Hemimegalencephaly (HME) is a developmental abnormality of the central nervous system, identified by an abnormal increase in the size of one cerebral hemisphere. HME may present as either a syndromic or isolated case. To date the literature on HME has focused primarily on non-fetal pediatric patients, largely related to surgical resection specimens of the HME hemisphere. We present the case of a male fetus at 22 weeks gestation with intracranial abnormalities identified on a follow-up ultrasound. Gross examination of the fetal brain confirmed the increased size of the right cerebral hemisphere. The ipsilateral brain stem and cerebellum were not involved. Light microscopy demonstrated the presence of accelerated cortical differentiation along with several migrational anomalies in the HME hemisphere. Based on the gross and microscopic findings, a diagnosis of fetal hemimegalencephaly was made. The periventricular proliferative zone of the abnormal hemisphere contained a normal population of neuroepithelial precursor cells. An exhaustive immunohistochemical study found immunoreactivity for calretinin and synaptophysin, while the Ki-67 proliferation labeling was not increased in the HME hemisphere. Our case is the first autopsied report on fetal hemimegalencephaly and confirms that the key pathogenic changes may present as early as 20-22 weeks gestation. The major pathological features of our case are in keeping with a disturbance in accelerated neuronal differentiation and migrational abnormalities.

H&eacute;mim&eacute;galenc&eacute;phalie (HME) est une anomalie du d&eacute;veloppement du syst&egrave;me nerveux central, identifi&eacute; par une augmentation anormale de la taille d&#39;un h&eacute;misph&egrave;re c&eacute;r&eacute;bral. HME peut se pr&eacute;senter comme un ou l&#39;autre cas de syndromique ou isol&eacute;es. &Agrave; ce jour de la documentation sur l&#39;HME se concentre principalement sur les patients p&eacute;diatriques non-foetales, en grande partie li&eacute;s &agrave; une ex&eacute;r&egrave;se chirurgicale de l&#39;h&eacute;misph&egrave;re de l&#39;HME. Nous pr&eacute;sentons le cas d&#39;un foetus de sexe masculin &agrave; 22 semaines de gestation avec intracr&acirc;niennes anomalies identifi&eacute;es sur une &eacute;chographie de suivi. Examen brut du cerveau foetal a confirm&eacute; l&#39;augmentation de la taille de l&#39;h&eacute;misph&egrave;re c&eacute;r&eacute;bral droit. Le homolat&eacute;ral du tronc c&eacute;r&eacute;bral et du cervelet ne participaient pas. Microscopie photonique ont d&eacute;montr&eacute; la pr&eacute;sence de diff&eacute;renciation de corticale acc&eacute;l&eacute;r&eacute;e ainsi que plusieurs anomalies de migration dans l&#39;h&eacute;misph&egrave;re de l&#39;HME. Se fondant sur les constatations des macroscopique et microscopiques, un diagnostic de h&eacute;mim&eacute;galenc&eacute;phalie f&oelig;tale a &eacute;t&eacute; effectu&eacute;. La zone prolif&eacute;rative p&eacute;riventriculaire de l&#39;h&eacute;misph&egrave;re anormale contenue une population normale de cellules pr&eacute;curseurs neuro&eacute;pith&eacute;liales. Une &eacute;tude immunohistochimique exhaustive trouv&eacute; immunor&eacute;activit&eacute; pour calretinin et synaptophysine, tandis que l&#39;&eacute;tiquetage de prolif&eacute;ration Ki-67 n&#39;a pas augment&eacute; dans l&#39;h&eacute;misph&egrave;re de l&#39;HME. Notre cas est le premier rapport autopsi&eacute; sur h&eacute;mim&eacute;galenc&eacute;phalie f&oelig;tale et confirme que les principaux changements pathog&egrave;nes peuvent pr&eacute;senter aussi t&ocirc;t que 20-22 semaines de gestation. Les principales caract&eacute;ristiques pathologiques de notre esp&egrave;ce concordent avec une perturbation de la diff&eacute;renciation neuronale acc&eacute;l&eacute;r&eacute;e et anomalies de la migration.

