目前在血管疾病(如外周动脉疾病(PAD))的研究中,尚缺乏满足需求的临床前动物模型。尽管在诊断和治疗方面已取得显著进展,2018年全球PAD患者仍超过2亿人1,且患者数量持续增长。尽管已有多种新型治疗策略被报道2,3,4,5,6,7,但这些治疗方法成功转化为临床应用仍面临巨大挑战。因此,迫切需要建立可靠且具有临床相关性的in vivo实验模型,以模拟人类疾病的病理状态,从而深入研究这些新型治疗手段在治疗PAD中的潜在机制和有效性6,7

高脂血症和动脉粥样硬化(AS)是外周动脉疾病(PAD)发生的主要危险因素。载脂蛋白E基因敲除(ApoE-/-)小鼠在高脂饮食条件下表现出脂肪代谢异常和高脂血症,并继而形成动脉粥样硬化斑块,因此ApoE-/-小鼠是模拟临床相关PAD模型的最佳选择。目前,临床前的后肢缺血(HLI)动物模型通常通过股动脉双结扎(DLFA)方法建立,该方法是全球实验室最广泛采用的技术8,9,10,11,12,13,14,15,用于模拟慢性基础上的急性缺血。然而,该方法通常需要较大且具有侵入性的切口。此外,它不可避免地导致实验动物(尤其是小鼠)遭受更严重的疼痛损伤和炎症反应,从而影响后续实验结果5,6,16,17。本文介绍了一种在APOE-/-小鼠中建立慢性基础上急性HLI模型的方法,仅需使用极小切口即可完成。

注意:所有实验操作均按照欧盟指南 EC 2010/63/EU 进行,并已获得德国当地法律法规的批准(35-9185.81/G[1]239/18)。使用十只雄性 ApoE-/- C57BL/6J 背景的小鼠,体重为 29.6–38.0 g,在 12 小时光照/12 小时黑暗循环条件下饲养,并饲喂西方饮食(含 1.25% 胆固醇和 21% 脂肪)和水 随意摄取 持续12周,从8周龄开始。在20周龄小鼠上按如下所述进行HLI手术。

1. 在ApoE-/-小鼠中诱导HLI

  1. 准备手术所需的设备和工具(参见材料表图1)。 使用前通过高压灭菌法对手术器械进行灭菌,并在手术过程中使用玻璃珠灭菌器。
  2. 在所有手术操作前,通过皮下注射(S.C.)给予小鼠咪达唑仑(5 mg/kg)、美托咪啶(0.05 mg/ml/kg)和芬太尼(0.5 mg/kg)混合物进行麻醉。
    1. 麻醉起效后,使用兽用眼膏涂抹眼睛以防止干燥,并确认前肢和后肢无足底回缩反射。
  3. 随后将小鼠置于加热垫上,以维持核心体温在约37 °C。使用棉签和脱毛膏,小心清除右侧后肢皮肤上的毛发。
    注意:脱毛膏应足量使用,覆盖整个后肢,尤其是将要切开的腹股沟区域。脱毛时间应少于3分钟,之后用湿润的棉签擦拭2至3次以清除脱毛膏。
  4. 将小鼠仰卧位放置在解剖显微镜下的加热垫上。使用皮肤消毒剂 (参见 材料表) 对小鼠皮肤进行消毒。随后使用尖头镊子和手术剪在腹股沟区域中部做一个约3-4 mm的切口。手术步骤示意图见图2
  5. 借助精细尖头镊子小心去除皮下脂肪组织,暴露近端股骨神经血管束。使用精细尖头镊子小心刺破股鞘膜。用生理盐水湿润的棉签将股动脉(FA)小心地与股神经(FN)和股静脉(FV)分离。
  6. 将两根7-0可吸收缝线穿过近端FA,并使用弹簧剪打双结,在两结之间切断FA。
  7. 为暴露远端FA,将一根7-0可吸收缝线穿过切口下缘,并轻轻将切口牵拉至后肢右侧膝部区域。
  8. 小心拨开皮下组织以暴露神经血管束。使用精细尖头镊子刺破股鞘膜,并将FA从FV和FN中分离出来。
  9. 将两根7-0可吸收缝线穿过远端FA,并打双结。使用弹簧剪在两结之间切断FA。
    注意:每只小鼠的左后肢未进行结扎,作为对照。
  10. 随后使用6-0可吸收缝线缝合切口。将小鼠置于清洁笼子中的加热垫上,并持续监测其生命体征直至苏醒。术后给予镇痛药物:皮下注射布托啡诺(0.1 mg/kg体重,每8小时一次,持续48 h)。术后48 h开始,在饮水中给予安乃近(24 mg/5 mL水,相当于每日4次,每次200 mg/kg)。

2. 磁共振成像

注意:DLFA 手术后一天,小鼠必须接受 MRI 扫描以评估 FA 阻塞情况。

  1. 将小鼠放入透明诱导室中,使用含1.5-2%异氟烷的空气麻醉小鼠,直至其翻正反射消失。
  2. 将小鼠置于配备咬合固定器的加热动物床面上,并通过激光控制系统将其定位至磁体方向。维持体温在37±1 °C。
  3. 在图像采集过程中,持续使用含1.5-2%异氟烷的空气维持麻醉,并通过压力探头监测呼吸。
  4. 采用三维(3D)飞行时间(TOF)血管成像序列,在横断面方向采集图像,参数为回波时间(TE)/重复时间(TR)/翻转角(FA)= 2 ms/12 ms/13°,信号平均4次,采集矩阵为178 x 144,重建至256 x 192,共采集121层切片,最终获得各向同性分辨率为0.15 mm3的图像。为抑制静脉信号,在后肢远端位置设置饱和带。

3. 临床评估与随访

  1. 评估1中的功能恢复情况st, 3rd, 5th,以及 7th 术后数天内采用功能评分Tarlov量表18,19 (表1).

