测量光学牙科印模的全牙弓变形

本文介绍了一种测量从口内扫描仪获取的全牙弓数字印模各部分变形程度的实验方案,该方案使用具有标准几何结构的3D打印金属模型。

摘要

自牙医开始使用口内扫描仪获取三维图像进行数字化印模以来,数字工作流程已被广泛用于制作牙科修复体或口腔矫治器。由于需要在患者口腔内进行扫描,口内扫描仪是一种带有小型光学窗口的手持设备,通过拼接多个小区域的数据来完成整体图像。在全牙弓印模过程中,印模体可能发生形变,从而影响修复体或矫治器的适合性。为了测量这些形变,本研究设计并采用金属3D打印技术制作了一个标准样本。所设计的参考几何结构可为每个印模建立独立的坐标系,并测量圆柱体顶面圆心的x、y和z方向位移,从而评估印模的形变程度。为验证该方法的可靠性,将原始计算机辅助设计(CAD)数据与工业扫描仪获取的参考数据进行对比,计算并比较圆柱体的坐标值。两组数据之间的坐标差异大多小于50 µm,但由于后牙区倾斜设计的圆柱体在3D打印时存在加工公差,其z方向坐标偏差相对较大。然而,由于打印模型本身设定了新的基准标准,因此并不影响测试评估结果。参考扫描仪的重复性为11.0 ± 1.8 µm。该测试方法可用于识别和改进口内扫描仪的固有缺陷,或通过测量全牙弓数字印模各部位的形变程度,建立优化的扫描策略。

引言

在传统的牙科治疗过程中,固定修复体或可摘义齿是在由石膏制成并使用硅橡胶或不可逆水胶体材料印模获得的模型上制作的。由于间接制作的修复体最终需在口腔内戴入,因此已有大量研究致力于克服此类制造流程中产生的一系列误差1,2。近年来,通过获取三维图像而非制取印模,并在虚拟空间中对模型进行操作,利用计算机辅助设计(CAD)技术进行修复体制作的数字化方法已得到应用3。早期,这种光学印模技术仅应用于单颗或少数几颗牙齿的龋齿治疗等有限范围。然而,随着三维扫描仪基础技术的发展,全牙弓的数字化印模现已广泛用于大范围固定修复体、局部或全口可摘义齿、正畸矫治器以及种植外科导板等修复体的制作4,5,6,7。在单侧牙弓等较短区域,数字化印模的精度令人满意。然而,由于口内扫描仪是一种手持设备,需通过狭窄的光学窗口获取图像并进行拼接以完成全牙列扫描,因此在完成U形牙弓扫描后可能出现模型扭曲。因此,基于该模型制作的大范围修复体可能无法在患者口腔中良好就位,需要大量调改。

已有诸多研究报道了使用口内扫描仪获取的虚拟印模的准确性,相关研究采用了不同的研究模型和测量方法。根据研究对象的不同,可分为针对真实患者的临床研究8,9,10,11,12,以及在为研究单独制作的模型上开展的体外研究13,14,15,16。临床研究的优势在于能够评估真实临床环境下的情况,但变量控制困难,且无法无限增加临床病例数量。由于在评估所需变量方面存在局限,目前临床研究的数量并不充足。相比之下,已有大量体外研究通过控制变量来评估口内扫描仪的基本性能17。研究模型包括天然牙的部分或完整牙弓18,19,20,21,22、全部牙齿缺失的无牙颌23、种植体以一定间隔安装并分布的情况24,25,26,27,以及多数牙齿保留而仅部分缺牙的形态16,28。然而,针对手持式口内扫描仪所生成虚拟印模形变的研究,仍局限于通过与参考数据叠加生成的色谱图进行定性评估,并以单个数值表示整体偏差。由于大多数研究仅考察牙弓局部区域的非方向性距离偏差,因此难以对完整牙弓的三维形变进行精确测量。

本研究通过使用带有坐标系的标准模型,探究口内扫描仪进行光学印模时牙弓形态的变形情况。本研究旨在提供一种评估口内扫描仪精度性能的方法,此类扫描仪因光学硬件和处理软件的不同而表现出各异的特性。

方案

1. 掌握样本制备

  1. 模型制备
    1. 在下颌全牙弓模型上,仅保留左侧和右侧尖牙、第二前磨牙及第二磨牙颈部的1/5,其余部分的人工牙齿予以去除。
  2. CAD设计
    1. 使用参考扫描仪获取标准样本的数据。
    2. 利用逆向工程软件,在已修整的六颗牙齿顶部设计圆柱体(顶部直径为2 mm,圆柱高度为7 mm)。
    3. 在左侧第二磨牙后方添加三个参考球体(直径为3.5 mm),用于在逆向工程软件中定义参考三维坐标系。
    4. 将其中一个球体置于左侧第二磨牙圆柱体的远中侧及颊侧,以确保所有圆柱体的坐标值均为正值。
    5. 设计左侧第二磨牙上的圆柱体,使其向内侧倾斜30°,右侧第二磨牙上的圆柱体则向远中倾斜30°,其余圆柱体与模型表面垂直。
  3. 金属3D打印
    1. 使用金属3D打印机以CoCr合金制造一个模拟模型,用作患者的牙列(图1)。

