&#x916A;&#x6C28;&#x9178;&#x6FC0;&#x9176;&#x6291;&#x5236;&#x5242; PKC412 D816V &#x5957;&#x4EF6;&#x7A81;&#x53D8;&#x4E0E;&#x80A5;&#x5927;&#x7EC6;&#x80DE;&#x767D;&#x8840;&#x75C5;&#x60A3;&#x8005;&#x7684;&#x6D3B;&#x52A8;&#x3002;

The majority of patients with systemic mast cell disease express the imatinib-resistant Asp816Val (D816V) mutation in the KIT receptor tyrosine kinase. Limited treatment options exist for aggressive systemic mastocytosis (ASM) and mast cell leukemia (MCL). We evaluated whether PKC412, a small-molecule inhibitor of KIT with a different chemical structure from imatinib, may have therapeutic use in advanced SM with the D816V KIT mutation. We treated a patient with MCL (with an associated myelodysplastic syndrome (MDS)/myeloproliferative disorder [MPD]) based on in vitro studies demonstrating that PKC412 could inhibit D816V KIT-transformed Ba/F3 cell growth with a 50% inhibitory concentration (IC50) of 30 nM to 40 nM. The patient exhibited a partial response with significant resolution of liver function abnormalities. In addition, PKC412 treatment resulted in a significant decline in the percentage of peripheral blood mast cells and serum histamine level and was associated with a decrease in KIT phosphorylation and D816V KIT mutation frequency. The patient died after 3 months of therapy due to progression of her MDS/MPD to acute myeloid leukemia (AML). This case indicates that KIT tyrosine kinase inhibition is a feasible approach in SM, but single-agent clinical efficacy may be limited by clonal evolution in the advanced leukemic phase of this disease.

La majorit&#xE9; des patients atteints de la maladie syst&#xE9;mique de cellules de m&#xE2;t exprimer l&#x26;#39;r&#xE9;sistants &#xE0; l&#x26;#39;imatinib Asp816Val (D816V) mutation dans la kinase du r&#xE9;cepteur KIT tyrosine. Options de traitement limit&#xE9;es existent pour la mastocytose syst&#xE9;mique agressive (ASM) et la leuc&#xE9;mie &#xE0; mastocytes (MCL). Nous avons &#xE9;valu&#xE9; si PKC412, un inhibiteur de petite mol&#xE9;cule de KIT avec une structure chimique diff&#xE9;rente de l&#x26;#39;imatinib, peuvent avoir un usage th&#xE9;rapeutique dans les pays avanc&#xE9;s SM avec la mutation KIT D816V. Nous avons trait&#xE9; un patient avec MCL (avec un syndrome associ&#xE9; my&#xE9;lodysplasique (MDS) / syndrome my&#xE9;loprolif&#xE9;ratif [MPD]) bas&#xE9;e sur des &#xE9;tudes in vitro d&#xE9;montrant que PKC412 pourrait inhiber D816V KIT-transform&#xE9; Ba/F3 la croissance des cellules avec une concentration inhibitrice 50% (IC50) de 30 nm &#xE0; 40 nm. Le patient pr&#xE9;sentait une r&#xE9;ponse partielle avec une r&#xE9;solution importante d&#x26;#39;anomalies de la fonction h&#xE9;patique. En outre, PKC412 traitement a entra&#xEE;n&#xE9; une baisse significative du pourcentage de p&#xE9;riph&#xE9;riques mastocytes sang et des s&#xE9;rum niveau d&#x26;#39;histamine et a &#xE9;t&#xE9; associ&#xE9;e &#xE0; une diminution de la phosphorylation de KIT et de la fr&#xE9;quence de mutation D816V KIT. Le patient est d&#xE9;c&#xE9;d&#xE9; apr&#xE8;s 3 mois de traitement en raison de la progression de ses SMD / SMP &#xE0; la leuc&#xE9;mie my&#xE9;lo&#xEF;de aigu&#xEB; (LMA). Ce cas indique que inhibition de la kinase KIT tyrosine est une approche r&#xE9;alisable dans SM, mais un seul agent l&#x26;#39;efficacit&#xE9; clinique peut &#xEA;tre limit&#xE9;e par l&#x26;#39;&#xE9;volution clonale dans la phase avanc&#xE9;e de cette maladie leuc&#xE9;mique.

Die Aktivit&#xE4;t der Tyrosinkinase-Inhibitor PKC412 bei einem Patienten mit Mastzellleuk&#xE4;mie mit dem KIT D816V Mutation.

