A unique interventional approach for modulating sensory signaling involves targeting neurons in the sensory ganglia through use of pharmacological or gene therapies. This has previously been accomplished in the trigeminal system via stereotactic ganglionic microinjections. However, this procedure can be time consuming and requires expensive stereotactic equipment. The current paper describes a percutaneous approach for injecting the trigeminal ganglion with an electrical-stimulation needle inserted through the infraorbital foramen, infraorbital canal and foramen rotundum. The needle tip was positioned in the ganglion by eliciting a twitch in the ipsilateral masseter following electrical stimulation. The technique was imaged using computerized tomography and methylene blue dye to verify the site of the injection. We validated this technique by reproducing the results from our prior study that involved injection of resiniferatoxin (RTX) into the trigeminal ganglion using the stereotactic approach. Both techniques for intraganglionic injection of RTX produced a specific deletion of C-fiber neurons, as demonstrated by inhibition of neurogenic-induced plasma extravasation and behavioral assays. Thus, we propose a novel and simple technique for studying mechanisms of peripheral sensory modulation of orofacial pain via direct application of drugs, tracers or viral vectors around trigeminal sensory neuronal cell bodies. This technique minimizes trauma to brain structures that may have an impact on pain perception.

Peripheral targeting of the trigeminal ganglion via the infraorbital foramen as a therapeutic strategy.

Une approche unique d&#x26;#39;intervention pour la signalisation sensorielle de modulation consiste &#xE0; cibler les neurones dans les ganglions sensitifs par l&#x26;#39;utilisation de traitements pharmacologiques ou d&#x26;#39;un g&#xE8;ne. Cela a d&#xE9;j&#xE0; &#xE9;t&#xE9; accompli dans le syst&#xE8;me trig&#xE9;minal par st&#xE9;r&#xE9;otaxie microinjections ganglionnaires. Investigations of new analgesic treatments ideally are coupled with the use of compassionate methods for pain testing in animals. Recently, we described a novel operant thermal testing device that can be used to quantify orofacial pain. The objective of the current study was to differentiate thermal allodynia from hyperalgesia using this operant thermal assay. Rats were trained to complete a task whereby they had a conflict between a positive reward and tolerance for thermal nociceptive stimulation. They were subjected to cool to hot temperatures (24-45 degrees C) and evaluated under naÃ¯ve (untreated), capsaicin cream (0.075%), capsaicin/morphine, or morphine test conditions. The following outcome measures were evaluated: reward intake; licking contacts; facial contacts; time to complete 25, 50, and 75% of the events (licks and face contacts); facial contact duration; ratio of reward/stimulus contacts; and ratio of facial contact duration/event. Capsaicin produced an increase in mechanical sensitivity and a significant thermal allodynic effect at 42 degrees C and hyperalgesic effect at 45 degrees C. These effects were blocked with morphine pre-treatment. The temporal profile for completing the task was also significantly altered following capsaicin treatment. These data demonstrate that using the operant orofacial assay in conjunction with capsaicin cream can provide a reproducible, sensitive, minimally invasive, and powerful approach for quantifying and studying enhanced thermal pain within the trigeminal system. This technique provides an alternative to reflex tests of orofacial sensitivity, and it presents a pivotal link for translating basic pain research into clinic trial strategies.

Differentiation between capsaicin-induced allodynia and hyperalgesia using a thermal operant assay.

Id&eacute;alement, les enqu&ecirc;tes sur les nouveaux traitements analg&eacute;siques sont coupl&eacute;s avec l&#39;utilisation de m&eacute;thodes de compassion pour la douleur tests chez les animaux. R&eacute;cemment, nous avons d&eacute;crit un roman op&eacute;rant thermique appareil qui permet de quantifier la douleur orofaciale. La pr&eacute;sente &eacute;tude visait &agrave; diff&eacute;rencier allodynie thermique d&#39;hyperalg&eacute;sie utilisant cette analyse thermique op&eacute;rante. Resiniferatoxin (RTX) is an ultrapotent capsaicin analog that binds to the transient receptor potential channel, vanilloid subfamily member 1 (TRPV1). There is a large body of evidence supporting a role for TRPV1 in noxious-mediated and inflammatory hyperalgesic responses. In this study, we evaluated low, graded, doses of perineural RTX as a method for regional pain control. We hypothesized that this approach can provide long-term, but reversible, blockade of a portion of nociceptive afferent fibers within peripheral nerves when given at a site remote from the neuronal perikarya in the dorsal root ganglia. Following perineural RTX application to the sciatic nerve, we demonstrated a significant inhibition of inflammatory nociception that was dose- and time-dependent. At the same time, treated animals maintained normal proprioceptive sensations and motor control, and other nociceptive responses were largely unaffected. Using a range of mechanical and thermal algesic tests, we found that the most sensitive measure following perineural RTX administration was inhibition of inflammatory hyperalgesia. Recovery studies showed that physiologic sensory function could return as early as two weeks post-RTX treatment, however, immunohistochemical examination of the DRG revealed a partial, but significant reduction in the number of the TRPV1-positive neurons. We propose that this method could represent a beneficial treatment for a range of chronic pain problems, including neuropathic and inflammatory pain not responding to other therapies.

Resiniferatoxina (RTX) es un an&#xE1;logo de la capsaicina ultrapotente que se une al canal receptor de potencial transitorio, subfamilia miembro vaniloide 1 (TRPV1). Hay un gran cuerpo de evidencia que apoya el papel de TRPV1 en las respuestas mediadas por hiperalg&#xE9;sicas nociva e inflamatorias. Resiniferatoxina perineural inhibe selectivamente la hiperalgesia inflamatoria

Rodent models of orofacial pain typically use methods adapted from manipulations to hind paw; however, limitations of these models include animal restraint and subjective assessments of behavior by the experimenter. In contrast to these methods, assessment of operant responses to painful stimuli has been shown to overcome these limitations and expand the breadth of interpretation of the behavioral responses. In the current study, we used an operant model based on a reward-conflict paradigm to assess nociceptive responses in three strains of mice (SKH1-Hrhr, C57BL/6J, TRPV1 knockout). We previously validated this operant model in rats and hypothesized in this study that wild-type mice would demonstrate a similar thermal stimulus-dependent response and similar operant pain behaviors. Additionally, we evaluated the effects on operant behaviors of mice manipulated genetically (e.g., TRPV1 k.o.) or pharmacologically with resiniferatoxin (RTX), a lesioning agent for TRPV1-expressing neurons. During the reward-conflict task, mice accessed a sweetened milk reward solution by voluntarily position their face against a neutral or heated thermode (37-55 degrees C).

Detecting behaviors related to orofacial pain in rodent models often relies on subjective investigator grades or methods that place the animal in a stressful environment. In this study, an operant-based behavioral assay is presented for the assessment of orofacial tactile sensitivity in the rat.
