使用单晶压电聚焦冲击波源治疗肩袖钙化性肌腱炎的治疗方案

肩关节钙化性肌腱炎是一种相对常见的疾病,具有多种治疗方法。本文讨论了由单晶压电装置产生的聚焦冲击波的适应证,描述了一种治疗方案,并展示了初步治疗结果。

摘要

聚焦冲击波已成为治疗肩部钙化性肌腱炎的一种高效且无创的治疗选择。目前有三种类型的聚焦冲击波发生器:液电式、电磁式和压电式。根据我们的文献检索,尚无关于使用单晶压电发生器治疗肩部钙化性肌腱炎的疗效报道。在一项连续的回顾性研究中,共纳入23例冈上肌腱钙化Gärtner I型和II型的患者,我们对其进行了三次高能压电聚焦冲击波治疗(每次治疗4,000次脉冲,频率为6 Hz)。在末次随访时(平均随访14个月),影像学检查显示,82.6%的病例钙化灶完全吸收;8.7%的病例钙化灶部分消失;其余8.7%的病例则无明显变化。单晶压电发生器的治疗成功率与已报道的液电式和电磁式设备相当。

引言

钙盐结晶沉积可出现在肌肉骨骼系统的不同部位,但最常见于肩部区域。Gondos1 报道,69%的钙化病例发生在肩部。钙化性肩袖腱病的特征是肩袖肌腱内存在羟基磷灰石沉积。据估计,该病在普通人群中的患病率介于2.7%至20%之间2

肩关节钙化性肌腱炎通常影响30至60岁的患者2。女性发病率(57%–76.7%)高于男性3。钙盐沉积最常见于冈上肌肌腱远端4,但也有报道显示可发生于冈下肌、小圆肌、肩胛下肌以及肱二头肌长头4

30至60岁的女性,若钙化灶长度超过1.5 cm,出现症状的概率最高5。尽管该病常可自行缓解,但这一过程往往会被中断。在此类情况下,会出现疼痛和功能障碍等症状,必须采取积极的治疗措施。

Gärtner的放射学分类6将影像分为三种类型。I型表现为密度较高、边界清晰的影像,对应于形成期。II型影像呈混合表现,可表现为密度较高但边界弥散,或透亮度较高但边界清晰。最后,III型为吸收期的特征性表现,呈现为透亮度较高且边界弥散的沉积物。对于Gärtner I型和II型,必须采取积极的治疗措施,包括体外冲击波治疗、超声引导下干预或手术;而对于III型病例,短期内自发吸收的可能性非常高6

初始治疗首选保守疗法。经典保守治疗通常包括休息、镇痛药、非甾体与甾体类抗炎药、康复治疗以及局部注射。研究表明保守治疗效果良好,尤其在钙化物吸收期效果更佳,但仍有27%至39%的病例报告保守治疗失败7,8,9。已有多个预后因素被证实对保守治疗效果具有显著影响7,8。双侧肩部受累、钙化灶体积较大、钙化位于肩峰前部区域,以及钙化沉积物向内侧扩散超过肩锁关节水平,均属于预后不良的因素7,8。 Gärtner III期钙化以及超声检查中钙化灶未出现声影消失,被认为是保守治疗预后良好的预测指标7

当保守治疗失败时,许多患者最终会演变为慢性肩痛携带者,其临床特征与慢性非钙化性肩袖肌腱病相似。保守治疗失败后的常规替代方案是手术治疗。Gschwend10提出了肩袖钙化灶手术治疗的三个明确指征:症状持续进展、持续且难以控制的疼痛,以及保守治疗无效。手术治疗可采用开放或关节镜方式进行。尽管开放手术在历史上曾取得良好疗效11,但关节镜技术近年来已日益普及12,13。肌肉骨骼超声及超声引导下干预(UGI)在近年得到显著发展,并已在临床实践中广泛应用14,15,16

当保守治疗失败后,在采取侵入性治疗之前,体外冲击波治疗(ESWT)已成为一种有效的选择。其治疗作用不仅限于机械效应,更基于机械转导机制——即细胞能够识别机械刺激并产生生物学反应的现象17。然而,冲击波治疗仍存在局限性。与仅依赖冲击波机械效应的碎石术不同,冲击波治疗还需患者产生相应的生物学反应,而这种反应并非总能发生。

通用术语 "体外冲击波" 包含两种不同的技术:聚焦冲击波和径向压力波17,18,19两种技术均具有治疗效果,但在物理特性和适应证方面存在差异。聚焦冲击波具有较宽的频率范围(约150 kHz至100 MHz)、较大的压力振幅(高达150 MPa),上升时间短、脉冲宽度小,随后伴随一个低应力波(可达-25 MPa)。18,19聚焦冲击波由电液、电磁和压电源产生。17,18,19.

