Cardiogenic shock (CS) presents a medical challenge with limited treatment options. Positive end-expiratory pressure (PEEP) during mechanical ventilation has been linked with clinical benefits in patients with CS. We investigated if increasing PEEP levels could unload the left ventricle (LV) in CS in a large animal model of LV-CS.

Cardiovascular Effects of Increasing Positive End-Expiratory Pressure in A Model of Left Ventricular Cardiogenic Shock in Female Pigs.

Acute intermediate-high-risk pulmonary embolism (PE) causes a pathological increase in pulmonary artery pressure and right ventricle afterload that may progress to right ventricle failure and cardiopulmonary collapse. Patients may require mechanical ventilation, further increasing pulmonary vascular resistance (PVR) and artery pressure. We aimed to investigate the ability of ventilator settings adjustments to reduce pulmonary artery pressure in a porcine model.

Mechanical ventilation in acute pulmonary embolism: a randomized, experimental, crossover study.

Cardiogenic shock (CS) is associated with high mortality and medical therapies have failed to improve survival. Treatment with lactate is associated with improved cardiac function which may benefit this condition. Comprehensive hemodynamic assessment of lactate administration in CS is lacking, and the mechanisms underlying the cardiovascular effects of lactate in CS have not yet been elucidated. In this study we aimed to study the cardiovascular and cardiometabolic effects of treatment with lactate in experimental ischemic CS.

Acute pulmonary embolism (PE) is a leading cause of cardiovascular death, primarily due to abrupt increased pulmonary vascular resistance (PVR) leading to acute right ventricular (RV) failure. Ketone bodies, especially 3-hydroxybutyrate (3-OHB), have shown potential to increase cardiac output (CO) and reduce PVR in pulmonary hypertension, suggesting possible benefits in PE. We hypothesized that 3-OHB would induce pulmonary vasorelaxation and increase CO in a porcine model of acute PE.