Hemimegalenzephalie (HME) ist eine Entwicklung des zentralen Nervensystems, gekennzeichnet durch eine abnorme Zunahme der Gr&ouml;&szlig;e einer zerebralen Hemisph&auml;re. HME kann als syndromale oder isolierten Fall vorlegen. Die Literatur &uuml;ber die HME bis heute konzentriert sich haupts&auml;chlich auf nicht-fetalen p&auml;diatrischen Patienten, weitgehend mit Bezug zu chirurgische Resektion Exemplare der HME Hemisph&auml;re. Pr&auml;sentieren wir Ihnen den Fall eines m&auml;nnlichen F&ouml;tus bei 23 Wochen Schwangerschaft mit intrakranielle Anomalien auf ein Follow-up Ultraschall identifiziert. Grobe Untersuchung des fetalen Gehirns best&auml;tigt die erh&ouml;hte Gr&ouml;&szlig;e der rechten zerebralen Hemisph&auml;re. Die ipsilaterale Hirnstamm und Kleinhirn waren nicht beteiligt. Lichtmikroskopie demonstrierte das Vorhandensein der beschleunigten kortikale Differenzierung zusammen mit mehreren migrantischen Anomalien in der HME-Hemisph&auml;re. Auf der Grundlage der Brutto- und mikroskopischen Ergebnisse erfolgte eine Diagnose von fetalen Hemimegalenzephalie. Die periventrikul&auml;re proliferative Zone der anormalen Hemisph&auml;re enthalten eine normale Bev&ouml;lkerung von Gliazellen Vorl&auml;uferzellen. Eine ersch&ouml;pfende Immunohistochemical Studie Immunoreactivity f&uuml;r Calretinin und Synaptophysin, w&auml;hrend der Ki-67-Vermehrung-Kennzeichnung in der HME-Hemisph&auml;re nicht erh&ouml;ht wurde. Unser Fall ist der erste Autopsie Bericht &uuml;ber fetale Hemimegalenzephalie und best&auml;tigt, dass die wichtigsten pathogenen Ver&auml;nderungen so fr&uuml;h wie 20-22 Wochen Schwangerschaft darstellen k&ouml;nnen. Pathologischen Haupteigenschaften von unserem Fall sind im Einklang mit einer St&ouml;rung im beschleunigten neuronale Differenzierung und migrantischen Fehlbildungen.

Hemimegalenzephalie: eine fetale Fall Neuropathologische Best&auml;tigung und &Uuml;berpr&uuml;fung der Literatur.

Emimegaencefalia (HME) &egrave; un&#39;anomalia dello sviluppo del sistema nervoso centrale, identificato da un anomalo aumento delle dimensioni di un emisfero cerebrale. HME pu&ograve; presentarsi come sia un caso isolato o sindromico. Ad oggi la letteratura sulla HME &egrave; concentrata principalmente su pazienti in et&agrave; pediatrica non fetali, in gran parte legati a campioni di resezione chirurgica dell&#39;emisfero HME. Presentiamo il caso di un feto maschio a 22 settimane di gestazione con anomalie intracraniche identificate su un follow-up ecografico. Esame lordo del cervello fetale ha confermato l&#39;aumento delle dimensioni dell&#39;emisfero cerebrale destro. Non sono stati coinvolti gli omolaterale del tronco cerebrale e del cervelletto. Microscopia chiara ha dimostrato la presenza di differenziazione corticale accelerata insieme a diverse anomalie migrational nell&#39;emisfero HME. Basata sui risultati lordi e microscopici, &egrave; stata fatta una diagnosi di emimegaencefalia fetale. La zona proliferativa periventricolare dell&#39;emisfero anormale conteneva una popolazione normale di cellule precursori neuroepithelial. Uno studio immunohistochemical esauriente trovato immunoreattivit&agrave; per calretinina e sinaptofisina, mentre l&#39;etichettatura di proliferazione Ki-67 &egrave; stato aumentato non nell&#39;emisfero HME. Nostro caso &egrave; il primo rapporto autopsied su emimegaencefalia fetale e conferma che le modifiche principali patogene possono presentare fin dal 20-22 settimane di gestazione. Principali caratteristiche patologiche del nostro caso sono in sintonia con un disturbo nella differenziazione neuronale accelerata e anomalie migrational.

