摘要

心源性休克定义为持续性低血压,并伴有终末器官低灌注的证据。经皮心室辅助装置(PVAD)用于治疗心源性休克,以改善血流动力学。目前,Impella 是最常用的 PVAD,可主动将血液从左心室泵入主动脉。PVAD 可减轻左心室负荷,增加心输出量,并改善冠状动脉灌注。通常在可行的情况下,PVAD 通过股动脉途径,在心脏导管室借助透视引导下置入。对于严重外周动脉疾病患者,可采用其他替代入路植入 PVAD。本文综述了 PVAD 的作用机制及其在心源性休克治疗中应用的支持性数据。

引言

心源性休克(CS)定义为持续性低血压(收缩压 <90 mmHg 持续 >30 分钟,或需要使用血管加压药或正性肌力药物)、终末器官灌注不足(尿量 <30 mL/h、四肢湿冷或乳酸 >2 mmol/L)、肺充血(肺毛细血管楔压(PCWP)≥15 mmHg)以及心功能下降(心脏指数 <2.2 辐射通量密度公式 \(L/\text{min} \cdot \text{m}^2\)。)1,2,其病因是原发性心脏疾病。急性心肌梗死(AMI)是导致 CS 的最常见原因3。CS 发生于 5–10% 的 AMI 患者中,历史上与显著的死亡率相关3,4。机械循环支持(MCS)装置,如主动脉内球囊反搏(IABP)、经皮心室辅助装置(PVAD)、体外膜肺氧合(ECMO)以及经皮左心房至主动脉装置,常用于 CS 患者5。常规使用 IABP 在 AMI-CS 患者中并未显示出对临床结局或生存率的改善1。鉴于 AMI-CS 的预后较差、在 AMI-CS 患者中开展临床试验的困难,以及 IABP 在 AMI-CS 中应用的阴性结果,临床医生正越来越多地关注其他形式的 MCS。

方案

本方案是我们机构的标准诊疗规范。

1. PVAD 的置入(例如 Impella CP)

  1. 在透视和超声引导下,使用微穿刺针于股骨头下半部分获取股总动脉通路9,10。置入微穿刺鞘,并对股动脉进行血管造影,以确认动脉切开位置恰当11
  2. 在股动脉中置入6 Fr鞘管。
  3. 若怀疑存在髂股动脉病变,将猪尾导管插入腹主动脉下段,对髂股系统进行血管造影,以确认无显著的外周动脉疾病(PAD),以免影响PVAD的置入。若存在中度病变或髂动脉钙化,考虑使用更长的25 cm 14 French鞘管,使鞘管尖端位于腹主动脉相对健康的节段。
  4. 在坚硬的0.035"导丝上,依次使用8、10和12 Fr扩张器逐步扩张动脉切开部位。随后在透视引导下插入14 Fr可剥离鞘管,确保鞘管尖端推进时无阻力。
  5. 给予肝素冲击剂量(约100 U/kg体重),使活化凝血时间(ACT)目标值达到250至300秒。替代抗凝药物包括比伐卢定和阿加曲班。
  6. 使用猪尾导管配合0.035"J形头导丝进入左心室(LV)。撤出J形导丝,测量左心室舒张末压(LVEDP)。
  7. 将套件中提供的长交换导丝(0.018")的尖端塑形,并插入左心室内,使其在左心室心尖处形成稳定的弯曲。
  8. 在置入前确保ACT达到目标值(250至300秒)12,13
  9. 撤出猪尾导管,将泵通过预组装的红色装载腔(例如EasyGuide)沿导丝送入,直至导丝从标签附近穿出。
  10. 在固定导管的同时,轻拉标签以移除红色装载腔。
  11. 在透视引导下,沿0.018"导丝以小步进方式将装置推进至左心室。
  12. 将泵置于左心室内,使其入口位于主动脉瓣下方4 cm处,并确保未接触二尖瓣腱索。若过于靠近心尖,可能引发室性早搏(PVCs)并触发“吸力警报”"suction alarms"。撤出0.018"导丝后立即启动泵,并拉紧多余部分,使泵紧贴主动脉小弯侧。
  13. 监测控制台,确保电机电流呈搏动性,且显示主动脉波形。若显示心室波形,可能需要将泵稍向后撤。
  14. 若需将装置留置原位,应撤除可剥离鞘管,并将预装在装置上的再定位鞘管插入。
  15. 再次通过透视检查装置位置,并在控制台上复查波形。
  16. 在装置置入前和置入后,触诊(或用多普勒检测)远端下肢动脉搏动,包括足背动脉和胫后动脉,并在患者病历中准确记录。
  17. 若无法触及搏动或多普勒信号,可考虑通过装置侧壁的导丝再进入端口,或通过另一通路进行下肢血管造影,以确认下肢血流无梗阻。
  18. 若血流受阻,应在将患者转运至冠心监护病房(CCU)前放置再灌注鞘管。对于存在PAD且发生梗阻性血流风险较高的患者,强烈建议在置入14 Fr鞘管前(即上述步骤1.4之后)预先放置再灌注鞘管。
  19. 由经过PVAD使用培训的医护人员在重症监护病房(CCU)内对接受PVAD治疗的患者进行监测。

