非插管视频辅助胸腔镜手术

本文介绍了一种保留自主呼吸的非气管插管胸腔镜手术方案。

摘要

在全身麻醉下进行双腔气管插管是目前肺叶切除术、肺楔形切除术和全肺切除术中最常用的插管技术。然而,全身麻醉伴随气管插管的使用具有较高的肺部并发症发生率。保留自主呼吸的无气管插管麻醉是一种替代方案。无插管技术可最大限度地减少气管插管和全身麻醉带来的不良影响,例如与插管相关的气道损伤、通气相关性肺损伤、残余神经肌肉阻滞以及术后恶心和呕吐。然而,目前许多研究并未详细描述无插管操作的具体步骤。本文介绍了一种简洁的保留自主呼吸的无插管方案,用于开展胸腔镜辅助手术。文章明确了从无插管转为插管麻醉所需的转换条件,并讨论了无插管麻醉的优势与局限性。本研究共对58例患者实施了该干预措施,并展示了回顾性研究的结果。与采用插管全身麻醉的患者相比,接受无插管胸腔镜手术的患者术后肺部并发症发生率更低、手术时间更短、术中出血量更少、麻醉后监护室(PACU)停留时间更短、胸腔引流管留置天数更少、术后引流量更少,且住院时间更短。

引言

近十年来,非气管插管胸腔镜手术(NIVATS)麻醉逐渐被临床实践所接受1,2,3。尽管这一新型策略有助于患者快速康复,并避免了全身麻醉(GA)和单肺通气的相关并发症4,但许多外科医生仍认为该方法不如传统的肺隔离技术理想。

血氧水平随年龄增长而下降,部分患者可能出现肺功能降低或处于临界状态。对于此类患者,全身麻醉可能增加并发症风险,包括麻醉苏醒延迟、气道并发症、声音嘶哑、低氧血症以及杓状软骨脱位5,6,7,8,9。相比之下,多项研究记录显示,采用非插管胸腔镜手术(NIVATS)管理的患者住院时间更短,且与全身麻醉相比,低风险患者的呼吸系统并发症减少10;此外,即使在肺功能极差的高风险患者中,也已有成功实施手术的报道11,12,13

手术期间通过精心给予的局部麻醉或联合镇静的区域神经阻滞实现自主呼吸,但在非插管胸腔镜手术(NIVATS)中,咳嗽反射及意外的肺部运动可能带来问题。目前对于纵隔扑动、刺激性咳嗽或呼吸急促等问题,尚无足够重视,也无标准治疗方法,而这些情况可能干扰手术进程。初步观察结果显示,七氟烷可在维持自主呼吸的同时降低NIVATS期间的呼吸频率和纵隔扑动的发生率14。因此可以假设,吸入七氟烷可能预防咳嗽并减少对机械通气的需求,从而降低术后肺部并发症(PPCs)的发生。

首先,本报告提供了一项逐步操作方案,详细阐述了非气管插管视频辅助胸腔镜手术的实施过程。其次,开展了一项回顾性研究,以探讨非气管插管麻醉对术后转归可能带来的益处。

方案

本研究于2018年12月10日获得宁波大学医学院附属医院伦理委员会(KY20181215)批准。

1. 纳入标准

  1. 纳入所有接受肺大疱切除术、肺楔形切除术或肺叶切除术的患者(年龄 >18 岁)。

2. 排除标准

  1. 根据美国麻醉医师协会(American Society of Anesthesiologists, ASA)身体状况分级大于>3级的患者予以排除15
  2. 体重指数(body mass index, BMI)16 大于>30 kg/m2 的患者予以排除。
  3. 患有食管癌、全肺切除术以及需切除肋骨的开胸心脏手术的患者予以排除。
  4. 既往有支气管扩张、毁损肺或慢性阻塞性肺疾病(chronic obstructive pulmonary disease, COPD)病史的患者予以排除。
  5. 术前存在明确的多重肺部感染或炎症,以及其他严重全身性疾病的患者予以排除。
  6. 患有凝血功能障碍以及严重精神或神经系统疾病的患者予以排除。

麻醉前的准备工作

  1. 术前禁食 8 小时。
  2. 将手术室温度调节至 24 °C 至 26 °C 之间。
  3. 将一根 20 G 头翼式输液针插入患者非手术侧手背静脉。
  4. 监测患者的心电图、血压、脉搏血氧饱和度(SpO2)和呼吸频率。
  5. 在每位患者前额放置双频指数(BIS)四通道传感器。
    注意:必要时持续监测桡动脉压和中心静脉压。

4. 超声引导下胸段椎旁阻滞

  1. 将患者置于侧卧位。
  2. 将超声探头直接置于第3胸椎和第7胸椎棘突上方,获取棘突的横断面图像。
    注意:图像中央为高回声的棘突,伴有后方声影;棘突两侧带有后方声影的骨性结构依次为椎板和横突。
  3. 将超声探头向外侧移动,以完整显示横突。
  4. 继续将超声探头向外移动,以观察横突、横突肋关节及肋骨。
  5. 将超声探头向尾侧移动,直至图像中显示横突、胸膜以及两者之间的胸段椎旁间隙。
  6. 注射2 mL 1% 利多卡因进行局部皮肤麻醉。
  7. 在超声引导下,采用平面内技术,从外侧向内侧插入阻滞针。
  8. 注射前仔细进行回抽,确保未见血液回流。
    ​注意:此步骤的目的是防止局部麻醉药引起全身毒性反应。
  9. 先注射2 mL生理盐水,然后通过超声观察胸膜前移及胸段椎旁间隙增宽的情况。
  10. 在T3和T7水平分别注射15 mL 0.375% 罗哌卡因。

5. 麻醉诱导

  1. 向咽喉部喷洒1%左旋布比卡因(3 mL)。
  2. 注射1.5 µg/kg芬太尼和1-1.5 mg/kg异丙酚。
  3. 为持续供氧,可佩戴面罩或插入喉罩气道(双管面罩;体重30-50 kg选用#3号,50-70 kg选用#4号,70-100 kg选用#5号)。氧气由此进入患者的气道 通过 面罩或喉罩
    ​注意:调整注射剂量以使BIS值维持在40-60之间16.