Hemimegalencephaly (HME) es una anormalidad del desarrollo del sistema nervioso central, identificado por un aumento anormal en el tama&ntilde;o de un hemisferio cerebral. HME puede presentar como un caso aislado o sindr&oacute;mico. Hasta la fecha la literatura sobre HME ha centrado principalmente en pacientes pedi&aacute;tricos no fetales, en gran parte relacionadas con muestras de la resecci&oacute;n quir&uacute;rgica del hemisferio HME. Presentamos el caso de un feto masculino en 22 semanas de gestaci&oacute;n con anormalidades intracraneales identificadas en un ultrasonido de seguimiento. Examen macrosc&oacute;pico del cerebro fetal confirm&oacute; el aumento del tama&ntilde;o del hemisferio cerebral derecho. El ipsolateral tronco cerebral y el cerebelo no participaron. La microscopia ligera demostr&oacute; la presencia de diferenciaci&oacute;n acelerada cortical junto con varias anomal&iacute;as migratorias en el hemisferio HME. Basado en los resultados macrosc&oacute;pica y microsc&oacute;picos, se hizo un diagn&oacute;stico de hemimegalencephaly fetal. La zona proliferativa periventricular del hemisferio anormal conten&iacute;a una poblaci&oacute;n normal de c&eacute;lulas precursoras de neuroepiteliales. Un estudio exhaustivo immunohistochemical encontrado inmunoreactividad para calretinin y synaptophysin, mientras que el etiquetado de la proliferaci&oacute;n de Ki-67 no fue aumentado en el hemisferio HME. Nuestro caso es el primer informe de autopsia en hemimegalencephaly fetal y confirma que los principales cambios pat&oacute;genos pueden presentar tan pronto como 20-22 semanas de gestaci&oacute;n. Las caracter&iacute;sticas patol&oacute;gicas principales de nuestro caso est&aacute;n en consonancia con un disturbio en la diferenciaci&oacute;n neuronal acelerada y anormalidades campo-ciudad.

MGMT expression in tumors has been correlated with response to treatment with temozolomide therapy. Few medical therapies are available for Nelson syndrome, and the efficacy of such therapeutics remains limited. The aim of the present study was to assess immunohistochemical expression of MGMT in ACTH-secreting pituitary adenomas of patients with Nelson syndrome. Our material consisted of eight specimens from ACTH-secreting pituitary adenomas of patients with Nelson syndrome. Immunohistochemical staining for MGMT was performed using the streptavidin-biotin-peroxidase complex method. MGMT immunoreactivity was assessed microscopically and recorded as an estimated percentage of nuclear MGMT immunostaining (0 = none, 1=<10%, 2=<25%, 3=<50%, 4=>50%). Five of the eight specimens (65%) exhibited no MGMT immunoreactivity, with two out of eight cases (25%) showing slight MGMT staining (<10%) and one out of eight cases (12%) demonstrating moderate MGMT positivity (<25%). Patient male/female ratio was 3:5, with average patient age being 62.4 (range 57â€“66). Our findings suggest that temozolomide therapy may be of potential use in patients with Nelson syndrome, as these tumors express absent/low levels of MGMT. Absent or low MGMT staining in brain and other neoplasms has been shown to correlate with successful treatment with temozolomide, and recent reports of aggressive pituitary adenomas suggest similar outcomes.

