute;mica. Se necesitan nuevas ideas en el racional para tal falta de inter&eacute;s. Satisfacci&oacute;n en el trabajo es un concepto central en investigaci&oacute;n organizacional y de comportamiento que se entiende por grandes empresas como Google, IBM y Toyota. Evaluaci&oacute;n similar puede utilizarse adem&aacute;s para mejorar la satisfacci&oacute;n del participante en la residencia de cirug&iacute;a. M&Eacute;TODOS: Se realiz&oacute; una encuesta entre los residentes de cirug&iacute;a 2039 en tres pa&iacute;ses europeos analizando la satisfacci&oacute;n en el trabajo con el instrumento de satisfacci&oacute;n Global de trabajo (validado en m&eacute;dicos de urgencias). Factores cruciales que cubren diferentes aspectos de la residencia de cirug&iacute;a donde identificados usando el instrumento GJS combinado con el an&aacute;lisis de regresi&oacute;n log&iacute;stica m&uacute;ltiple. Resultados: Con una tasa de respuesta global del 23%, hemos identificado insatisfacci&oacute;n de aprendiz en un tercio de los residentes. Factores que afectan la satisfacci&oacute;n relacionadas casi exclusivamente a cuestiones de formaci&oacute;n, como la asignaci&oacute;n de procedimientos de cirug&iacute;a seg&uacute;n aptitudes (o 4.2), cursos de formaci&oacute;n (o 2.7), la disponibilidad de un curr&iacute;culo de capacitaci&oacute;n estructurada (o 2,4), ense&ntilde;anza de cabecera y disponibilidad de conferencias de la morbi-mortalidad (o 2.3). Un buen clima de trabajo entre los residentes (o 3.7) y la opci&oacute;n para trabajo de tiempo parcial (o 2.1) tambi&eacute;n fueron factores importantes para la satisfacci&oacute;n del alumno. Aumento de horas de trabajo tuvieron un modesto (o 0.98)-aunque acumulado negativo efecto. El sexo de los alumnos no se relacion&oacute; con satisfacci&oacute;n del participante. CONCLUSI&Oacute;N: Medici&oacute;n validado de satisfacci&oacute;n laboral utilizada en la industria parece ser una herramienta eficaz para evaluar la satisfacci&oacute;n de aprendiz en la residencia de cirug&iacute;a y as&iacute; identificar la clave factores que contribuyen. Mejora de las estructuras conceptuales de la formaci&oacute;n y las condiciones de trabajo puede facilitar el reclutamiento, disminuir el abandono y atraer a candidatos motivados con posiblemente la mejor calidad de la atenci&oacute;n.

Satisfacci&oacute;n del participante en programas de residencia de cirug&iacute;a: herramientas modernas de gesti&oacute;n asegurar&aacute;n la motivaci&oacute;n del alumno y el &eacute;xito.

Intestinal transplantation initiates a functionally relevant inflammatory response by activation of resident macrophages within the muscularis associated with dysmotility. Infliximab is used successfully as a potent anti-inflammatory agent for the treatment of chronic inflammatory bowel diseases and as rescue therapy in acute steroid-resistant rejection in selected settings in clinical small bowel transplantation. We hypothesize that additional perioperative treatment with infliximab diminishes initiation of the inflammatory cascade and improves motility in small bowel grafts using a standard tacrolimus immunosuppressive protocol.

Combination therapy of tacrolimus and infliximab reduces inflammatory response and dysmotility in experimental small bowel transplantation in rats.

Transplantation intestinale initie une r&eacute;ponse inflammatoire fonctionnellement pertinente par l&#39;activation des macrophages r&eacute;sidents au sein de la musculeuse associ&eacute;e dysmotilit&eacute;. Infliximab est utilis&eacute; avec succ&egrave;s comme un puissant agent anti-inflammatoire pour le traitement des maladies inflammatoires chroniques de l&#39;intestin et comme traitement de secours dans des rejet aigu cortico-r&eacute;sistant dans les param&egrave;tres s&eacute;lectionn&eacute;s dans la transplantation clinique de l&#39;intestin gr&ecirc;le. Nous &eacute;mettons l&#39;hypoth&egrave;se que p&eacute;riop&eacute;ratoire suppl&eacute;mentaires le traitement par infliximab diminue l&#39;initiation de la cascade inflammatoire et am&eacute;liore la motilit&eacute; de l&#39;intestin gr&ecirc;le greffes utilisant un protocole immunosuppresseur tacrolimus standard.

Virtual reality devices are becoming the backbone for laparoscopic training in surgery. However, without knowledge of the achievable metrics of basic training within the trainee group, these simulators cannot be used effectively.
