Pelvic ring injuries (PRI) are among the most difficult injuries to deal with in orthopedic trauma. When these injuries are accompanied by hemodynamic instability their management becomes significantly more complex. A methodical assessment and expeditious triage are required for these patients followed by adequate resuscitation. A major triage decision is whether these patients should undergo arterial embolization in the angiography suit or prompt packing and pelvic stabilization in the operating room. Patient characteristics, fracture type and injury characteristics are taken into consideration in the decision-making process. In this review we discuss the acute evaluation, triage and management of PRIs associated with hemodynamic instability. An evidence based and protocol driven approach is necessary in order to achieve optimal outcomes in these patients.

Management of Pelvic Ring Injury Patients With Hemodynamic Instability.

We describe the surgical management of a girl with dwarfism and congenital spondyloepiphyseal dysplasia, who presented in adolescence with coxa vara and bilateral pseudarthrosis between the femoral neck and the diaphysis, with asymmetric distal migration at both sites and leg length discrepancy. The patient at 16 years underwent valgus osteotomy in situ and femoral neck fixation in the left hip. The right hip was operated 19 months later.

Keblish's lateral surgical approach for total knee arthroplasty enables a direct release of the lateral structures in valgus deformities. In recent years, the development of quadriceps-sparing techniques has improved early functional recovery after arthroplasty, which has been well described using a medial approach. The authors present Keblish's lateral subvastus approach for genu valgum deformities, with key surgical considerations and preliminary outcomes. The novelty of this technique lies in its combination of a lateral subvastus approach with Keblish's Z-plasty, aiming to optimize quadriceps recovery while ensuring secure knee closure following valgus deformity correction. The Keblish's lateral subvastus approach provided adequate surgical exposure without any observed complications in this small short follow-up series, allowing for early recovery of active knee extension and satisfactory clinical outcomes. Keblish's lateral subvastus approach for total knee arthroplasty: A technical note.

Total hip arthroplasty is among the most successful orthopedic procedures; however, approximately 10%-20% of patients require revision surgery within 15-20 years. Potential failure mechanisms at the bone-implant interface include stress shielding and excessive micromotion. This experimental work aims at examining the optimal ratio of femur length and stem length with respect to primary stability and stress shielding effect, employing a validated finite element analysis.
