方案

所有参与者均签署了书面知情同意书。本研究已获得日本顺天堂大学埼玉医疗中心机构审查委员会批准(RIN 15–104)。

1. rSO2 监测设备

  1. 准备一台近红外光谱(NIRS)设备,用于测量组织氧合。该设备具有四个通道,可同时对最多四个器官进行测量。
  2. 为NIRS监测准备测量传感器,通过在两个波长下发射近红外光来评估各器官的rSO2值。

2. 安装测量传感器

  1. 每次血液透析前,让每位患者仰卧休息至少5分钟。
  2. 将测量传感器分别贴附于前额、右侧季肋区和下肢,以评估脑部、肝脏和下肢肌肉的rSO2
  3. 脑组织氧合监测
    1. 将测量传感器贴附于优势半球的前额部位。
  4. 肝脏氧合监测
    1. 准备超声设备,测量患者肝脏表面至体表的深度,确认该深度在20–30 mm范围内。随后,将测量传感器贴附于右侧季肋区。
      注意:该设备所测得的rSO2值对应于体表下20–30 mm深度的组织。在某些情况下,由于皮下脂肪较厚,肝脏位置可能深于体表30 mm以上。
  5. 肌肉氧合监测
    1. 将测量传感器贴附于右侧或双侧下肢。
  6. 传感器连接与设备启动
    1. 将各传感器连接至设备导线,然后开启设备,开始氧合监测。

3. 穿刺透析瘘管并开始监测

  1. 穿刺透析通路
    1. 穿刺患者的透析通路以开始血液透析(HD)治疗。此时,使用透析机配备的数字血压计测量血压,并用注射器采集血样。
  2. 开始监测
    1. 开始HD治疗后,启动对三个器官组织氧合的监测:脑、肝和小腿肌肉。
  3. HD期间rSO2的监测
    1. 观察各器官rSO2值的变化,并定期测量血压,同时进行HD治疗期间的常规监测,包括心率、静脉压和血容量。确认传感器与导联的贴附部位及其连接状态。

HD 治疗前的脑组织氧饱和度(rSO2)值低于健康受试者,且合并糖尿病(DM)的 HD 患者的脑 rSO2 值低于不合并 DM 的 HD 患者(图 116。此外,尽管在 HD 过程中血压(BP)未下降,组织氧合仍持续存在,但我们偶然观察到因透析中低血压导致的脑和肝组织 rSO2 变化(图 2)。由于实现了连续监测,组织氧合的变化比间歇测量的血压更早被发现。数据以均值 ± 标准误表示。采用非配对样本的方差分析对三组间进行比较。

近红外光谱(NIRS)监测主要用于评估脑组织氧饱和度(rSO2),尤其适用于需要体外循环的心血管或脑血管手术。在包括血液透析(HD)治疗在内的体外循环过程中,某些器官可能出现相对性缺血7,17,18;然而,组织氧合水平是否确实降低仍不明确。血液透析过程中出现的肌肉痉挛或腹痛,可能是由于器官灌注不足导致透析中低血压的先兆症状之一。然而,在血液透析治疗中,目前尚无用于实时评估组织氧合状态的方法。因此,我们关注利用上述方案中的监测设备来评估器官的氧合状况。该方案对血液透析患者无创,且有助于实时确认组织氧合状态的变化。