MGMT Ausdruck in Tumoren hat mit ansprechen auf die Therapie mit Temozolomid Therapie aufeinander bezogen worden. Einige medizinische Therapien stehen f&uuml;r Nelson-Syndrom und die Wirksamkeit solcher Therapeutika bleibt begrenzt. Das Ziel der vorliegenden Studie war, Immunohistochemical Ausdruck MGMT in ACTH-sezernierende hypophys&auml;rer Adenome von Patienten mit Nelson-Syndrom. Unser Material bestand aus acht Proben von ACTH-sezernierende hypophys&auml;rer Adenome von Patienten mit Nelson-Syndrom. Immunhistochemische F&auml;rbung f&uuml;r MGMT erfolgte mit der Streptavidin-Biotin-Peroxidase-komplexe-Methode. MGMT-Immunoreactivity wurde mikroskopisch beurteilt und als gesch&auml;tzte Vomhundertsatz des nuklearen MGMT Paraffinschnitten (0 = aus, 1 = &lt; 10 %, 2 = &lt; 25 %, 3 = &lt; 50 %, 4-=&gt; 50 %). F&uuml;nf der acht Exemplare (65 %) zeigte keine MGMT-Immunoreactivity, mit zwei von acht F&auml;llen (25 %) zeigen leichte Verf&auml;rbung MGMT (&lt; (10 %) und einer von acht F&auml;llen (12 %) zeigen moderate MGMT Positivit&auml;t (&lt; (25 %). Immunoreattivit&agrave; MGMT &egrave; stato valutato microscopicamente e registrato come una percentuale stimata di immunostaining MGMT nucleare (0 = nessuna, 1 = &lt; 10%, 2 = &lt; 25%, 3 = &lt; 50%, 4 =&gt; 50%). Cinque degli otto esemplari (65%) non esposte nessuna immunoreattivit&agrave; MGMT, con due degli otto casi (25%) che mostrano lieve colorazione MGMT (&lt; 10%) e uno di otto casi (12%) dimostrando moderano positivit&agrave; MGMT (&lt; 25%). Expresi&oacute;n de MGMT en tumores se ha correlacionado con la respuesta al tratamiento con terapia de temozolomida. Algunos tratamientos m&eacute;dicos est&aacute;n disponibles para el s&iacute;ndrome de Nelson, y la eficacia de estas terapias sigue siendo limitada. El objetivo del presente estudio fue evaluar la expresi&oacute;n inmunohistoqu&iacute;mica de MGMT en adenomas pituitarios secretor de ACTH de pacientes con s&iacute;ndrome de Nelson. Nuestro material consisti&oacute; en ocho muestras de adenomas pituitarios secretor de ACTH de pacientes con s&iacute;ndrome de Nelson. La coloraci&oacute;n immunohistochemical para MGMT se realiz&oacute; mediante el m&eacute;todo complejo estreptavidina-biotina-peroxidasa. Inmunoreactividad MGMT se evaluaron microsc&oacute;picamente y registrado como un porcentaje estimado de inmunotinci&oacute;n nuclear de MGMT (0 = ninguna, 1 = &lt; 10%, 2 = &lt; 25%, 3 = &lt; 50%, 4 =&gt; 50%). Cinco de las ocho muestras (65%) no exhibieron inmunoreactividad MGMT, con dos de los ocho casos (25%) que muestran leve coloraci&oacute;n de MGMT (&lt; (10%) y uno de cada ocho casos (12%) que demuestran moderan positividad MGMT (&lt; (25%). Paciente raz&oacute;n hombre/mujer fue 3:5, con edad promedio de paciente siendo 62,4 (gama 57&ndash;66). Nuestros resultados sugieren que la terapia con temozolomida pueden de uso potencial en pacientes con s&iacute;ndrome de Nelson, como estos tumores expresan ausente bajo niveles de administraci&oacute;n ausente o MGMT baja coloraci&oacute;n en el cerebro y otras neoplasias ha demostrado correlacionar con tratamiento acertado con temozolomida y los informes recientes de adenomas pituitarios agresivos sugieren resultados similares.

Expresi&oacute;n inmunohistoqu&iacute;mica baja de MGMT en ACTH secretoras de tumores pituitarios de pacientes con s&iacute;ndrome de Nelson.

