内镜超声引导下胆道引流:恶性胆道梗阻中的内镜超声引导下肝胃吻合术

内镜超声引导下胆道引流术(EUS-BD)是恶性胆道梗阻时进行胆道减压的一种替代方法。本文介绍了一例不可切除的恶性肝门部胆道梗阻患者接受内镜超声引导下肝胃吻合术(EUS-HGS)的技术操作。

摘要

对于无法手术切除的恶性胆道梗阻患者,通常需要进行胆道引流以减轻胆道系统的压力。在可行的情况下,内镜逆行胰胆管造影术(ERCP)是主要的胆道引流方法。当ERCP失败时,经皮经肝胆道引流术(PTBD)可作为补救性措施。而内镜超声引导下的胆道引流术(EUS-BD)则提供了一种可行的替代方案,其中一种方法是内镜超声引导下肝胃吻合术(EUS-HGS)。本文将介绍在无法手术切除的恶性肝门部胆道梗阻病例中,采用EUS-HGS技术实现胆道引流的具体操作过程。

本病例为一名71岁女性,出现无痛性黄疸并伴有体重减轻,持续2周。计算机断层扫描(CT)显示肝门区存在一个4 × 5 cm的肿瘤,伴有淋巴结肿大及肝转移。对该病灶行超声内镜引导下细针穿刺活检(FNB),结果符合胆管癌。就诊时其血清总胆红素水平为212 µmol/L(<15)。

采用线性超声内镜定位肝脏左叶扩张的肝内胆管(IHD)。确定第3段扩张的肝内胆管后,使用19 G穿刺针进行穿刺。在X线透视引导下注入造影剂以显示肝内胆管。随后使用0.025英寸导丝进入肝内胆管。接着使用6 Fr电切扩张器及4 mm胆道球囊扩张器对瘘道进行扩张。在X线透视引导下置入一根长度为10 cm的部分覆膜金属支架。支架远端置于肝内胆管内,近端则置于超声内镜的工作通道中,随后释放至胃腔内。患者于术后第3天出院。术后第2周和第4周随访显示,血清总胆红素水平分别为30 µmol/L和14 µmol/L。上述结果表明,EUS-HGS是一种治疗不可切除的恶性胆道梗阻所致胆汁引流的安全方法。

引言

恶性胆道梗阻患者在就诊时常已处于不可切除的晚期阶段1,2因此,姑息性内镜胆道减压常被用于这些病例的治疗3,4,5根据目前的建议,内镜逆行性胰胆管造影术(ERCP)是可行情况下的首选胆道引流方法;若该方法失败或存在禁忌证,则采用经皮经肝胆道引流术(PTBD)作为补救措施6,7,8然而,PTBD 存在某些相关并发症,包括败血症、胆管炎、出血、电解质丢失、渗漏、伤口感染和局部不适;据报道,这些并发症的发生率高达 53.2%9内镜超声引导下胆道引流术(EUS-BD)的出现为填补这一空白提供了一种可行的替代性胆道引流方法。EUS-BD 的主要技术是利用内镜超声成像引导进入胆道系统 通过 胃肠道以对梗阻的胆道系统提供治疗性减压。

EUS-BD 首次于2001年实施,此后,这种胆道引流方法逐年发展完善10。目前常用的 EUS-BD 方法包括:EUS 引导下的胆肠吻合术(EUS-CDS)、EUS 引导下的肝胃吻合术(EUS-HGS)、EUS 引导下的顺行支架置入术(EUS-AS)以及 EUS 引导下的会师技术(EUS-RV)11,12。迄今为止,EUS-BD 的适应证包括:ERCP 失败的患者、因十二指肠梗阻导致乳头无法接近的患者,以及具有解剖结构改变的术后患者13,14,15

EUS-HGS 包括经胃壁将左肝内胆管引流至胃内。其主要优势在于可实现内引流,这更符合生理状态,且最重要的是,与经皮经肝胆管引流术(PTBD)相比,可显著提高患者的舒适度12,16。该方法适用于肝门部及远端恶性胆道梗阻。本文以一例不可切除的恶性肝门部肿瘤为例,介绍EUS-HGS作为EUS-BD方法之一的技术操作过程。

一名71岁女性患者出现无痛性黄疸,并在2周内体重减轻4公斤。体格检查发现患者存在巩膜黄染,腹部检查无明显异常。计算机断层扫描(CT)显示肝门区存在一个4 × 5 cm的肿瘤,累及右肝管分叉处、肝总管汇合处及肝外胆管,导致明显的肝内胆管扩张,并伴有淋巴结肿大和肝转移(图1A)。对该病灶行超声内镜引导下细针穿刺活检(FNB),结果显示为腺癌,免疫组化CK7和CA19-9阳性,符合肝门部胆管癌的诊断(图1B)。患者就诊时总胆红素水平为212 µmol/L(<15),CA19-9水平为305 U/mL。为缓解胆道梗阻,实施了超声内镜引导下肝胃吻合术(EUS-HGS)。