4. 组织学评估

  1. 手术后第7天,注射戊巴比妥钠(115 mg/kg)处死小鼠。
  2. 通过左心室灌注含1%多聚甲醛(PFA)的磷酸盐缓冲液(PBS)(每只小鼠100 mL)。将小鼠双侧腓肠肌(Gm)在4% PFA中固定,4 °C过夜。
  3. 按照先前描述的方案20将样本包埋于石蜡中。
    1. 使用切片机对石蜡包埋的组织块切片,厚度为4–5 µm。借助圆形画笔,将切下的组织切片放入维持在42 °C的水浴中。
    2. 将载玻片以45°角插入水中,并小心地将其置于待收集的一组切片下方。
    3. 小心地将载玻片从水中提起,使切片附着于载玻片上,并在37 °C的台式孵育箱中干燥过夜。
  4. 对石蜡切片进行苏木精-伊红(HE)染色。
    1. 将带有切片的载玻片放入玻片架中。准备3个盛有新鲜二甲苯的容器,将载玻片依次放入每个容器中5分钟,以脱去切片中的石蜡。
    2. 依次将切片浸入96%、80%、70%、50%、30%乙醇及去离子水中,每步5分钟,进行再水化。
    3. 用苏木精溶液染色10分钟。
    4. 将切片转移至盛有去离子水的容器中,并在流动的自来水下冲洗5分钟。
    5. 在显微镜下观察苏木精染色强度。若染色可清晰识别细胞核,则继续下一步操作;若染色强度不足以识别细胞核或染色较浅,需将载玻片再次放入苏木精溶液中1分钟,重复水洗步骤(步骤4.4.4),然后再次检查。
    6. 用伊红-Y溶液复染5分钟。
    7. 依次将切片浸入去离子水以及30%、50%、70%、80%、96%乙醇中,每步10秒,进行脱水。随后,将切片依次放入3个盛有新鲜二甲苯的容器中,每步10秒。
    8. 将载玻片水平放置于显微镜载玻片储存盒中,切片面朝上。在载玻片上滴加足量封片介质,并加盖盖玻片进行封片。
  5. 对石蜡切片进行免疫组织化学(IHC)染色。
    1. 重复步骤4.4.1–4.4.2的脱蜡和再水化操作。随后,将切片浸入含10 mM柠檬酸钠缓冲液(pH 6)的容器中,并在微波炉中加热至沸腾。
      注意:由于样品过热或加热不足会导致染色不一致,应将温度维持在略低于沸点的水平,持续10分钟。
    2. 接着,在室温下静置冷却30分钟。之后,用PBS洗涤切片三次,每次5分钟。小心擦干样品周围区域,并使用疏水笔在样品周围画一个大圆圈。
      注意:切勿触碰样品。使用疏水笔标记可形成疏水边界,有助于减少抗体溶液的使用量。
    3. 将切片置于含0.3% H2O2的PBS中孵育10分钟,以淬灭内源性过氧化物酶活性。随后,在湿盒中室温下用400 µL封闭缓冲液(含3%牛血清白蛋白和0.3%非离子型去污剂的PBS)封闭切片1小时。
    4. 用PBS洗涤切片5分钟。然后加入100–400 µL稀释后的抗CD31抗体(1:250),确保完全覆盖切片。接着,在湿盒中4 °C孵育过夜。
      注意:确保切片被抗体溶液完全覆盖。
    5. 去除一抗,用PBS洗涤切片三次,每次5分钟。
    6. 将1滴DAB浓缩液加入1 mL DAB稀释液中,充分混匀,配制3,3'-二氨基联苯胺(DAB)混合液。随后,向切片上加入100–400 µL DAB混合液,并密切肉眼观察约2分钟,直至出现可接受的染色强度。
      注意:确保切片被DAB混合液完全覆盖。
    7. 之后,在流动自来水下冲洗5分钟。按照步骤4.4.3–4.4.5所述进行苏木精染色。
    8. 按照步骤4.4.6所述进行伊红-Y染色,按照步骤4.4.7所述进行脱水操作。
    9. 使用封片介质加盖盖玻片封片。使用ImageJ软件在5个随机选取的视野(40倍)中估算CD31阳性区域所占百分比(%),该指标可作为微血管密度的评估参数,方法如前所述21

5. 统计分析

  1. 使用统计分析软件将结果表示为均值 ± 标准差,并对比较进行非配对 t 检验。当 P < 0.05 时,认为差异具有统计学显著性。

ApoE-/- 小鼠的特征
对10只小鼠成功实施了DLFA手术以建立HLI模型,术后无小鼠死亡。为追踪体重变化,在DLFA手术前(术前)和术后7天分别对小鼠进行称重。术前体重范围为29.6至38.0 g(平均34.74 ± 2.47 g),术后体重范围为26.5至34.1 g(平均30.77 ± 2.15 g),显著低于术前体重(P < 0.05,图3A)。手术时间范围为15至47分钟(平均34.2 ± 8.82分钟,不包括麻醉时间)。10只小鼠的切口大小范围为3至5 mm(平均4.2 ± 0.63 mm)。