Tumori cerebrali congenite sono rari, contabilit&agrave; per 0,5% al 4% di tutti i tumori cerebrali pediatrici. Uno studio retrospettivo di 10 anni basato su autopsia e relazioni cliniche neurochirurgiche con una diagnosi di tumore cerebrale congenita/fetale/neonatale identificati 6 casi. Quattro casi sono stati diagnosticati antenatally di Neuroradiologia. I risultati clinici in 5 casi ha portato morte; 1 paziente con papilloma del plesso coroideo &egrave; ancora vivo e ha subito successo resezione del tumore. Patologie tumorali consistevano di 2 teratomi, 2 plesso coroideo papillomi, 1 gemistocytic astrocitoma e glioblastoma 1 multiforme. Inoltre &egrave; stata eseguita una revisione della letteratura di tutti i casi fetali specifici per le patologie presentate in questa serie. Relativa alla letteratura, questa serie contiene un raro caso di astrocitoma gemistocytic congenita. Ulteriormente questa serie mette in luce le differenze diagnostiche, istologiche, prognostiche e terapeutiche tra tumori cerebrali congenite e i tumori della stessa patologia in popolazioni pediatriche ed adulte pi&ugrave; anziani.

Tumores cerebrales cong&eacute;nitos son raros, contabilidad de 0.5 a 4% de todos los tumores cerebrales pedi&aacute;tricos. Un estudio retrospectivo de 10 a&ntilde;os basado en autopsia y neuroquir&uacute;rgicos informes cl&iacute;nicos con diagn&oacute;stico de tumor cerebral cong&eacute;nita/fetal/neonatal identificaron 6 casos. Cuatro casos fueron diagnosticados prenatal de Neurorradiolog&iacute;a. Los resultados cl&iacute;nicos en 5 casos resultaron en la muerte; 1 paciente con papiloma del plexo coroideo resecci&oacute;n exitosa del tumor y sigue vivo. Patolog&iacute;as del tumor consisti&oacute; en 2 teratomas, 2 papilomas del plexo coroides, 1 astrocitoma gemistoc&iacute;tico y 1 glioblastoma multiforme. Tambi&eacute;n se realiz&oacute; una revisi&oacute;n de la literatura de todos los casos fetales espec&iacute;ficas a las patolog&iacute;as que se presentan en esta serie. En relaci&oacute;n a la literatura, esta serie contiene un caso raro de astrocitoma gemistoc&iacute;tico cong&eacute;nita. Congenital brain tumors are rare, accounting for 0.5% to 4% of all pediatric brain tumors. A 10-year retrospective study based on autopsy and neurosurgical clinical reports with a diagnosis of congenital/fetal/neonatal brain tumor identified 6 cases. Four cases were diagnosed antenatally by neuroradiology. Clinical outcomes in 5 cases resulted in death; 1 patient with choroid plexus papilloma underwent successful resection of the tumor and is still alive. Tumor pathologies consisted of 2 teratomas, 2 choroid plexus papillomas, 1 gemistocytic astrocytoma, and 1 glioblastoma multiforme. A literature review of all fetal cases specific to the pathologies presented in this series was also performed. Relative to the literature, this series contains a rare case of congenital gemistocytic astrocytoma. This series further sheds light on the diagnostic, histological, prognostic, and therapeutic differences between congenital brain tumors and tumors of the same pathology in older pediatric and adult populations.

Les tumeurs c&eacute;r&eacute;brales cong&eacute;nitales sont rares, comptabilit&eacute; pour 0,5 % &agrave; 4 % de toutes les tumeurs de cerveau p&eacute;diatriques. Une &eacute;tude r&eacute;trospective de 10 ans fond&eacute;e sur l&#39;autopsie et des rapports cliniques neurochirurgies avec un diagnostic de tumeur c&eacute;r&eacute;brale cong&eacute;nitale/foetal et n&eacute;onatal 6 cas. Quatre cas ont &eacute;t&eacute; diagnostiqu&eacute;s avant la naissance de neuroradiologie. Les r&eacute;sultats cliniques dans 5 cas entra&icirc;n&eacute; la mort ; 1 patient avec des plexus choro&iuml;des a subi la r&eacute;section de la tumeur avec succ&egrave;s et est encore en vie. Pathologies tumorales se composait de 2 t&eacute;ratomes, 2 papillomes des plexus choro&iuml;des, 1 gemistocytic astrocytome et 1 glioblastome multiforme. Une recension de tous les cas foetales sp&eacute;cifiques pour les pathologies pr&eacute;sent&eacute;es dans cette s&eacute;rie a &eacute;galement &eacute;t&eacute; effectu&eacute;e. Par rapport &agrave; la litt&eacute;rature, cette s&eacute;rie contient un cas rare de l&#39;astrocytome gemistocytic cong&eacute;nitale. De plus, cette s&eacute;rie met en lumi&egrave;re les diff&eacute;rences de diagnostics histologiques, pronostiques et th&eacute;rapeutiques entre tumeurs c&eacute;r&eacute;brales cong&eacute;nitales et les tumeurs de la m&ecirc;me pathologie chez les populations p&eacute;diatriques et adultes plus &acirc;g&eacute;es.

