We examined innate immune responses to the intracellular bacterium Rhodococcus equi and show that infection of macrophages with intact bacteria induced the rapid translocation of NF-kappa B and the production of a variety of proinflammatory mediators, including TNF, IL-12, and NO. Macrophages from mice deficient in MyD88 failed to translocate NF-kappa B and produced virtually no cytokines in response to R. equi infection, implicating a TLR pathway. TLR4 was not involved in this response, because C3H/HeJ macrophages were fully capable of responding to R. equi infection, and because RAW-264 cells transfected with a dominant negative form of TLR4 responded normally to infection by R. equi. A central role for TLR2 was identified. A TLR2 reporter cell was activated by R. equi, and RAW-264 cells transfected with a dominant negative TLR2 exhibited markedly reduced cytokine responses to R. equi. Moreover, macrophages from TLR2(-/-) mice exhibited diminished cytokine responses to R. equi. The role of the surface-localized R. equi lipoprotein VapA (virulence-associated protein A), in TLR2 activation was examined. Purified rVapA activated a TLR2-specific reporter cell, and it induced the maturation of dendritic cells and the production of cytokines from macrophages. Importantly, TLR2(-/-)-deficient but not TLR4(-/-)-deficient mice were found to be compromised in their ability to clear a challenge with virulent R. equi. We conclude that the efficient activation of innate immunity by R. equi may account for the relative lack of virulence of this organism in immunocompetent adults.

Natalizumab represents an effective biological therapy to treat relapsing-remitting forms of multiple sclerosis and Crohn's disease by blocking the migration of inflammatory cells to the brain and gut. Natalizumab, however, is associated with a risk of progressive multifocal leukoencephalopathy (PML) caused by the reactivation of JC virus. The emergence of PML in this setting has moved PML from being a rare disease mostly seen in HIV-infected individuals to become an important cause of complications in patients receiving immunomodulatory treatments. The incidence of PML associated with natalizumab treatment is approximately 1.5:750, but this increases to approximately 1:100 in patients after 24-36 doses based on available estimates of individuals who have a prior history of immunosuppressive treatment and are antibody positive to JC virus. Natalizumab treatment has raised questions about the pathogenesis of PML but also has provided the opportunity to investigate sites of virus latency and mechanisms of trafficking to the brain.

Progressive multifocal leukoencephalopathy (PML) induced by JC virus (JCV) is a risk for natalizumab-treated multiple sclerosis (MS) patients. Here we characterize the JCV-specific T cell responses in healthy donors and natalizumab-treated MS patients to reveal functional differences that may account for the development of natalizumab-associated PML. CD4 and CD8 T cell responses specific for all JCV proteins were readily identified in MS patients and healthy volunteers. The magnitude and quality of responses to JCV and cytomegalovirus (CMV) did not change from baseline through several months of natalizumab therapy. However, the frequency of T cells producing IL-10 upon mitogenic stimulation transiently increased after the first dose. In addition, MS patients with natalizumab-associated PML were distinguished from all other subjects in that they either had no detectable JCV-specific T cell response or had JCV-specific CD4 T cell responses uniquely dominated by IL-10 production. Additionally, IL-10 levels were higher in the CSF of individuals with recently diagnosed PML. Thus, natalizumab-treated MS patients with PML have absent or aberrant JCV-specific T cell responses compared with non-PML patients, and changes in T cell-mediated control of JCV replication may be a risk factor for developing PML. Our data suggest further approaches to improved monitoring, treatment and prevention of PML in natalizumab-treated patients.

Changes in JC Virus-Specific T Cell Responses during Natalizumab Treatment and in Natalizumab-Associated Progressive Multifocal Leukoencephalopathy.

Changements dans les r&eacute;ponses des lymphocytes T sp&eacute;cifiques du Virus JC pendant le traitement par le Natalizumab et leucoenc&eacute;phalopathie multifocale Progressive associ&eacute;e &agrave; Natalizumab.