径向压力波是一种声波,其压力峰值可达30 MPa,上升时间远短于聚焦冲击波(约3 µs)18,19。径向压力波通过压缩空气在圆柱形导管内加速一个弹丸产生。弹丸撞击导管末端的施用器,从而产生向目标组织扩散的径向压力波17,18,19

聚焦冲击波治疗肩袖钙化具有等级 "A" 的推荐级别17。这意味着有高质量的科学证据支持,且得到具有一致研究结果的 I 级研究的支持。而对于径向波,在治疗肩袖钙化方面的推荐等级为 "I",表示现有证据不足以做出推荐17

聚焦冲击波治疗肩部钙化性肌腱病的疗效已与开放手术20和关节镜手术21进行了比较,结果相当。然而,冲击波治疗的并发症发生频率更低、严重程度更轻20,21,且该方法具有更高的成本效益。Haake22报道,手术治疗费用(€13,400–23,450)与聚焦冲击波治疗费用(€2,700–4,300)之间存在显著差异。这一结果与其他研究一致,后者显示冲击波治疗的成本较关节镜手术降低五至七倍23,24。也有研究将冲击波治疗与超声引导下的干预措施进行比较,但结果存在争议15,25。多项研究4,17,26,27,28,29,30,31,32,33报道,在治疗肩部钙化性肌腱炎时,较高能量水平的治疗效果更佳。Verstraelen27在一项Ⅰ级证据研究中报道,使用高能量可显著提高钙化物的吸收率。这一优势使聚焦式设备明显优于径向式设备,因为前者能够产生更高的能量水平。大量研究已报道了液电式4,17,34,35和电磁式4,17,36,37,38聚焦设备的良好疗效。另有研究报道使用多晶体压电设备治疗肩袖钙化39。截至目前,尚未见有关单晶体压电设备治疗技术及其疗效的报道。

本报告旨在描述使用单晶压电装置的治疗方案,并报告初步结果。

方案

该方案遵循布宜诺斯艾利斯英国医院人类研究伦理委员会的指导原则。

1. 患者评估

  1. 临床评估
    1. 对患者进行临床评估,以排除来自其他解剖区域或肩部其他相关病变所引起的症状。
    2. 包括肩部视诊和触诊,评估主动与被动活动范围,并实施诱发疼痛的操作及完整性检查。此外,还需评估颈椎和肘关节。
  2. 影像学评估
    1. 采用肩关节前后位(AP)X线投照、肩胛骨侧位及腋位影像。
    2. 根据 Gärtner6 分类法按钙化病变的演变阶段进行分类。必要时申请磁共振成像(MRI)以排除肩关节其他相关病变。
  3. 纳入标准
    1. 纳入经分类为 Gärtner I 型和 II 型钙化、且已接受至少3个月保守治疗无效的患者。
    2. 治疗肩袖肌腱病时,切勿向肩峰下间隙注射皮质类固醇,因其对肌腱弹性纤维具有不利影响40,41
  4. 排除标准:排除患有 Gärtner III 型钙化或患侧肩关节存在其他相关病变的患者。