2. 参考数据的采集与软件分析

  1. 使用测试用口内扫描仪对模型进行扫描。
    1. 通过使用工业级模型扫描仪扫描金属模型来获取参考图像。
  2. 通过提取参考球体上的点建立坐标系。
    1. 将参考图像载入逆向工程分析软件,以计算每个圆柱体位置的参考坐标。
    2. 选择 Ref. geometry | Create | Sphere | Pick boundary points 命令,并在参考球体表面选取彼此间距最远的四个点,以提取球体(补充图 1 和 补充图 2)。
    3. 计算三个参考球体的球心位置。
    4. 使用 Ref. geometry | Create | Plane | Pick points 命令连接三个球体的球心并创建一个平面(补充图 3)。
    5. 将所形成的平面设为 XY 平面。
    6. 选择 Ref. geometry | Create | Plane | Offset plane 命令,在 xy 平面上方创建一个切平面(补充图 4)。
    7. 通过选择 Ref. geometry | Create | Point | Project on ref. plane 命令,创建切平面与两个舌侧球体相交的点(补充图 5)。
    8. 使用 Ref. geometry | Create | Plane | Pick points 命令,在所创建的点与两个舌侧球体的球心之间生成一个平面(补充图 6)。
    9. 使用 Inspection | Dimension | Linear 命令测量该平面到颊侧球体球心的距离(补充图 7)。
    10. 使用 Geometry | Create | Plane | Offset Plane 命令,创建一个通过颊侧球体中点的平行平面(补充图 8)。
    11. 将所形成的平面设为 YZ 平面(补充图 9)。
  3. 设定 x、y 和 z 轴。
    1. 将颊侧球体的球心设为坐标系的“原点”。
    2. 设定一条通过原点并平行于其余两个球体球心连线的直线,作为沿模型前后方向的 Y 轴。
    3. 在 xy 平面上设定一条通过原点且垂直于 y 轴的直线作为 X 轴。
    4. 使用 Ref. geometry | Create | Coordinate | Pick origin & X, Y direction 命令,以颊侧球体球心为原点创建新的坐标系(补充图 10)。
    5. 设定一条垂直于 xy 平面并通过原点的直线作为 Z 轴(补充图 11)。
  4. 将此信息从扫描坐标系转换至新建立的坐标系。
    1. 使用 Ref. geometry | Bind to shell 命令,将此过程中创建的几何结构固定在扫描数据之上(补充图 12)。
    2. 执行 Ref. geometry | Transform | Coordinate | Align coordinate 命令,从基础坐标系转换至新创建的坐标系(补充图 13)。
    3. 由此,以三个参考球体为基准,为金属标准样本分配坐标系(补充图 14)。
  5. 从主区域的圆柱体中提取测量点。
    1. 通过逆向工程过程,提取待分析六个圆柱体上缘圆心的 x、y 和 z 坐标,以评估指定区域的形变情况。
    2. 为此,使用 Ref. geometry | Create | Cylinder | Pick boundary points 命令,在圆柱体顶部边缘指定至少 10 个点,并在圆柱体底部与牙齿接触的椭圆上指定相同数量的点(补充图 15、补充图 16 和 补充图 17)。
    3. 获取圆柱体顶部中心的提取坐标。通过将其与待评估口内扫描仪获取的数字印模中相同圆柱体的坐标值进行比较,评估各位置的三维形变。

结果

根据原始设计的CAD数据以及使用工业级模型扫描仪对3D打印金属主模型标本进行扫描所获得的参考图像,计算出的每个圆柱体的坐标如表1所示。两者之间的差异小于50 µm,但3D打印主模型标本中右下第二磨牙圆柱体的z坐标值偏低。尽管该金属模型是通过高端工业级3D打印机制造的,仍发现其中一个圆柱体的高度存在微小差异。虽然设计过程使用了CAD软件,但在后续实验中,该金属模型被作为参考标准,由多种测试用口内扫描仪进行扫描,其差异可忽略不计。如果其他评估人员使用相同的共享数据制作新的模型并执行相同流程,则应再次使用工业级参考扫描仪对该新模型进行扫描,以获取参考坐标,然后继续后续步骤。表2显示了使用工业扫描仪对主模型标本进行五次重复扫描所获得的坐标值。从标准差评估来看,平均偏差为45 µm,其中右下第二磨牙圆柱体的y坐标值表现出较大的离散性。可以得出结论:该参考扫描仪的精度足以用于提取零点及六个圆柱体的参考坐标。

讨论

在通过评估获得的数字化印模实体来评价口内扫描仪准确性的研究中,最常用的方法是将数字化印模数据与参考图像进行叠加,并计算壳体与壳体之间的偏差值12,13,14,15,20,23。然而,该方法仅限于计算配对数据之间的偏差值,或通过色谱图对偏差分布进行定性评估。在一项通过在色谱图上选择分析点来测量局部区域偏差的研究中,并未考虑 x、y 和 z 方向上的偏差29。此外,这些方法存在局限性,即必须先与参考数据进行叠加后才能分析。数据点之间的配准可能各不相同,且分析结果会因排序标准的不同而有所差异。在涉及患者的临床试验中,由于无法使用位于口腔外的工业扫描仪经...