Progressive multifokale Leukoenzephalopathie (PML) induziert durch JC-Virus (JCV) ist ein Risiko f&uuml;r Patienten mit Natalizumab behandelt multipler Sklerose (MS). Hier pr&auml;gen wir die JCV-spezifischen T-Zell Reaktionen in gesunde Spender und Natalizumab behandelten MS-Patienten, funktionale Unterschiede zeigen, die die Entwicklung der Natalizumab-assoziierte PML erkl&auml;ren kann. CD4 und CD8 T Zelle Antworten speziell f&uuml;r alle JCV-Proteine wurden bereitwillig bei MS-Patienten und gesunden Probanden ermittelt. Das Ausma&szlig; und die Qualit&auml;t der Antworten JCV und Zytomegalievirus (CMV) &auml;nderte sich nicht vom Ausgangswert &uuml;ber mehrere Monate der Natalizumab-Therapie. Allerdings erh&ouml;ht sich die H&auml;ufigkeit der T-Zellen, die Produktion von IL-10 auf mitogenen Stimulierung vor&uuml;bergehend nach der ersten Dosis. Dar&uuml;ber hinaus wurden MS-Patienten mit Natalizumab-assoziierte PML von allen anderen F&auml;chern unterschieden, entweder keine nachweisbaren JCV-spezifischen T-Zell-Antwort hatten oder JCV-spezifischen CD4 T Zelle Antworten von IL-10 Produktion eindeutig dominiert hatte. Dar&uuml;ber hinaus lagen IL-10 Ebenen des GFK von Individuen mit k&uuml;rzlich diagnostizierten PML. So Natalizumab behandelten MS-Patienten mit PML haben fehlend oder aberrante JCV-spezifischen T-Zell-Antworten im Vergleich zu nicht-PML-Patienten, und &Auml;nderungen in T-Zell-vermittelten Kontrolle der JCV-Replikation m&ouml;glicherweise einen Risikofaktor f&uuml;r die Entwicklung von PML. Unsere Daten zeigen weitere Ans&auml;tze f&uuml;r verbesserte &Uuml;berwachung, Behandlung und Pr&auml;vention von PML in Natalizumab behandelten Patienten.

Ver&auml;nderungen bei JC-Virus-spezifische T-Zell-Antworten w&auml;hrend der Natalizumab-Behandlung und Natalizumab-assoziierte Progressive multifokale Leukoenzephalopathie.

Leucoencefalopatia multifocale progressiva (PML) indotta dal virus JC (JCV) &egrave; un rischio per i pazienti trattati con natalizumab sclerosi multipla (SM). Qui ci caratterizzano il JCV-specifiche risposte delle cellule T in donatori sani e trattati con natalizumab pazienti affetti da MS per rivelare differenze funzionali che possono spiegare lo sviluppo di PML natalizumab associate. CD4 e CD8 T le risposte delle cellule specifiche per tutte le proteine JCV sono state prontamente identificate in pazienti affetti da MS e volontari sani. Dalla linea di base attraverso diversi mesi di terapia natalizumab non ha cambiato la grandezza e la qualit&agrave; delle risposte di JCV e citomegalovirus (CMV). Tuttavia, la frequenza di cellule T che producono IL-10 dopo stimolazione con mitogeni transitoriamente aumentato dopo la prima dose. Inoltre, MS pazienti con PML natalizumab associate erano distinti da tutti gli altri soggetti che non avevano o rilevabile risposta delle cellule T JCV-specifica o avevano risposte delle cellule T CD4 specifici JCV univocamente dominata dalla produzione di IL-10. Inoltre, i livelli di IL-10 erano pi&ugrave; elevati nel QCS di individui con PML recentemente diagnosticati. Cos&igrave;, trattati con natalizumab MS pazienti con PML sono assenti o aberrante JCV-specifiche delle cellule T risposte rispetto ai pazienti non-PML, e le modifiche nel controllo delle cellule T-mediata della replica di JCV possono essere un fattore di rischio per lo sviluppo di PML. I nostri dati suggeriscono ulteriori approcci a migliore monitoraggio, trattamento e prevenzione di PML in pazienti trattati con natalizumab.