La maggior parte dei pazienti con malattia sistemica mastociti esprimono l&#39;imatinib-resistente Asp816Val (D816V) mutazione della chinasi della tirosina del recettore KIT. Opzioni di trattamento limitato esistono per Mastocitosi sistemica aggressiva (ASM) e mast cell leucemia (MCL). Abbiamo valutato se PKC412, un inibitore della piccolo-molecola di KIT con una struttura chimica diversa da imatinib, pu&ograve; avere uso terapeutico in SM avanzate con la mutazione D816V KIT. Abbiamo trattato un paziente con MCL (con una sindrome mielodisplastica associata (MDS) / mieloproliferative disordine [MPD]) sulla base di studi in vitro che dimostrano che PKC412 potrebbe inibire la crescita cellulare trasformato KIT D816V Ba/F3 con una concentrazione inibente il 50% (CL50) di 30 nM a 40 nM. Il paziente esposto una risposta parziale con notevole risoluzione di anomalie della funzione epatica. Inoltre, il trattamento PKC412 ha provocato un calo significativo della percentuale di sangue periferico di mastociti e livello sierico di istamina ed &egrave; stato associato con una diminuzione della fosforilazione di KIT e frequenza di mutazione D816V KIT. Il paziente &egrave; morto dopo 3 mesi di terapia a causa della progressione della sua MDS/MPD a leucemia mieloide acuta (LMA). Questo caso indica che KIT tirosina chinasi inibizione &egrave; un approccio fattibile in SM, ma efficacia clinica del singolo agente pu&ograve; essere limitato da evoluzione clonale in fase avanzata di leucemiche di questa malattia.

La mayor&#xED;a de los pacientes con enfermedad sist&#xE9;mica de los mastocitos expresan la resistente al imatinib Asp816Val (D816V) mutaci&#xF3;n en el receptor quinasa KIT tirosina. Limitadas opciones de tratamiento existen para la mastocitosis sist&#xE9;mica agresiva (ASM) y la leucemia de mastocitos (MCL). Se evalu&#xF3; si PKC412, un inhibidor de mol&#xE9;cula peque&#xF1;a del KIT con una estructura qu&#xED;mica diferente de imatinib, puede tener un uso terap&#xE9;utico en la avanzada de SM con la mutaci&#xF3;n KIT D816V. Se han tratado un paciente con LMC (con un s&#xED;ndrome asociado mielodispl&#xE1;sico (SMD) / trastorno mieloproliferativo [MPD]), basado en estudios in vitro que demuestran que PKC412 podr&#xED;a inhibir D816V KIT-transformado Ba/F3 crecimiento de las c&#xE9;lulas con una concentraci&#xF3;n inhibitoria del 50% (IC50) de 30 nm a 40 nm. El paciente mostr&#xF3; una respuesta parcial, con una resoluci&#xF3;n significativa de anomal&#xED;as de la funci&#xF3;n del h&#xED;gado. Adem&#xE1;s, PKC412 tratamiento result&#xF3; en una disminuci&#xF3;n significativa en el porcentaje de sangre perif&#xE9;rica de c&#xE9;lulas cebadas y el nivel de histamina en suero y se asoci&#xF3; con una disminuci&#xF3;n de la fosforilaci&#xF3;n de KIT y la frecuencia de la mutaci&#xF3;n KIT D816V. El paciente falleci&#xF3; despu&#xE9;s de 3 meses de terapia debido a la progresi&#xF3;n de sus SMD / SMP con leucemia mieloide aguda (LMA). Although a large proportion of patients with polycythemia vera (PV) harbor a valine-to-phenylalanine mutation at amino acid 617 (V617F) in the JAK2 signaling molecule, the stage of hematopoiesis at which the mutation arises is unknown. Here we isolated and characterized hematopoietic stem cells (HSC) and myeloid progenitors from 16 PV patient samples and 14 normal individuals, testing whether the JAK2 mutation could be found at the level of stem or progenitor cells and whether the JAK2 V617F-positive cells had altered differentiation potential. In all PV samples analyzed, there were increased numbers of cells with a HSC phenotype (CD34+CD38-CD90+Lin-) compared with normal samples. Hematopoietic progenitor assays demonstrated that the differentiation potential of PV was already skewed toward the erythroid lineage at the HSC level. The JAK2 V617F mutation was detectable within HSC and their progeny in PV. Moreover, the aberrant erythroid potential of PV HSC was potently inhibited with a JAK2 inhibitor, AG490.

The JAK2 V617F mutation occurs in hematopoietic stem cells in polycythemia vera and predisposes toward erythroid differentiation.

To provide a comprehensive, thorough analysis of somatic mutation and promoter hypermethylation of the von Hippel-Lindau (VHL) gene in the cancer genome, unique to clear cell renal cancer (ccRCC). Identify relationships between the prevalence of VHL gene alterations and alteration subtypes with patient and tumor characteristics.