Relativ zur Literatur, enth&auml;lt dieser Serie einen seltenen Fall von kongenitaler Gemistocytic Astrozytom. Diese Serie weiter wirft Licht auf die Diagnose, histologische, prognostische und therapeutische Unterschiede zwischen angeborenen Hirntumoren und Tumoren der gleichen Pathologie in &auml;lteren Kindern und Erwachsenen Bev&ouml;lkerungsgruppen.

Malignancies lacking specific features of cellular maturation are termed "undifferentiated" and represent 5-10% of all human tumors. They are encountered at a variety of sites but do not, as a rule, arise in the sellar region. A 39-year-old male with a history of testicular seminoma and an unsuccessful biopsy of a third ventricular neoplasm, presented with visual disturbances and memory loss. Light microscopically, the tumor consisted entirely of undifferentiated spindle cells. No germ cell component was noted. An exhaustive immunohistochemical study found immunoreactivity for vimentin and desmin, but for no other myoid markers. Polymerase chain reaction showed no X;18 translocation. Based upon these studies, a diagnosis of "undifferentiated sarcoma" was made. Our case, being highly unusual among reported sellar neoplasms, underscores the difficulties inherent in the differential diagnosis of undifferentiated neoplasms.

Polymerase chain reaction a montr&eacute; aucun X; 18 translocation. D&#39;apr&egrave;s ces &eacute;tudes, un diagnostic de &laquo; sarcome indiff&eacute;renci&eacute; &raquo; a &eacute;t&eacute; effectu&eacute;. Notre cas, &eacute;tant tr&egrave;s inhabituel chez les n&eacute;oplasmes sellar signal&eacute;s, souligne les difficult&eacute;s inh&eacute;rentes dans le diagnostic diff&eacute;rentiel des tumeurs indiff&eacute;renci&eacute;es.

Eine ersch&ouml;pfende Immunohistochemical Studie Immunoreactivity Vimentin und Desmin, aber keine anderen Myoid-Marker. Polymerase-Kettenreaktion zeigte keine X; 18 Translokation. Basierend auf diesen Studien, wurde die Diagnose eines &quot;undifferenzierte Sarkom&quot; gemacht. Unserem Fall wird h&ouml;chst ungew&ouml;hnlich unter gemeldeten sellar Tumoren, unterstreicht die Schwierigkeiten bei der Differentialdiagnose der undifferenzierten Tumoren.

The master regulatory gene Bmi1 modulates key stem cell properties in neural precursor cells (NPCs), and has been implicated in brain tumorigenesis. We previously identified a population of CD133+ brain tumor cells possessing stem cell properties, known as brain tumor initiating cells (BTICs). Here, we characterize the expression and role of Bmi1 in primary minimally cultured human glioblastoma (GBM) patient isolates in CD133+ and CD133- sorted populations. We find that Bmi1 expression is increased in CD133- cells, and Bmi1 protein and transcript expression are highest during intermediate stages of differentiation as CD133+ BTICs lose their CD133 expression. Furthermore, in vitro stem cell assays and Bmi1 knockdown show that Bmi1 contributes to self-renewal in CD133+ populations, but regulates proliferation and cell fate determination in CD133- populations. Finally, we test if our in vitro stem cell assays and Bmi1 expression in BTIC patient isolates are predictive of clinical outcome for GBM patients. Bmi1 expression profiles show a marked elevation in the proneural GBM subtype, and stem cell frequency as assessed by tumor sphere assays correlates with patient outcome.