2. 应用技术

  1. 患者体位:将患者置于舒适的位置,并确保能够充分暴露待治疗区域。
    1. 冈上肌或冈下肌肌腱位置:将患者置于仰卧位或坐位进行治疗。
    2. 肩胛下肌:将患者置于仰卧位进行治疗。
    3. 小圆肌:将患者置于坐位、侧卧位或斜卧位,以便更好地接触肩关节后部。
  2. 耦合垫片
    注意:提供三种尺寸的垫片,每种尺寸对应特定的治疗聚焦深度(图1)。
    1. 根据聚焦点所需的穿透深度选择合适的耦合垫片尺寸(表1)。在肩部钙化病例中,垫片的选择需根据患者的体型和钙化部位而定。最常用的尺寸为大号和中号。
  3. 待治疗区域的定位:结合解剖标志确定待治疗区域的位置,必要时可借助超声图像(图2)。
    注意:准确定位钙化区域并将其作为治疗焦点至关重要4,42。利用解剖标志并结合超声定位有助于准确识别目标位置。是否需要使用诊断性超声进行定位尚存争议,因有研究未发现其使用带来显著差异43。若将聚焦波应用于大血管、神经或靠近胸膜的区域,则存在风险44。应采用避开这些结构的解剖入路。
  4. 操作者体位:使用此类设备时,操作者需手持治疗头。应保持符合人体工学的操作姿势。
    注意:操作者必须始终关注患者的状况,因已有治疗过程中发生晕厥的罕见报道44
  5. 麻醉:不使用麻醉。禁止使用局部麻醉,因为液体的存在会改变组织的声阻抗。通常情况下,患者对此类设备治疗的耐受性良好,即使在高能量水平下操作亦如此。
  6. 接触凝胶:在待治疗区域的皮肤上涂抹足够的接触凝胶。
  7. 导航控制:打开计算机并进入触摸屏界面。选择屏幕顶部的手动选项,即可显示各项治疗参数的控制界面。使用旋转选择旋钮设置参数,包括强度、频率和冲击次数。
  8. 强度:设定治疗的初始强度。所用设备的能量通量密度(EFD)以毫焦耳每平方毫米组织(mJ/mm2)为单位进行分级。
    1. 根据患者的耐受程度逐步增加强度。起始时使用低强度,至少达到0.40 mJ/mm2以上的能量水平,以获得可重复的治疗效果19,20,21,22,23,24,25,26。通常使用的平均最大强度为0.50 mJ/mm2
  9. 频率:选择所需频率。设备频率范围为1–25 Hz,建议使用4–6 Hz。
  10. 冲击次数:使用旋转选择旋钮设定冲击次数。每次治疗至少应用4,000次冲击。
  11. 治疗过程中的参数调整:在治疗过程中可调整频率,尤其是强度。可通过治疗头上的显示屏在不停止治疗的情况下便捷地修改参数。
  12. 治疗间隔:每周进行一次治疗,共进行三次。

3. 处理后方案

  1. 首次治疗后,应告知患者,在约5%的病例中,可能会出现伴有剧烈疼痛的急性吸收过程。如果出现不适,建议患者在疼痛区域进行冰敷,每次冰敷时间不超过10分钟。
    注意:建议使用对乙酰氨基酚,而非非甾体抗炎药。冲击波可调节炎症过程,在治疗期间最好避免通过其他方式干扰该过程。
  2. 制动:无需对治疗肢体进行固定。建议患者避免可能引起疼痛的剧烈活动和大幅度关节活动。
  3. 康复计划
    1. 随着治疗次数的增加,若患者耐受良好,可逐步增大活动范围,并加入加强肱骨头下压肌群的锻炼。
    2. 若未出现症状复发,可加入针对关节活动极限范围的恢复性训练。同时建议加强三角肌三个部分及肩胛稳定肌群的力量训练。
  4. 影像学随访:
    1. 在治疗结束后6周进行首次X线检查,12周时进行第二次检查。若在12周后观察到部分吸收过程尚未完成,则继续进行影像学随访。
    2. 若未见任何变化,可选择新的治疗模块或转为侵入性治疗手段。
      注意:许多患者在首次复查X线片中即可观察到显著改变;而在其他病例中,与基线检查相比变化程度不一,甚至部分患者在长期随访X线片中也可能未见明显变化。

结果

本机构开展了一项针对因肩袖肌腱钙化沉积引起肩痛患者系列病例的回顾性研究。纳入标准为 Gärtner 分期 I 和 II 期的钙化,以及至少经过3个月保守治疗但疗效不满意者。排除标准为 Gärtner III 期钙化、患侧肩关节合并其他病变、既往接受过局部皮质类固醇注射,以及患侧肩关节有手术史的患者。

研究组由23例连续患者组成(表2),其中女性13例,男性10例,平均年龄为52.8岁。在82.6%的病例中(19个肩关节)累及冈上肌,13%的病例中(3个肩关节)累及冈下肌,4.4%的病例中(1个肩关节)累及肩胛下肌。本组所有病例均仅累及一条肌腱。

所有患者均采用先前所述的治疗方案。随访时间最短为6个月,平均为14个月(6–30个月)。与初始检查相比,若沉积物消失至少90%,则视为完全消失;若消失程度在40%至90%之间,则视为部分消失;若沉积物与初始状态相同,则归为无变化组;若吸收程度低于40%,则认为无显著意义。在所有病例中,钙化灶的大小均通过肩峰下间隙的冠状位影像(前后位)进行测量。所有既往...

讨论

本研究在一组回顾性评估的肩关节钙化性肌腱炎患者中,应用单晶压电装置产生的聚焦冲击波,取得了令人鼓舞的结果。根据我们进行的文献检索,这是首项报道单晶压电装置治疗效果的研究。最近,Louwerens39发表了一项使用压电冲击波装置治疗肩袖钙化的研究,但该作者在其研究中采用的是含多个压电晶体的发生器。