6. 麻醉维持

  1. 维持氧气流速为0.5-1 L/min。
  2. 人工气胸后,将七氟烷浓度维持在1.5%–2.0%。
    注意:若术中自主呼吸频率高于20次/分钟,或出现纵隔扑动或咳嗽,可注射0.5 µg/kg芬太尼。七氟烷进入患者的气道 通过 面罩或喉罩。
  3. 监测吸入氧浓度和呼气末二氧化碳(ETCO2) < 60 mmHg
  4. 监测体温,监测意识水平,并维持BIS值在40-60之间16,17.
  5. 若患者出现窦性心动过缓(心率 ≤ 50 次/分钟),则注射 20µg/kg 的阿托品。
  6. 若收缩压低于基础值的30%或低于90 mmHg,使用输液泵持续输注去甲肾上腺素,剂量为2 µg/kg/h。
  7. 在手术结束前15分钟进行动脉血气分析。
  8. 手术结束前30分钟注射1 mg/kg的氟比洛芬。

7. 胸腔镜迷走神经阻滞技术与胸膜浸润麻醉

  1. 进行右侧操作时,使用24 G带翼输液针在气管下段水平的迷走神经附近注入0.375%罗哌卡因(3 mL)。
  2. 进行左侧操作时,使用24 G带翼输液针在主动脉肺动脉窗水平的迷走神经附近注入0.5%罗哌卡因(3 mL)。
  3. 使用10 mL注射器将10 mL 2%利多卡因喷洒于脏层胸膜表面。

8. 从非插管麻醉转为插管全身麻醉

  1. 如果患者出现以下任一情况,需将非插管麻醉转为插管全身麻醉:
    -严重低氧血症(脉搏血氧饱和度 < 80%)
    -严重高碳酸血症(PaCO2 > 80 mmHg)
    -血流动力学不稳定:难治性心律失常和右心室衰竭
    -持续咳嗽导致手术难以或无法进行
    -术中出血需开胸手术

9. 术后护理

  1. 完全清醒后,嘱患者深呼吸并咳嗽,以使萎陷的肺复张。
  2. 将患者自控静脉镇痛(PCA)泵连接至静脉导管,PCA 储液袋内含 100 mL 溶液(含舒芬太尼 1 µg/kg 及 0.9% 氯化钠注射液),并以 2 mL/h 的速度滴定 PCA 溶液。
  3. 当咳嗽时无气体逸出、胸部 X 光片检查无明显液气胸、且 24 小时引流量 <300 mL 时,拔除胸腔引流管。

结果

回顾性收集了2016年1月至2022年12月期间连续接受非气管插管胸腔镜手术的58例患者的临床资料。术前由麻醉医师对患者进行访视,并在麻醉实施前详细解释麻醉知情同意书的内容。患者可自主选择两种麻醉方式中的一种(非气管插管胸腔镜手术组或全身麻醉组),并签署知情同意书。

GA 组患者接受 0.04 mg/kg 咪达唑仑、2.5 mg/kg 丙泊酚、0.3 mg/kg 依托咪酯、0.5 µg/kg 舒芬太尼和 1.2 mg/kg 罗库溴铵诱导麻醉。待肌松药充分发挥作用后,使用可视喉镜引导经口插入双腔支气管导管或带有支气管阻塞器的单腔气管导管。术前进行胸部X线检查,并在胸锁关节水平测量气管内径。若气管内径≥19 mm,选择 41 Fr 导管;气管内径≥17 mm 时选择 39 Fr;气管内径≥15 mm 时选择 37 Fr;气管内径≥13 mm 时选择 35 Fr;气管内径≥11 mm 时选择 32 Fr。插入双腔导管后,在平卧位和侧卧位下使用纤维支气管镜评估并调整导管位置。术中麻醉维持采用持续静脉输注丙泊酚 2–6 mg/kg/h 和瑞芬太尼 0.25 µg/kg/min,均为静脉麻醉。...

讨论

本方案的优势如下:(i)提供七氟烷吸入麻醉方案,以减少胸腔镜手术期间的咳嗽反射;(ii)在为接受胸部手术的患者提供安全、无痛的手术环境的同时,避免过度镇静;(iii)在考虑相关技术挑战的前提下,尽量减少患者术中自主呼吸及纵隔摆动。上述目标通过预先给予术中局部麻醉实现。

2004年,Pompeo等人首次报道了在无气管插管且保留自主呼吸麻醉的条件下完成胸腔镜肺结节切除术19。近年来,保留自主呼吸的非插管麻醉已成为胸科麻醉领域研究的热点。与传统的气管插管相比,该技术避免了插管可能引起的气道损伤,更好地保护了气管和支气管黏膜上皮的纤毛清除功能;同时减少了术中麻醉药物用量,避免了肌松药的残余效应20,21,并降低了术后低氧血症的风险22。此外,与气管插管相比,喉罩对咽部和喉部的损伤风险较低,侵入性更小,且置入更为简便23,<...