Brain tumors are the leading cause of childhood cancer mortality, with medulloblastoma (MB) representing the most frequent malignant tumor. The recent molecular classification of MB has reconceptualized the heterogeneity that exists within pathological subtypes by giving context to the role of key developmental signaling pathways in MB pathogenesis. The identification of cancer stem cell (CSC) populations, termed brain tumor-initiating cells (BTICs), in MB has provided novel cellular targets for the study of these aberrantly activated signaling pathways, namely, Sonic hedgehog (Shh) and Wingless (Wnt), along with the identification of novel BTIC self-renewal pathways. In this review, we discuss recent evidence for the presence of a MB stem cell that drives tumorigenesis in this malignant childhood tumor. We focus on evidence from cerebellar development, the recent identification of BTICs, the presence of activated developmental signaling pathways in MB, the role of epigenetic stem cell regulatory mechanisms, and how these developmental and epigenetic pathways may be targeted for novel therapeutic options.

In diesem Bericht diskutieren wir neuere Beweise f&uuml;r das Vorhandensein einer MB-Stammzelle, der Tumorentstehung in dieser Kindheit maligner Tumor antreibt. Los tumores cerebrales son la principal causa de mortalidad de c&aacute;ncer infantil, con meduloblastoma (MB) que representa el tumor maligno m&aacute;s frecuente. La reciente clasificaci&oacute;n molecular de MB ha reconceptualizado la heterogeneidad que existe dentro de los subtipos patol&oacute;gicos por dar contexto a la funci&oacute;n de clave del desarrollo v&iacute;as de se&ntilde;alizaci&oacute;n en la patogenesia de MB. La identificaci&oacute;n de poblaciones de c&eacute;lulas (CSC) de c&aacute;ncer, llamados c&eacute;lulas de iniciadoras del tumor de cerebro (BTICs), en MB ha proporcionado nuevas dianas celulares para el estudio de estos aberrantemente activados v&iacute;as de se&ntilde;alizaci&oacute;n, es decir, Sonic hedgehog (Shh) y Wingless (Wnt), junto con la identificaci&oacute;n de nuevos caminos de renovaci&oacute;n BTIC. Glioblastoma (GBM) is the most aggressive primary brain tumor in humans, with a uniformly poor prognosis. The tumor microenvironment is composed of both supportive cellular substrates and exogenous factors. We hypothesize that exogenous factors secreted by brain tumor initiating cells (BTICs) could predispose normal neural precursor cells (NPCs) to transformation. When NPCs are grown in GBM-conditioned media, and designated as "tumor-conditioned NPCs" (tcNPCs), they become highly proliferative and exhibit increased stem cell self-renewal, or the unique ability of stem cells to asymmetrically generate another stem cell and a daughter cell. tcNPCs also show an increased transcript level of stem cell markers such as CD133 and ALDH and growth factor receptors such as VEGFR1, VEGFR2, EGFR and PDGFRÎ±. Media analysis by ELISA of GBM-conditioned media reveals an elevated secretion of growth factors such as EGF, VEGF and PDGF-AA when compared to normal neural stem cell-conditioned media. We also demonstrate that tcNPCs require prolonged or continuous exposure to the GBM secretome in vitro to retain GBM BTIC characteristics. Our in vivo studies reveal that tcNPCs are unable to form tumors, confirming that irreversible transformation events may require sustained or prolonged presence of the GBM secretome. Analysis of GBM-conditioned media by mass spectrometry reveals the presence of secreted proteins Chitinase-3-like 1 (CHI3L1) and H2A histone family member H2AX. Collectively, our data suggest that GBM-secreted factors are capable of transiently altering normal NPCs, although for retention of the transformed phenotype, sustained or prolonged secretome exposure or additional transformation events are likely necessary.