Leucoencefalopat&iacute;a multifocal progresiva (LMP) inducida por virus JC (JCV) es un riesgo para los pacientes tratados con natalizumab de esclerosis m&uacute;ltiple (EM). Aqu&iacute; se caracterizan las respuestas de c&eacute;lulas T JCV espec&iacute;ficos en donantes sanos y pacientes con em tratados con natalizumab para revelar las diferencias funcionales que pueden explicar el desarrollo de PML natalizumab asociada. Respuestas de la c&eacute;lula CD4 y CD8 T espec&iacute;ficas para todas las prote&iacute;nas JCV fueron identificadas f&aacute;cilmente en pacientes con EM y voluntarios sanos. La magnitud y la calidad de las respuestas a JCV y el citomegalovirus (CMV) no cambiaron desde el inicio a trav&eacute;s de varios meses de terapia de natalizumab. Sin embargo, la frecuencia de las c&eacute;lulas T producen IL-10 sobre el est&iacute;mulo mitog&eacute;nico transitoriamente aument&oacute; despu&eacute;s de la primera dosis. Wnt/Î²-catenin is a neuroprotective pathway regulating cell fate commitment in the CNS and many vital functions of neurons and glia. Its dysregulation is linked to a number of neurodegenerative diseases. Wnt/Î²-catenin is also a repressor of HIV transcription in multiple cell types, including astrocytes, which are dysregulated in HIV-associated neurocognitive disorder. Given that HIV proteins can overcome host restriction factors and that perturbations of Wnt/Î²-catenin signaling can compromise astrocyte function, we evaluated the impact of HIV transactivator of transcription (Tat) on Wnt/Î²-catenin signaling in astrocytes. HIV clade B Tat, in primary progenitor-derived astrocytes and U87MG cells, inhibited Wnt/Î²-catenin signaling as demonstrated by its inhibition of active Î²-catenin, TOPflash reporter activity, and Axin-2 (a downstream target of Wnt/Î²-catenin signaling). Point mutations in either the core region (K41A) or the cysteine-rich region (C30G) of Tat abrogated its ability to inhibit Î²-catenin signaling. Clade C Tat, which lacks the dicysteine motif, did not alter Î²-catenin signaling, confirming that the dicysteine motif is critical for Tat inhibition of Î²-catenin signaling. Tat coprecipitated with TCF-4 (a transcription factor that partners with Î²-catenin), suggesting a physical interaction between these two proteins. Furthermore, knockdown of Î²-catenin or TCF-4 enhanced docking of Tat at the TAR region of the HIV long terminal repeat. These findings highlight a bidirectional interference between Tat and Wnt/Î²-catenin that negatively impacts their cognate target genes. The consequences of this interaction include alleviation of Wnt/Î²-catenin-mediated suppression of HIV and possible astrocyte dysregulation contributing to HIV neuropathogenesis.

WNT/&beta;-catenina es un neuroprotector regular compromiso de destino celular en el SNC y muchas funciones vitales de las neuronas y gl&iacute;a. Su desregulaci&oacute;n est&aacute; vinculada a una serie de enfermedades neurodegenerativas. WNT/&beta;-catenina es tambi&eacute;n un represor de la transcripci&oacute;n del VIH en varios tipos celulares, incluyendo los astrocitos, que son dysregulated en desorden neurocognitivas asociadas con el VIH. Dado que las prote&iacute;nas del VIH pueden superar factores de restricci&oacute;n de anfitri&oacute;n y que las perturbaciones de la se&ntilde;alizaci&oacute;n de Wnt/&beta;-catenina pueden comprometer la funci&oacute;n de astrositos, evalu&oacute; el impacto de transactivator de VIH de la transcripci&oacute;n (Tat) sobre la se&ntilde;alizaci&oacute;n de Wnt/&beta;-catenina en astrocitos. Clado C Tat, que carece el motivo dicysteine, no alter&oacute; de &beta;-catenina se&ntilde;alizaci&oacute;n, confirmando que el motivo de dicysteine es cr&iacute;tico para la inhibici&oacute;n de la se&ntilde;alizaci&oacute;n de &beta;-catenina Tat. TAT coprecipitado con TCF-4 (factor de transcripci&oacute;n se asocia con &beta;-catenina), lo que sugiere una interacci&oacute;n f&iacute;sica entre estas dos prote&iacute;nas. Adem&aacute;s, la ca&iacute;da de &beta;-catenina o TCF-4 hab&iacute;a mejorado acoplamiento de Tat en la regi&oacute;n de alquitr&aacute;n de la repetici&oacute;n terminal larga del VIH. Proinflammatory factors from activated T cells inhibit neurogenesis in adult animal brain and cultured human fetal neural stem cells (NSC). However, the role of inhibition of neurogenesis in human neuroinflammatory diseases is still uncertain because of the difficulty in obtaining adult NSC from patients. Recent developments in cell reprogramming suggest that NSC may be derived directly from adult fibroblasts. We generated NSC from adult human peripheral CD34+ cells by transfecting the cells with Sendai virus constructs containing Sox2, Oct3/4, c-Myc and Klf4. The derived NSC could be differentiated to glial cells and action potential firing neurons. Co-culturing NSC with activated autologous T cells or treatment with recombinant granzyme B caused inhibition of neurogenesis as indicated by decreased NSC proliferation and neuronal differentiation. Thus, we have established a unique autologous in vitro model to study the pathophysiology of neuroinflammatory diseases that has potential for usage in personalized medicine.