2. 术后护理

  1. 应用无菌敷料。
  2. 进入皮肤时,将装置置于45°角(在重新定位鞘下方放置纱布有助于维持此角度)。若未做到这一点,可能导致动脉切口渗血,进而形成血肿。此外,缝合时施加向前的压力有助于防止装置移位并预防出血。
    注意:使用膝关节固定器固定下肢也可限制装置移位,同时提醒患者不要弯曲或移动患肢。但固定不宜过紧,以免影响血液循环。
  3. 继续进行常规脉搏检查(可触及或使用多普勒)。

3. 定位

  1. 根据床旁即时超声的可用情况,选择在转运前或到达心脏重症监护室后立即进行床旁经胸超声心动图检查,以确认装置位置是否恰当。
  2. 采用胸骨旁长轴切面评估装置位置;若无法获得胸骨旁长轴切面,可改用剑下切面。理想情况下,从主动脉瓣至装置入口的距离应为 3–4 cm,以确保装置位置正确。
  3. 通过超声心动图观察装置与二尖瓣之间的相对位置。
  4. 当需要调整装置位置时,先将装置转至 P2 档位,然后松开无菌罩上的锁定机制,以推进或回撤装置。若猪尾端或入口过于靠近二尖瓣,可在推进或回撤过程中施加扭转力。
  5. 将装置锁定在新位置,并记录新的位置信息。
  6. 随后,将装置调至所需的辅助支持水平。
  7. 提高支持水平后,需重新评估装置位置,因为装置在转速升高时可能向前移位。
    注意:若装置已被拉回至主动脉瓣上方,建议在导管室通过透视引导下重新调整位置。

4. 断奶

  1. 当血管加压药/正性肌力药物已减至低剂量或完全停用时,可考虑开始撤机。应持续监测血流动力学指标,维持心输出功率(CPO)> 0.6 W。需密切监测右心室(RV)血流动力学,目标为维持右心房压(RAP)<12 mmHg,肺动脉脉动指数(PAPI)>1.014。还应每2–6小时检测一次pH值、混合静脉血氧饱和度和乳酸水平,以评估心脏做功及终末器官灌注情况。
  2. 每2小时降低1–2个功率等级,同时观察CPO、PAPI、RAP、平均动脉压(MAP)及尿量。若CPO下降至<0.6 W,RAP开始升高,尿量减少超过> 20 mL/h,或MAP <60 mmHg,则应将功率调回至先前水平。

5. 去除12

  1. 使用血管闭合装置关闭动脉切开入路,在撤出大口径鞘管时完全展开装置14。临时性血管内球囊填塞或"干区闭合技术"是确保大口径入路部位止血的有效且安全的方法15
  2. 将设备调至P1模式,将其回撤至主动脉,随后切换至P0模式,并在导管撤出体外时断开设备与控制台的连接。
    1. 注意:由于可能导致主动脉反流,设备在P0模式下不应跨过主动脉瓣停留。
  3. 若考虑手动止血,需等待活化凝血时间(ACT)<150,并按每French尺寸压迫3分钟。

如果发生大口径通路相关的并发症(如大出血和急性肢体缺血),PVAD 的血流动力学益处可能会被显著抵消28,29。因此,必须尽量减少该装置相关风险与并发症的发生。

为了减少穿刺部位并发症并降低穿刺尝试次数,在获取股动脉通路时应使用超声和透视引导10,30。采用微穿刺技术可使操作者在发现穿刺位置不当时将创伤降至最低9。在置入经皮心室辅助装置(PVAD)前进行主动脉-髂动脉造影,也有助于选择更合适的穿刺部位15。对于大口径通路患者,血管闭合装置和血管内球囊填塞在实现止血方面效果显著,应在装置移除时尽可能使用15,