Glioblastoma (GBM) es el tumor cerebral primario m&aacute;s agresivo en los seres humanos, con un pron&oacute;stico pobre uniformemente. El microambiente tumoral se compone de substratos celulares apoyo tanto a factores ex&oacute;genos. Presumimos que factores ex&oacute;genos secretados por tumor cerebral iniciar c&eacute;lulas (BTICs) podr&iacute;an predisponer a las c&eacute;lulas precursoras neuronales normal (PNJ) a la transformaci&oacute;n. Cuando son cultivadas en medios GBM-condicionada NPCs y se&ntilde;alados como &quot;tumor-condicionada NPCs&quot; (tcNPCs), se vuelven altamente proliferativas y exhiben la auto-renovaci&oacute;n aumento de c&eacute;lulas madre, o la capacidad &uacute;nica de c&eacute;lulas madre para generar asim&eacute;trico otra c&eacute;lula madre y una c&eacute;lula hija. tcNPCs tambi&eacute;n muestran un nivel de transcripci&oacute;n aumento de marcadores de c&eacute;lulas CD133 como ALDH y receptores de factor de crecimiento como VEGFR1 y VEGFR2, EGFR, PDGFR&alpha;. An&aacute;lisis de los medios por ELISA de GBM-condicionada medios revela una elevada secreci&oacute;n de factores de crecimiento como EGF, VEGF y PDGF-AA en comparaci&oacute;n con normales medios neurales de c&eacute;lulas madre-condicionada. Brain tumors represent the leading cause of childhood cancer mortality, with medulloblastoma (MB) being the most frequent malignant tumor. In this review we discuss the morphological and molecular heterogeneity of this malignant childhood brain tumor and how this key feature has implicated the presence of a MB stem cell. We focus on evidence from cerebellar development, histopathological and molecular subtypes of MB, the recent identification of brain tumor-initiating cells (BTICs, also referred to as MB stem cells), and the current limitations in studying the interplay between MB stem cells and tumor heterogeneity.

In diesen Bericht diskutieren wir die molekulargenetischer und morphologischer Heterogenit&auml;t dieser b&ouml;sartigen Kindheit Gehirntumor und wie dieses wichtige Feature das Vorhandensein einer Stammzelle MB verwickelt hat. Wir fokussieren auf Evidenz zerebell&auml;re Entwicklung, histopathologische und molekularen Subtypen von MB, die j&uuml;ngsten Identifikation von Tumor-Initiierung Gehirnzellen (BTICs, auch als MB-Stammzellen bezeichnet) und die Einschr&auml;nkungen im Zusammenspiel mit MB-Stammzellen und Tumor-Heterogenit&auml;t zu studieren.

I tumori cerebrali rappresentano la principale causa di mortalit&agrave; da cancro infantile, con medulloblastoma (MB), essendo il pi&ugrave; frequente tumore maligno. In questa recensione discutiamo l&#39;eterogeneit&agrave; morfologica e molecolare di questo tumore cerebrale maligno infantile e come questa caratteristica fondamentale ha implicato la presenza di una cellula staminale MB. Ci concentriamo su prove da sviluppo cerebellare, sottotipi istopatologici e molecolare di MB, la recente identificazione di cellule tumorali-avvio del cervello (BTICs, noto anche come cellule staminali MB) e le attuali limitazioni nello studiare l&#39;interazione tra cellule staminali MB e l&#39;eterogeneit&agrave; del tumore.

Tumores cerebrales representan la principal causa de mortalidad de c&aacute;ncer infantil, con meduloblastoma (MB) es el tumor maligno m&aacute;s frecuente. En esta revisi&oacute;n se discute la heterogeneidad morfol&oacute;gica y molecular de este tumor de cerebro maligno de la infancia y c&oacute;mo este rasgo ha implicado la presencia de una c&eacute;lula de v&aacute;stago de MB. Nos centramos en las pruebas de desarrollo cerebeloso, histopatol&oacute;gicos y moleculares subtipos de MB, la reciente identificaci&oacute;n de c&eacute;lulas iniciadoras del tumor de cerebro (BTICs, tambi&eacute;n conocidos como c&eacute;lulas MB) y las limitaciones actuales en el estudio de la interacci&oacute;n entre las c&eacute;lulas de v&aacute;stago de MB y heterogeneidad del tumor.