方案

该方案遵循马来亚大学医学中心的伦理准则。已获得患者的书面知情同意,并向其详细解释了操作过程。同时,患者也同意为教育目的制作该操作过程的视频。

1. 体位与镇静

  1. 将患者置于俯卧位。通过静脉输注丙泊酚进行中度镇静。确保由麻醉师根据需要调整输注剂量。

2. 确定合适的靶向扩张左肝内胆管

  1. 推进线性超声内镜,确保其通过食管胃交界处。
  2. 将回波内镜的顶端置于胃腔的小弯侧,然后识别扩张的左肝内胆管(IHD)。
  3. 进行透视成像以确定超声内镜的位置。超声内镜的顶端必须如前所述置于胃腔的小弯侧。在整个操作过程中,应始终维持超声内镜的位置,并通过定期透视成像检查加以稳定。
  4. 识别扩张的第三段IHD。轻微向上调整回声内镜尖端的角度,以利于下一步的穿刺操作。
    ​注意:后续操作步骤涉及附件的插入和更换 通过 在整个操作过程中,超声内镜的工作通道。

3. 针穿刺进入目标扩张的左肝内胆管

  1. 进行多普勒超声检查,以确保目标肝内胆管周围无血管走行。
  2. 使用19 G穿刺针穿刺第3段肝内胆管(图2A)。用预装7 mL造影剂的10 mL注射器轻柔抽吸胆汁,以确认胆道穿刺成功。
  3. 随后注入造影剂,使左侧肝内胆管及其余胆道系统显影。

4. 导丝操作

  1. 在X线透视引导下,使用0.025英寸的导丝进入左侧肝内胆管。
  2. 将导丝经肝内胆管对侧插入右侧肝内胆管(图2B)。
    ​注意:理想情况下,应将导丝深入操纵至胆总管;然而,由于本例中存在阻塞性肝门部肿瘤,此操作无法实现。

5. 瘘管通道扩张

  1. 使用6 Fr电切刀以及4 mm胆道球囊扩张器扩张瘘管(图2C)。
    注意:需要扩张瘘管以利于后续胆道支架的置入。
  2. 球囊扩张约持续5秒。
    注意:操作过程中需同时通过超声和透视影像监测器械的位置,确保导丝可见且超声内镜位置保持不变。此举可确保在更换器械时顺利过渡,至关重要。

6. 支架置入与释放

  1. 在荧光透视引导下置入一根直径10 mm、部分覆膜的胆道支架(长度为10 cm)(图3A)。
    注意:该支架远端有3 cm未覆膜部分,近端有7 cm覆膜部分。未覆膜部分必须位于肝内胆管内(图3C)。支架的覆膜部分约有3 cm保留在胃腔内,以防止支架向内移位(图3B)。
  2. 确保支架远端在肝内胆管(IHD)内打开。将支架近端置于超声内镜的工作通道内,随后释放至胃腔,并观察到胆汁经支架内流出。

结果

该操作在约30分钟内完成。操作后未出现并发症,患者于次日恢复经口进食。胆红素水平降至92 µmol/L,术后第3天出院。复查CT影像显示支架位置良好,胆道梗阻已解除。随访结果显示,术后2周胆红素水平为30 µmol/L,术后4周为14 µmol/L(表1)。

2020年6月至2021年6月期间,本机构共对15例不可切除的恶性胆道梗阻患者实施了EUS-HGS。平均年龄为65.5(±10.1)岁,男性占46.7%。最常见的病因为胰腺癌(46.7%),而解剖结构改变(33.3%)是最常见的手术指征。临床成功定义为术后4周内胆红素水平下降超过50%。技术成功率为86.7%,临床成功率为81.8%,与手术相关的不良事件发生率为6.7%。唯一发生的手术相关不良事件为出血,需行经动脉栓塞治疗。1例患者因术后102天发生支架再闭塞而需再次干预,术中置入双猪尾塑料胆道支架。 通过 现有的金属支架。两名患者因癌症晚期在不到4周内死亡。我们的临床经验结果如图所示。 表2.

讨论

上述病例描述表明,在治疗胆道恶性肿瘤时,与现有的ERCP和PTBD等方法相比,EUS-HGS可作为一种替代性的胆道引流方法。在上述步骤中,准确识别并进入正确的肝内胆管、导丝操作以及支架释放是确保该手术能够成功且安全实施的三个关键步骤。

在选择理想的肝脏节段以进行胆道通路建立时,可选择第2段和第3段的肝内胆管(IHDs),但通常优先选择第3段而非第2段17这是因为在第二阶段,所使用的器械和附件有可能穿过食管进入纵隔,从而导致纵隔炎和纵隔气肿等并发症。18此外,支架在食管内打开的风险可能导致延迟性食管穿孔,进而累及纵隔19. 话虽如此,获取 通过 只要超声内镜的位置位于胃腔内,第二区段并非禁忌。
